Robotic virtual endoscopy: development of a multidirectional rigid endoscope.

INTRODUCTION: The use of neuroendoscopy has increased in the past 20 years. Despite an increase in the number of indications for use, novel adjuncts and modifications to existing endoscopes remain all but nonexistent. We introduce a robotic virtual endoscope with applications for neurosurgery that could serve as a novel step in the evolution of future endoscopic technologies. METHODS: Over the past 8 years, we have worked on the construction of a prototype endoscope with three degrees of freedom that was designed to allow for enhanced safety while maximizing the benefits of virtual field rendition and robotic control. We have developed a prototype to examine a cerebral ventricular model in vitro that functions via either a direct video- or computer-based interface. RESULTS: Assessment of viewing angulation with robotic feedback has verified the accuracy of the prototype. Models support the ability of the endoscope to localize regions identified via a software interface. CONCLUSION: The endoscope is a rigid virtual robotic endoscope that provides complete visual coverage of a three-dimensional space by controlling an adjustable viewing direction with three degrees of freedom.

Ellis-van Creveld syndrome and dyserythropoiesis.

Ellis-van Creveld (EVC) syndrome or chondroectodermal dysplasia is a rare autosomal recessive disorder characterized by a variable spectrum of clinical findings. Classical EVC syndrome comprises a tetrad of clinical manifestations of chondrodystrophy, polydactyly, ectodermal dysplasia, and cardiac defects. In several case reports, dysplasia involving other organs has also been identified. Hematologic abnormalities have been rarely reported in patients with EVC syndrome. Here, we report a case of a 3-year-old Hispanic boy with EVC syndrome and marked dyserythropoiesis. The dyserythropoiesis may be part of an isolated myelodysplastic change or a primary myelodysplastic syndrome and likely represents an unusual EVC syndrome association. To our knowledge, this association has not been previously reported.

A proposed clinical decision support system (CDSS) output message.

When a clinical decision support system (CDSS) or other ancillary system detects an abnormal or problematic condition, healthcare providers must be notified in a timely manner so that appropriate corrective action can be taken. Delivery of clinical alerts to the appropriate providers is the role of a notification system.1,2 The content generating system must therefore communicate to the notification system the content to be delivered and any additional directives regarding the resulting notifications. This work is sanctioned by the Health Level Seven, Inc., Clinical Decision Support Technical Committee (CDS TC) and is based on previous proposals to the CDS TC.3 The proposed message model is presented as an XML schema.4
