Analysis of a delayed epidemic model with pulse vaccination and saturation incidence.

Pulse vaccination is an important strategy for the elimination of infectious diseases. An SEIRS epidemic model with time delays and pulse vaccination is formulated in this paper. Using the discrete dynamical system determined by the stroboscopic map, we obtain the exact infection-free periodic solution of the impulsive epidemic system and prove that the infection-free periodic solution is globally attractive if the vaccination rate is larger than theta*. Moreover, we show that the disease is uniformly persistent if the vaccination rate is less than theta*. The permanence of the model is investigated analytically. Our results indicate that a long latent period of the disease is sufficient condition for the extinction of the disease.

Fuzzy logic in medicine and bioinformatics.

The purpose of this paper is to present a general view of the current applications of fuzzy logic in medicine and bioinformatics. We particularly review the medical literature using fuzzy logic. We then recall the geometrical interpretation of fuzzy sets as points in a fuzzy hypercube and present two concrete illustrations in medicine (drug addictions) and in bioinformatics (comparison of genomes).

The fuzzy polynucleotide space: basic properties.

MOTIVATION: Any triplet codon may be regarded as a 12-dimensional fuzzy code. Sufficient information about a particular sequence may not be available in certain situations. The investigator will be confronted with imprecise sequences, yet want to make comparisons of sequences. Fuzzy polynucleotides can be compared by using geometrical interpretation of fuzzy sets as points in a hypercube. RESULTS: We introduce the space of fuzzy polynucleotides and a means of measuring dissimilitudes between them. We establish mathematical principles to measure dissimilarities between fuzzy polynucleotides and present several examples in this metric space. We calculate the frequencies of the nucleotides at the three base sites of a codon in the coding sequences of Escherichia coli K-12 and Mycobacterium tuberculosis H37Rv, and consider them as points in that fuzzy space. We compute the distance between the genomes of E.coli and M.tuberculosis.

Midpoints for fuzzy sets and their application in medicine.

Using Kosko's hypercube, we identify a fuzzy set with a point in a unit hypercube. A non-fuzzy or crisp subset of a set is a vertex of the hypercube. We introduce some new ideas: the definition of the fuzzy segment joining two given fuzzy subsets of a set, the set of midpoints between those two fuzzy subsets, and the set of equidistant points from given points. We present some basic properties and relations between these concepts and provide a complete description of fuzzy segments and midpoints. In the majority of cases, there is no unique midpoint; one has an infinite set of possibilities to choose from. This situation is totally different from classical Euclidean geometry where, for two given points, there is a unique midpoint. We use the obtained results to study two sets of medical data and present two applications in medicine: the fuzzy degree of two concurrent food and drug addictions, and a fuzzy representation of concomitant causal mechanisms of stroke.

Characterizing cellular immune response to kinetoplastid membrane protein-11 (KMP-11) during Leishmania (Viannia) panamensis infection using dendritic cells (DCs) as antigen presenting cells (APCs).

In vitro peptide binding assays and DCs pulsed with recombinant KMP-11 (rKMP-11) plus six 20-mer overlapping peptides covering the entire protein of Leishmania (Viannia) panamensis (L(V)p) promastigotes were used to identify T-cell epitopes in this protein. Such in vitro binding assays, using HLA DRB1* 0101, -0401, -0701 and -1101 alleles, demonstrated that two peptide sequences (DEEFNKKMQEQNAKFFADKP and FKHKFAELLEQQKAAQYPSK) exhibited high HLA DRB1* 0401 allele binding capacity. rKMP-11 specific T-cell proliferation and cytokine production, derived from 13 volunteers exposed to the parasite, suggested that using autologous DCs as APCs becomes advantageous in uncovering T-cell epitopes promoting proliferation and differences in IFN-gamma and IL-4 production in T-cells from volunteers with ACTIVE and CURED undetectable disease when other APCs were used. The two peptides which bound in vitro to the HLA DRB1* 0401 allele were immunogenic in HLA DRB1* 04 volunteers, thus validating the use of in vitro binding assays for predicting epitopes in this protein. The experimental approach used here may prove useful for characterizing T-cell epitopes in a protein useful in designing peptide-based vaccine candidates for Leishmania and other intracellular pathogens.

An exact formula for the number of alignments between two DNA sequences.

Given two DNA sequences, one is usually interested in measuring their similarity. For this purpose we have to consider different alignments of both the sequences. The number of alignments grows rapidly with the length of the sequences. In this short communication, we give an exact formula for the number of possible alignments using the theory of difference equations.

Biomathematical modeling and analysis of blood flow in an intracranial aneurysm.

The basic hypothesis of this study is that the intracranial aneurysm may enlarge and rupture due to dynamic instabilities of the blood flow and pressure inside the aneurysm. The specific question we attempted to answer is: which parameter(s) of aneurysmal geometry can serve as a reliable predictor(s) for aneurysmal rupture? We consider an idealized cylindrical aneurysm of the human common carotid artery and develop a mathematical model of blood flow through a normal artery and aneurysm connected in series. The mathematical model is nonlinear. It comprises nonlinear rheological properties of the normal artery and aneurysmal materials, and the inertial and resistance properties of the blood flow. The model equations were solved numerically and analyzed by methods of nonlinear dynamics. The critical aneurysmal diameter (CAD) is defined as a boundary point between the stable and unstable states of the model equations. The results confirm that a limit point of flow stability can occur only for a certain difference between aneurysmal and artery radii which are pre-disposed from a difference in their material properties. It was shown that CAD is dependent on both aneurysmal length and age of patient. Finally, the results suggest that the ratio between aneurysmal and normal artery diameters is a more reliable predictor of the aneurysmal rupture than the diameter alone. We conclude that an aneurysm diameter twice that of the normal artery could be dangerous.

1H-NMR structures of the Plasmodium falciparum 1758 erythrocyte binding peptide analogues and protection against malaria.

A conserved high activity erythrocyte binding peptide (HAEBP) derived from the 175-erythrocyte binding antigen (EBA-175), coded 1758, was synthesized and analyzed for antigenic and protective activities in Aotus monkeys, together with several of its analogues. Conformational analysis by 1H Nuclear Magnetic Resonance in TFE-solution was done for some of them, as well as the 1758 parent peptide. We show that the conserved 1758 HAEBP (being neither immunogenic nor protective) has an alpha helical structure, whilst its analogues contain beta-turn structures. The 13790 peptide (highly immunogenic and protective for some monkeys) shows a type I beta-turn structure distorted in psi(i + 1) psi(i + 2) angles, whilst immunogenic and non-protective (as well as the non-immunogenic and non-protective peptides) have type III' beta-turns. An understanding of native peptide's correlation with altered peptide three-dimensional structure and resulting immunogenicity and protective activity may lead to a more rational design of multi-antigenic, multi-stage P. falciparum subunit based malaria vaccines.

Identification and polymorphism of Plasmodium vivax RBP-1 peptides which bind specifically to reticulocytes.

Plasmodium vivax merozoite preferentially invades reticulocytes probably using PvRBP-1 as ligand. One hundred and ninety-five, 15-mer peptides has been synthesised from PvRBP-1 sequence; tested in reticulocyte- or erythrocyte-binding assays. Twenty-five peptides (K(d)=76-380 nM) specifically defined four reticulocyte-binding regions. It has been reported that a highly conserved Region-I recombinant fragment binds specifically to reticulocytes. HABP-critical residues for reticulocyte-binding were highly conserved in 20 Colombian P. vivax clinical isolates, suggesting an important biological function. There were six overlapping reticulocyte-binding sites for these peptides according to enzyme sensitivity and mutual competition-binding assays; located on 26- and 41-kDa reticulocyte membrane surface proteins.

[The assessment of the nutritional status and risk profile of elderly residents].

UNLABELLED: An observational study in a long term geriatric care institution and medical wards, to systematically introduce the assessment of nutritional status, cognitive function and risk profile of residents, allowed the identification of "at risk" patients. METHODS: Data were collected on 198 residents, over a period of 6 months. RESULTS: The mean age was 86.6 years (range 80-99). Some data were not available for 54 patients (i.e. story of weight loss 3 months before admission: all of them had >90 years); 49% of patients were higly dependent and 30% of patients were severely malnourished (malnutrition was prevalent in cancer patients). Of the 19 patients dead during the study 6 were malnuorished, 6 at risk and for 6 a thorough assessment was not possible. DISCUSSION: The prognostic importance weight loss over the last 6 months is widely aknowledged, however this information is not easily retrievable in geriatric populations; demented patients are more at risk of malnutrition. The collection of data on nutritional status allows the identification of patients who need a close surveillance and the implementation of preventive measures.
