Unravelling the transcriptomic characteristics of bronchoalveolar lavage in post-covid pulmonary fibrosis.

BACKGROUND: Post-Covid Pulmonary Fibrosis (PCPF) has emerged as a significant global issue associated with a poor quality of life and significant morbidity. Currently, our understanding of the molecular pathways of PCPF is limited. Hence, in this study, we performed whole transcriptome sequencing of the RNA isolated from the bronchoalveolar lavage (BAL) samples of PCPF and compared it with idiopathic pulmonary fibrosis (IPF) and non-ILD (Interstitial Lung Disease) control to understand the gene expression profile and associated pathways. METHODS: BAL samples from PCPF (n = 3), IPF (n = 3), and non-ILD Control (n = 3) (individuals with apparent healthy lung without interstitial lung disease) groups were obtained and RNA were isolated for whole transcriptomic sequencing. Differentially Expressed Genes (DEGs) were determined followed by functional enrichment analysis and qPCR validation. RESULTS: A panel of differentially expressed genes were identified in bronchoalveolar lavage fluid cells (BALF) of PCPF as compare to control and IPF. Our analysis revealed dysregulated pathways associated with cell cycle regulation, immune responses, and neuroinflammatory processes. Real-time validation further supported these findings. The PPI network and module analysis shed light on potential biomarkers and underscore the complex interplay of molecular mechanisms in PCPF. The comparison of PCPF and IPF identified a significant downregulation of pathways that were more prominent in IPF. CONCLUSION: This investigation provides crucial insights into the molecular mechanism of PCPF and also outlines avenues for prospective research and the development of therapeutic approaches.

Neuropsychiatric symptoms and medical illness in patients with dementia: an exploratory study.

OBJECTIVE: To determine the association between neuropsychiatric symptoms and the presence of medical illness among outpatients with mild dementia. METHOD: The Neuropsychiatric Inventory (NPI) was used to assess neuropsychiatric symptoms, and the Cumulative Illness Rating Scale (CIRS) was used to evaluate physical impairment, in 44 outpatients diagnosed as having dementia (Alzheimer disease, n=22; vascular dementia, n=13; mixed dementia, n=9). The tests used were standard parts of a memory assessment program at a college of osteopathic medicine. Pearson product moment correlations were used to assess any associations between NPI and CIRS scores. RESULTS: Significant associations were identified between several NPI-assessed symptoms and degree of medical illness as measured by the CIRS. Neurobehavioral problems were significantly correlated (P<.05) with illness in the following body organ systems: gastrointestinal (lower), genitourinary, neurologic, ophthalmologic/otolaryngologic, psychiatric, and respiratory. CONCLUSION: The authors' preliminary data underscore the importance of primary care physicians assessing patients with dementia for comorbidity of psychiatric illnesses when conducting medical examinations.

Diffuse multifocal chorangiomatosis of the placenta with multiple intestinal stenosis of the fetus: combination of rare causes for nonimmune hydrops fetalis.

Non-immune causes of hydrops fetalis are rare but frequently fatal. Identification of the cause for hydrops fetalis is essential to institute therapy. Chorangiomatosis and intestinal stenosis have not been previously reported as aetiological factors for the development of hydrops. We report a fetus born with hydrops associated with both of these conditions. A 1575 gms preterm neonate was born to a multigravida at 31 weeks of gestation. Emergency caesarean section was performed after detection of hydrops by prenatal ultrasonography. Baby had generalized edema at birth and died after 3 days due to progressive heart failure. At autopsy there was generalized edema and effusions. There were multiple stenotic segments in the intestine. Placenta showed numerous chorangiomas varying from 0.2 to 3 cm in diameter, hence diagnosed as diffuse multifocal chorangiomatosis. Examination of the placenta at neonatal autopsy is an essential part of assessment for hydrops fetalis. Chorangiomatosis is a rare and under reported condition which can cause prematurity, intrauterine growth retardation, pre-eclampsia and rarely hydrops fetalis.

Lingual tactile acuity, taste perception, and the density and diameter of fungiform papillae in female subjects.

A growing body of evidence suggests that individuals who differ in taste perception differ in lingual tactile perception. To address this issue, spatial resolution acuity was estimated for 83 young adult females (52 Asians and 31 Caucasians) by their ability to examine with the tongue and identify embossed letters of the alphabet. Ratings of the magnitude of the bitterness of 0.0032M 6-n-propylthiouracil (PROP) were obtained to characterize subjects' taste perception. The density and diameter of fungiform papillae on the anterior tongues of the Asian subjects were measured also. Subjects who rated the bitterness of PROP as very or intensely strong (supertasters) were found to be about 25% more tactually acute than subjects who rated the bitterness as moderate to strong (medium tasters) and twice as acute as subjects who rated it as nondetectable or weak (non-tasters; P<.0001): The threshold heights for letter recognition averaged 2.8, 3.5 and 5.4 mm, respectively, for the Asian subjects and 2.6, 3.2, and 5.1 mm for the Caucasian subjects. The thresholds correlated highly with subjects' ratings of bitterness (rho=-0.84, P<.0001), and for the Asian subjects with the density (rho=-0.84, P<.0001) and diameter (rho=0.66, P<.0001) of fungiform papillae. Mean densities varied from 54.4 cm(-2) (non-tasters) to 106.5 cm(-2) (medium tasters) to 143.7 cm(-2) (supertasters; P<.0001). These findings confirm that individuals who differ in taste (PROP) sensitivity also differ in lingual tactile acuity. Tactile and taste sensitivities covary and reflect individual differences in the density and diameter of fungiform papillae on the anterior tongue.
