[A case of renal clear cell carcinoma metastasis to the oropharynx].

A rare case of 61-yrs-old male was reported, in whom distant metastasis from renal clear-cell carcinoma to the oropharynx had been diagnosed. It was presented as pedunculated tumor at the level of palatine tonsil, 11 months after nephrectomy, and treated by surgical excision. Other metastatic sites--pulmonary and skeletal had also developed and a patient died 16 months after the time of primary diagnosis.

[Evaluation of intensity of angiogenesis in granulation tissue in chronic otitis media--preliminary report].

In chronic otitis media molecular pathogenetic mechanisms are still unknown, however, angiogenesis may play a role. The aim of the study was to determine the intensity of angiogenesis in granulation tissue in chronic otitis media of different clinical course. Twenty six granulation tissue specimens (twenty two--from chronic otitis media, four--from prolongated otitis externa) taken during surgery were analyzed. The angiogenesis (microvessel density) was measured in paraffin-embedded tissue by an immunohistochemical method, by staining for endothelial cells with a monoclonal antibody against CD 34. The presence of CD 34 was found in all specimens. The expression was more intense in tissue samples from the group with good clinical course (good healing and without recurrence) than those in the group with poor healing and recurrence (mean number of dots for mm2 589,2 vs 533,3, respectively) and from the group without bacterial infection by Pseudomonas aeruginosa than those with this infection (mean number of dots for mm2 645,5 vs 440,8, respectively), but differences were not significant. In conclusion it is suggested that angiogenesis may contribute to different clinical course of chronic otitis media.

[Oncological failure after partial laryngectomy for laryngeal carcinoma].

The aim of the study was an evaluation of oncological failures after partial laryngectomies for treatment of laryngeal carcinoma. A retrospective analysis was done on 325 partial laryngectomies performed during 18 years (1980-1997) at Otolaryngology Department Medical University of Lódź. Seventy four failures (22.8%) were observed, among them local recurrences--in 43 patients (13.2%), nodal recurrences--in 9 patients (2.8%), distant metastases--in 6 patients (1.8%), second primary malignant neoplasms--in 11 patients (3.4%) and spread of neoplastic disease--in 8 patients (2.5%). Local recurrence was statistically the most often type of failure--58.1% in comparison with frequency of the others (p<0.001). The failures, according to type of partial laryngectomy, occurred rarely after cordectomy--17.5%, the most frequently were noted after supraglottic horizontal laryngectomy--42.3% (p<0.01) and fronto-lateral/fronto-anterior laryngectomy--30.9% (p<0.05). Majority of failures were diagnosed within first 2 years after primary treatment--55.4% of all failures in comparison with third (12.2%), fourth and fifth years (5.4%), but they were also often observed over 5 years (27%) (p<0.05). A statistical difference in percentage of failures in the periods until 5 years and over 5 years was noted only for cordectomy (23.8% and 4.9% respectively, p<0.001) and supraglottic laryngectomy (56.3% and 12.5% respectively, p<0.05). Systematic control of patients after partial laryngectomies is necessary all over their life.

[Malignant neoplasms of external and middle ear].

INTRODUCTION: Neoplasms of external and middle ear are rare, which cause several problems in diagnosis and therapy. The purpose of the study was to analyze retrospectively patients with malignant neoplasms of the ear. METHODOLOGY: The study was carried out on 53 patients treated for malignant ear neoplasms in single institution during 25 years (1978-2002). RESULTS: The most frequent neoplasm was squamous cell carcinoma--36 cases (67.9%), then basal cell carcinoma--9 cases (16.9%). Neoplasm primarily involved auricle in 26 patients (49.1%), external auditory canal in 15 patients (28.3%) and middle ear in 12 patients (22.6%). They were classified according to Stell and McCormick (1985) as follows: stage I--29 tumours (54.7%), stage T2--15 tumours (28.3%), stage T3--9 tumours (17%). Facial nerve paresis occurred in 15 patients (28.3%). The most often treatment modality was surgery--32 persons (60.4%), then surgery followed by radiotherapy--17 persons (32.1%). The characteristics of neoplasms related to the site of location were described. The difficulties in precise histopathologic diagnosis and extent of disease were pointed out. RESULTS: Neoplasms of external and middle ear constitute a group of various histopathological and clinical tumours, which differ in diagnostic difficulties, treatment and prognosis. A diagnosis was often made in advanced stages of neoplasms, especially for middle ear tumours, that diminished a possibility of effective treatment.

[Impact of tinnitus on quality of life in the patients' self-assessment].

Tinnitus may have multidirectional impact on patients and influence unfavourably their quality of life and psychology. The aim of the study was to investigate the aetiology, audiological parameters, and tinnitus characteristics and to evaluate the impact of tinnitus on patients life and emotional status by using the self-report questionnaire. A group of 157 consecutive patients referred to the Otiatry Clinic Medical University in Lodz were enrolled in the study. It was found that tinnitus negatively impact on 2/3 of patients and 1/3 of them consider it as annoying. The most frequent aetiological factor was noise exposure (40.8%). There was no correlation between tinnitus loudness and degree of hearing loss on the one hand and self-perceived handicap and annoyance of tinnitus on the other. It was stated that younger age and higher tinnitus pitch-match frequencies may influence patients' quality of life.

[Electronystagmographic evaluation of vestibular system in tinnitus patients with degenerative cervical spine lesions].

Vertebrobasilar insufficiency is described as one of the reasons for tinnitus, especially of patients with degenerative cervical spine lesions showed in radiographic imaging. In this study we present the electronystagmographic results of 76 tinnitus patients in two groups: 56 persons with and 18 persons without degenerative cervical findings. All patients underwent clinical otoneurological, audiological and additionally Doppler sonography of vertebral artery examination. We found incorrect cervical radiography in 76.3% tinnitus patients and pathological vertebral artery blood flow in 20.7%, most frequently in patients with cervical degenerative lesions. Electronystagmographic outcome of vestibular system in tinnitus patients in both groups showed abnormal recordings in positional nystagmus in 51.7% and 27.8% persons, saccadic test in 43.1% and 44.4% persons and in smooth pursuit test in 41.3% and 33.3% persons respectively. There were no significant differences between groups in ENG results. It can suggest central vestibular impairment in these patients. ENG examination, seem to be a useful method in vestibular system evaluation in tinnitus patients. Radiographic imaging of cervical spine is less valuable in these patients.

[Outcome of the treatment for laryngeal carcinoma by partial laryngectomies].

The aim of the study was to retrospectively evaluate the results of treatment for early laryngeal carcinoma in a group of 325 patients who underwent from 1980 to 1997 partial laryngectomies. The most often type of operation was cordectomy--51.1%, then frontolateral/frontoanterior laryngectomy--20.9%, vertical laryngectomy--15.1%, supraglottic horizontal laryngectomy--8%, hemilaryngectomy--4.9%. Estimate of survival time, from surgery to first oncologic failure, was obtained with the life-table method. As a failure we considered local and regional recurrence, distant metastasis, second primary neoplasm and spread of disease. The probability of disease-free survival for all partial laryngectomies reached 81.5%, 79.5% and 60.3% at 3, 5 and over 5 years, respectively. The rates of five-year and over 5 year disease-free survival for each type of operation were: for cordectomy--82.3% and 72.3%, for frontolateral/frontoanterior laryngectomy--74.0% and 45.2%, for vertical laryngectomy--87.6% and 60.6%, for hemilaryngectomy--86.2% and 59.7%, for supraglottic horizontal laryngectomy--53.8% and 29.9%, respectively. The comparison between survival rates, by the log-rank test, revealed statistical difference between supraglottic horizontal laryngectomy and following operations: cordectomy (p < 0.001), vertical laryngectomy (p < 0.01), hemilaryngectomy (p < 0.05). The differences between other types of laryngectomy were not significant statistically. The best results in treatment of laryngeal carcinoma by partial laryngectomies were achieved by operations in vertical plane (cordectomy, vertical laryngectomy and hemilaryngectomy), the less positive--by operations in horizontal plane (supraglottic horizontal laryngectomy).

[Impact of tinnitus on emotional state of patients according to General Health Questionnaire by Goldberg].

The aim of the study was to estimate the emotional state and the prediction of the emotional disorder of the patients who suffer from tinnitus. The General Health Questionnaire by Goldberg (GHQ) was used to this study. The study was performed in 129 patients aged 20-78 years who had been suffering from tinnitus for more than 3 months. Based on the results of the examination the patients were divided into two groups. The first group consisted of 58 patients without any emotional disorders. The second group consisted of 71 patients who would be supposed to suffer from the emotional disorders in the future. There were no statistical significant differences in the gender, age and the result of the audiometric tests between the groups of the patients. An analysis showed that the results of the GHQ were the statistical significant higher in the second group especially in the part where the symptoms of a depression were estimated.

[Glandular neoplasms of the external auditory canal--clinical and morphologic observations].

In the article cases of glandular neoplasms of the external auditory canal were presented. Ceruminous adenoma was diagnosed in three persons, adenoid cystic carcinoma--in three patients and ceruminous adenocarcinoma in one patient. All of them were treated surgically or by combined method with radiotherapy. Difficulties with exact histopathological diagnosis and with choice of a method of treatment were stressed. Classification of the glandular neoplasms of the external auditory canal was also presented underlying a histopathological and clinical diversity of these tumors, which had previously been incorrectly qualified in one group ceruminoma.

[Significance of support groups for patients after total laryngectomy for laryngeal carcinoma].

The aim of this study was to determine the relationship between the quality of life (QL) assessment and attachment to support group in patients after total laryngectomy. Material consisted of 60 patients with squamous cell carcinoma of the larynx, inclusive of 30 patients belonging to the supported group. The questionnaire of QL EORTC QLQ-C30, the EORTC head and neck cancer module QLQ-H&N35 and Hospital Anxiety and Depression Scale HAD were used. We found out that the somatic, performance and social dimension of QL of patients belonging to the supported group are higher than others patients.

[Prognostic factors in sudden deafness].

Sudden sensorineural hearing loss is still a diagnostic and therapeutic dilemma and is very difficult to predict recovery in it. Different factors may influence a prognosis like severity of hearing loss, duration of symptoms before treatment, presence of vertigo, type of audiogram and age of patients. The aim of the study was an evaluation of the hearing improvement in sudden deafness in relation to some of these elements. A retrospective analysis of 119 patients treated for sudden sensorineural hearing loss during 10 years was done. Clinical otolaryngological, neurological and ophthalmological examination, audiologic and ENG tests were carried out. Hearing improvement was obtained in 51 patients (43%). Hearing recovery was recorded in 38 patients (66%) in whom duration of disease before treatment was no longer than 7 days, in 9 patients (25%) with a period 8-14 days and in 4 patients (16%)--with period 15-30 days (66% vs 25% vs 16%, p < 0.001, 25% vs 16% p = 0.39). Hearing improvement was more frequent in patients with initially mild (51.6%) than severe (38.7%) and profound hearing loss (25%) (51.5% vs 25%, p < 0.05). Hearing recovery was observed in 18 patients (33.3%) with vertigo and in 33 patients (50.8%) without vertigo (p = 0.056). In analysis of age (five groups: until 30 years, 31-40 years, 41-50 years, 51-60 years, over 60 years) in comparison with hearing recovery it was not found any significance (45.5% vs 45.8% vs 46.4% vs 37% vs 38.9%, p = 0.94). It was stated that in patients with sudden deafness duration of the disease before treatment and level of hearing loss may significantly influence an outcome, also vertigo may worsen a recovery, contrary age of the patients does not seem as important prognostic factor.

[Angiosarcoma of the temporal bone].

A case of 55-yrs old female was reported, in whom angiosarcoma of temporal bone had been diagnosed. A mode of treatment was surgery including partial petrosectomy and labyrinthectomy. Radiotherapy was not given because of a possibility of further cerebrospinal liquid leakage after dura mater rupture, which was seen during first operation. Local recurrence had developed about 2 months after surgery and a patient died 10 months after the time of diagnosis. It was stressed that angiosarcoma is a very rare neoplasm and in our case it was presented as few--symptomatic lesion despite of it's high extent in temporal bone. A review of literature concerning angiosarcoma was also reported.

[Tonsillar carcinoma--clinical assessment and analysis of treatment results].

The aim of the study was an evaluation of the clinical signs and treatment results of the patients with tonsillar cancer treated in the ENT Clinic Medical University in Łódź during 10 years (1985-1994). In this study we analysed retrospectively the data of 40 subjects (52% of all group 76 patients). Twenty four patients (55%) had tumor with T3-T4 stage and 21 patients (52.5%)--palpable lymph neck nodes. The most frequent treatment modality was combined therapy (surgery with radio/chemotherapy) introduced in 25 persons (62.5%), surgery alone was performed in 10 cases (25%). Distant metastases developed in 6 patients (15%) and the second primary neoplasm in 5 patients (12.5%). We obtained 30% five years survival rate. We stress the importance of careful clinical assessment before planning the treatment.

[Vestibular system in patients with sensorineural hearing loss].

A near localization of anatomical structures of an inner ear and possibility of damage caused by the same etiopathogenic factor implies that sensorineural hearing loss may be accompanied by vertigo and dizziness. The aim of a study was to evaluate a frequency of vestibular system dysfunction in patients with sensorineural hearing loss and to analyse it according to severity and type of the hearing loss. We evaluated a group of 126 persons which were treated in ENT Department during the last year from sensorineural hearing loss. In all subjects' audiological and ENG examinations were performed. A 50% of patients complained of vertigo and 30% of them--of dizziness. Abnormal ENG recordings were registered in 72% of persons especially in positional and visual ocular-motor tests. In the group without vertigo incorrect ENG tests were often recorded. In the group with vertigo the peripheral and mixed lesions in ENG examination were noted statistically more frequent. We found lack of correlation between magnitude and type of hearing loss and vestibular impairment. Only in the group-residual hearing/deafness were observed significantly more frequent peripheral vestibular dysfunction.

Evaluation of oculomotor tests in patients with tinnitus.

Tinnitus as a symptom remains a serious multidisciplinary problem. Vertigo or dizziness is not noticed frequently in tinnitus patients, so vestibular function is not often studied. Because they are in close proximity to one another, the vestibular and hearing organs may influence each other. We decided to evaluate the results of the oculomotor reflex in tinnitus patients. We carried out clinical examinations and audiological and oculomotor tests in 50 tinnitus patients and 30 healthy persons. Such oculomotor tests as smooth-pursuit eye movement, optokinetic tests, and saccadic eye movement and gaze fixation were performed. Tinnitus was unilateral in 37 tinnitus patients (74%) and bilateral in 13 (26%). Twenty-two tinnitus patients (44%) reported previous remote episodes of vertigo and dizziness. Gaze-evoked nystagmus was absent in all cases. Morphological abnormalities and gain decreases in smooth-pursuit tests were recorded in 19 tinnitus patients (38%). Findings in the optokinetic test were incorrect in 10 (20%). The saccadic eye movement test showed disturbances in 18 tinnitus patients (36%). In 11 (22%), abnormal recordings were found in two tests and, in 10 (20%), abnormal recordings were found in all three tests. Our study suggests that computerized, quantitative electronystagmographic analysis in tinnitus patients should be interpreted carefully. A persistence of abnormal oculomotor recordings can suggest subclinical central vestibular system impairment.

[Smooth pursuit and saccadic tests outcome in healthy persons in different age].

UNLABELLED: The study of eye movement presents a unique opportunity to investigate the integrity of central ocular-motor pathways and structures, involved in the generation and control of ocularmotor reflexes, which is important in the diagnosis of rate balance system in otoneurology. In psychiatry, oculography is useful for the evaluation of schizophrenia, obsessive-compulsive and affective disorders as well as the effectiveness of psychoactive drugs. AIM: The aim of the study was to investigate the results of smooth pursuit and saccadic tests in healthy persons in respect to age and stimulus paradigm. METHOD: Sixty seven subjects in different age: 16--young (mean 29.8 years), 38--middle-aged (mean 53.2 years) and 13--elderly (mean 73.6 years) were enrolled in the study. Oculographic tests were performed on the computerized ENG system. The pursuit target was driven by a predictable sinusoidal task at various frequencies 30 Hz, 40 Hz, 50 Hz and amplitudes of 15 degrees, 20 degrees and 30 degrees. In the saccades test the target moved abruptly 15 degrees left and right of the centre. In all tests, several parameters and the 95% prediction interval (95 PI) were calculated. RESULTS: In the smooth pursuit test gain decreased with ageing. The differences between age groups (younger-middle-age and younger-elderly) were statistically significant for each target velocity. In this study we found no significant evidence that age has an impact on the parameters of saccadic tests. CONCLUSION: These findings suggest that the results of oculomotor tests should be quantified by the age of the patient and the target task.
