Heart rate and arrhythmia in patients with psoriasis vulgaris.

BACKGROUND: Psoriasis is a chronic inflammatory disease involving 1-3% of the human population worldwide. Many systemic diseases including cardiovascular disturbances have been described in psoriatic patients. However, there is a scarcity of data on heart rate, heart rate variability, arrhythmia and conduction abnormalities in this group of patients. METHODS: The study comprised 32 patients with chronic psoriasis vulgaris and negative personal history of heart problems. Severity of the disease was evaluated by Psoriasis Area and Severity Index (PASI). Twenty-four-h continuous electrocardiographic monitoring (24-h Holter ECG) was performed in all patients. RESULTS: Heart rate was significantly higher both during the day and at night in patients with psoriasis vulgaris than in the control group (p < 0.0001). There was a positive correlation between the increased heat rate, both during the day and at night, in psoriatic patients and severity of the disease expressed as PASI. Single supraventricular beats were significantly more frequently observed in psoriatic patients vs. the control group (p < 0.0001). CONCLUSIONS: An active inflammatory process observed in psoriasis seems to exert its influence on increased heart rate and supraventricular beats development. However, to confirm the above findings, further studies on larger groups of psoriatic patients, presenting different types of the disease are mandatory.

Interleukin 6 and 8 levels in plasma and fibroblast cultures in psoriasis.

Fibroblasts have been implicated in psoriatic inflammatory processes. The aim of the study was to evaluate soluble interleukin 2 receptor (sIL-2R), interleukin 6 (IL-6), and interleukin 8 (IL-8) plasma levels in psoriatic patients and IL-6 and IL-8 levels in fibroblast culture supernatants. Cytokines levels in plasma and supernatants were measured by ELISA. Plasma sIL-2R, IL-6, and IL-8 levels were higher before the treatment in comparison to healthy controls (P < 0.001) and decreased after treatment. Fibroblasts from healthy controls, psoriatic lesional skin, and noninvolved psoriatic skin, when stimulated with tumor necrosis factor alpha, released considerable amounts of IL-6 and IL-8. No significant difference between healthy controls and psoriatic fibroblasts was observed. Monitoring plasma sIL-2R levels could be employed as a reliable method of psoriasis activity. IL-8 and IL-6 plasma levels seem to reflect psoriasis activity, and treatment response, respectively. Fibroblasts are not a major source of increased IL-6 and IL-8 production in psoriasis.

Optimization of an enzymatic method for the determination of lysosomal N-acetyl-beta-D-hexosaminidase and beta-glucuronidase in synovial fluid.

BACKGROUND: Our goal was to develop a suitably sensitive assay for N-acetyl-beta-D-hexosaminidase (HEX) and beta-glucuronidase to allow their use as markers of joint diseases. METHODS: We optimized a spectrophotometric method for the determination of lysosomally derived HEX and beta-glucuronidase in synovial fluid on a microplate reader to improve its utility. HEX and beta-glucuronidase act on the 4-nitrophenyl derivatives N-acetyl-beta-glucosamine and beta-D-glucuronide, respectively, to produce 4-nitrophenol, which can be measured at 405 nm on a microplate reader. RESULTS: Maximum enzyme activity was observed at pH 4.7 in a citrate-phosphate buffer for HEX and at pH 4.5 in an acetate buffer for beta-glucuronidase. A 10-microL sample with 30 microL of substrate solution and 40 microL of appropriate buffer produced measurable amounts of 4-nitrophenol after incubation for 60 min at 37 degrees Celsius. Reactions were terminated by the addition of 200 microL of 200 mM borate buffer (pH 9.8). CONCLUSIONS: The assay is sufficiently sensitive for small volumes of synovial fluid, and is useful for the clinical diagnosis of joint diseases.

The number and distribution of blood dendritic cells in the epidermis and dermis of healthy human subjects.

Human blood dendritic cells (BDC) can be divided into three subsets: plasmacytoid DC (PDC) and two myeloid subsets--MDC1 and MDC2. Several studies revealed the presence of both MDC and PDC in blood of healthy subjects, however no precise literature data exist on the number and distribution of BDC in the skin. The aim of our study was to assess the number and distribution of BDC and their subtypes in the healthy skin. The-study included 30 healthy volunteers (age 18-51). Punch biopsies were taken from the buttock skin from each subject, and immunofluorescent staining was performed using monoclonal mouse IgG1 antibodies directed against BDCA-1, BDCA-2, BDCA-3 and BDC-4. The BDC were present both in the epidermis and dermis. PDC were detected mainly in the dermis (mean 1.2 cells per field). Myeloid subtypes were observed mainly in the middle layers of the epidermis and in the upper part of the dermis (mean 1.8 cells per field). The detection of blood dendritic cells in the skin proves their role in immune cutaneous surveillance.

[Quality of life predictors among patients suffering from systemic lupus erythematosus].

Researches on quality of life have been undertaken from dozens of years. However, there are few information about quality of life among people suffering from systemic lupus erythematosus (SLE). The aim of this study was to set predictors making possible quality of life anticipation. The participants of the studied group were 42 patients with inactive illness stage (disease severity were estimated by SLAM scale). Quality of life was measured by own questionnaire. Personal resources, which are the base of anticipation, were estimated by standardized tests (AIS, LOT, GSES). The results indicated that two of examined variables became the quality of predictors: helpless/hopeless strategy and illness acceptation.

Influence of systemic photochemotherapy on regulatory T cells and selected cytokine production in psoriatic patients: a pilot study.

BACKGROUND: Psoriasis is a chronic autoimmune inflammatory disease of the skin with strong genetic and environmental risk factors and is regarded as a Th1 cell-type disease. The aim of our study was to evaluate the effect of one-month PUVA (Psoralen Ultraviolet A) therapy on a regulatory T-cell subpopulation (CD4+CD25+) and the production of some cytokines. MATERIAL/METHODS: The study was performed on the group of 12 patients with severe psoriasis. They were put on PUVA therapy for one month. We analyzed the level of CD4+CD25+ regulatory T cells using a FACSCalibur cytometer and CellQuest Software. The production of IFN-gamma (interferon-gamma), TNF-alpha (tumor necrosis factor alpha), IL (interleukin) -10, IL-5, IL-4, and IL-2 by lymphocytes was estimated by using a CBA system. The control group consisted of 11 healthy volunteers. RESULTS: We found that the production of INF-gamma, TNF-alpha, IL-2, and IL-10 in psoriatic patients before PUVA application increased significantly compared with the control group. In patients after PUVA therapy we observed decreased production of TNF-alpha and a decreased number of CD4+CD25+ cells in the blood compared with the same group of patients before the treatment. CONCLUSIONS: It was demonstrated that systemic PUVA therapy led to a marked reduction in CD4+CD25+ T cells and a change in cytokine production.

Correlation of endostatin and tissue inhibitor of metalloproteinases 2 (TIMP2) serum levels with cardiovascular involvement in systemic sclerosis patients.

Fibrosis of oesophagus, lungs, heart, and kidney in the course of systemic sclerosis (SSc) may lead to dysfunction of the above organs or even patients death. Recent studies point out the role of angiogenesis and fibrosis disturbances in the pathogenesis of SSc. Heart fibrosis is one of the most important prognostic factors in SSc patients. So, the aim of our study was to examine cardiovascular dysfunction in SSc patients and its correlation with serum levels of vascular endothelial growth factor (VEGF), endostatin, and tissue inhibitor of metalloproteinase 2 (TIMP2). The study group comprised 34 patients (19 with limited scleroderma (lSSc) and 15 with diffuse scleroderma (dSSc)). The control group consisted of 20 healthy persons, age and sex matched. Internal organ involvement was assessed on the basis of specialist procedures. Serum VEGF, endostatin, and TIMP2 levels were evaluated by ELISA. We found cardiovascular changes in 15 patients with SSc (8 with lSSc and 7 with dSSc). The observed symptoms were of different characters and also coexisted with each other. Higher endostatin serum levels in all systemic sclerosis patients in comparison to the control group were demonstrated (P < .05). Also higher serum levels of endostatin and TIMP2 were observed in patients with cardiovascular changes in comparison to the patients without such changes (P < .05). The obtained results support the notion that angiogenesis and fibrosis disturbances may play an important role in SSc. Evaluation of endostatin and TIMP2 serum levels seems to be one of the noninvasive, helpful examinations of heart involvement in the course of systemic sclerosis.

Lack of effect of repeated suberythemal ultraviolet-B exposures on human blood dendritic subtypes.

BACKGROUND/PURPOSE: Dendritic cells (DC) play a major role in the afferent immune response. They are found as a minor cell population in the blood as three main subtypes that can be distinguished phenotypically: plasmacytoid DC (PDC), and myeloid DC1 and 2 (MDC1 and 2). The aim of the study was to examine the effect of repeated whole-body suberythemal ultraviolet (UV) B irradiation on the percentages of DC subsets in the blood and skin, and to test for photoadaptation by the subsequent administration of a local erythemal UV dose. METHODS: Thirty subjects in each group were irradiated with either 0.7 personal minimal erythema dose (MED) UVB daily for 10 days (whole body), or for the 10 days followed by a single three MED UVB exposure of a local body site, or with the single three MED UVB exposure of a local body site only. Blood was collected before and after the exposures and the percentage of DC and DC subtypes assessed by flow cytometry. Skin biopsies were collected at the same times, and the number and position of the DC subsets examined by immunofluorescent microscopy. RESULTS: The whole-body repeated UVB irradiations did not result in a change in the blood DC (BDC) or the subsets percentages in the blood, except that there was a small but significant rise in the percentage of the MDC2 subset. No alteration occurred following the local erythemal UVB exposure. The total number of BDC in the skin was small, with the PDC being located mainly in the dermis and the myeloid subtypes mainly in the epidermis. No change in cutaneous numbers or distribution was revealed following the irradiation protocols. CONCLUSIONS: Repeated whole-body suberythemal UVB irradiation does not cause a change in BDC or BDC subsets in the blood or skin, except for a small increase in the percentage of MDC2 in the blood. Local erythemal UVB irradiation does not alter the BDC subsets in blood or skin.

[Activity of N-acetylo-beta-hexosamninidase in joint fluid from knee and serum of patients with rheumatoid arthritis and osteoarthritis].

N-acetyl-beta-hexosoaminidase (Hex) is a lysosomal enzyme which releases N-acetylaminohexose from non reducing end of glycosaminoglycans, glycoproteins and glycolipids, taking part in degradation of these substances. It is not known what role Hex plays in the degradation of joints in rheumatoid arthritis and osteoarthritis. The aim of our work was evaluation of Hex activity in synovial fluid and serum of patients with rheumatoid arthritis and osteoarthritis. The investigation was performed on material taken from 28 patients with rheumatoid arthritis aged 22-74 years with III or IV stage of disease and 26 patients with osteo-arthritis aged 41-75 years. The synovial fluid was collected from the knee joint during puncture. Hex activity was also determined in serum of healthy people at the age 25-40 years which constitute a control group. Hex activity was determined by the method of Chatterjee et al. modified by Zwierz et al. The concentration of Hex activity in synovial fluid of patients with rheumatoid arthritis was 15.2 nmol/ml/min and 3-4 times excedeed the serum value in these patients. In osteoarthrosis patients Hex concentration in synovial fluid was 6.15 nmol/ml/min. In serum of all investigated groups, concentration of Hex activity was 4.0-4.7 nml/ml/min. The specific activity of Hex (calculated as a constant amount of protein) in synovial fluid of patients with rheumatoid arthritis was significantly higher than in serum of these persons (p<0.017 and 0.014 respectively).

Histological examination in assessment of ultraviolet-induced suppression of contact hypersensitivity response.

The evaluation of contact hypersensitivity (CHS) reaction is one of the methods used in the assessment of the immune status of an organism after UV radiation. The aim of the study was to compare usefulness of visual scoring system and histological morphometry in the assessment of CHS response after exposure of humans to solar simulated radiation (SSR). The study included 140 healthy volunteers, 33 people were irradiated for 2 days, 34 - for 10 days and 33 - for 30 days with SSR. Forty non-irradiated individuals served as controls. All the volunteers were sensitized with diphenylocyclopropenone (DPCP) 24 h after final exposure. Statistical analysis comparing intensity of CHS reaction based on visual score between irradiated groups and non-irradiated group revealed no differences (p>0.05). We found a significant difference in epidermal thickness between healthy skin and irradiated groups (p<0.05) and a positive correlation between intensity of spongiosis and clinical score for CHS response at 3.2 DPCP site (p<0.000001). A negative correlation between time of irradiation and spongiosis score was revealed (R=-0.28; p<0.001). We conclude that histological examination of biopsies taken from one of the series of elicitation sites is a reliable and sensitive method in the evaluation of CHS response after UVR.

Duration of Raynaud's phenomenon is negatively correlated with serum levels of interleukin 10 (IL-10), soluble receptor of interleukin 2 (sIL2R), and sFas in systemic sclerosis patients.

BACKGROUND: Dysfunction of blood vessels and endothelial cells is regarded as a primary triggering factor of the development of autoimmunological processes in systemic sclerosis (scleroderma, SSc). Literature data suggest that the duration of Raynaud's phenomenon may correlate with disease severity. MATERIAL/METHODS: The study comprised 34 patients with SSc, 19 with limited SSc (lSSc) and 15 with diffuse SSc (dSSc). The duration of Raynaud's phenomenon (before skin fibrosis development) was correlated with the serum levels of selected markers, measured by ELISA, reflecting disturbances in autoimmunological processes, angiogenesis, apoptosis, and fibrosis. RESULTS: A shorter duration of Raynaud's phenomenon was shown to correlate with higher serum levels of interleukin-10 (IL-10) in the whole SSc group and of aminoterminal propeptide of collagen III (PIIINP) in the lSSc subgroup (p<0.05). It also correlated with higher serum levels of soluble interleukin 2 receptor (sIL2R) and the soluble protein sFas in the whole SSc group and the lSSc subgroup (p<0.05). CONCLUSIONS: The results obtained confirm the importance of vasomotor disturbance duration in triggering fibrosis in the course of systemic sclerosis.

Interleukin 4 plasma levels in psoriasis vulgaris patients.

BACKGROUND: Psoriasis is regarded as a Th1-cell type disease. Interleukin-4, a Th2 type cytokine, is diminished in psoriatic skin. It has been postulated that switching the cytokine profile from Th1 to Th2 may be of great help in the treatment of psoriasis. Some recent reports demonstrate a favorable role of IL-4 in the treatment of psoriasis. Therefore we decided to evaluate how IL-4 plasma levels fluctuate in the case of favorable treatment outcome observed in patients with clinically active and stable disease. MATERIAL/METHODS: 17 patients with active psoriasis vulgaris, 17 with stable disease, and 22 age- and sex-matched healthy controls were included in the study. IL- 4 plasma levels were evaluated by the ELISA method twice--before treatment implementation and 4 weeks thereafter. RESULTS: We observed statistically significant higher IL-4 plasma levels in the active psoriasis group both before treatment implementation (1st examination) and after 4 weeks of treatment (2nd examination) compared with both the control group (p<0.001) and the stable psoriasis group (1st examination: p<0.001, 2nd examination: p<0.01). There was no statistically significant difference in IL-4 plasma levels between the stable psoriasis and control groups in both examinations (p>0.05). CONCLUSIONS: Our data seem to support the hypothesis of the existence of immunologically distinct psoriasis subtypes.

Activity of lysosomal exoglycosidases in submandibular glands of rats intoxicated by cadmium at doses related to human chronic environmental and occupational exposures.

Work in cadmium (Cd) smelter and smoking cigarettes damages teeth and oral mucosa which are protected by tissue and salivary glycoconjugates: glycoproteins, glycolipids, and proteoglycans. We worked out a rat model imitating human "environmental" and "occupational" exposure to cadmium using 5 mg Cd and 50 mg Cd/l in drinking water, respectively. In submandibulary glands of exposed to Cd rats, we found the time and dose dependent accumulation of Cd and simultanous decrease in activity of beta-N-acetylhexosaminidase (HEX). In homogenates of submandibulary glands of control rats, beta-N-acetylhexosaminidase showed the highest activity. The activities of alpha-mannosidase and beta-galactosidase were very low. None of these exoglycosidases were inhibited by Cd even at 44 mM concentration.

Evaluation of caspase 1 and sFas serum levels in patients with systemic sclerosis: correlation with lung dysfunction, joint and bone involvement.

Recent studies point out at the role of apoptosis disturbances in the development of systemic sclerosis(SSc). The aim of our study was to examine caspase 1 and sFas serum levels in scleroderma patients and correlate the obtained results with skin involvement and internal organ changes. We studied 29 patients (14 with limited and 15 with diffuse SSc). The extension of skin involvement was measured using Total Skin Score (TSS). Internal organ involvement was assessed by specialist procedures. Serum caspase 1 and sFas levels were measured by enzyme-linked immunosorbent assay. We found correlation between sFas serum level and duration of Raynaud's phenomenon and TSS; caspase 1 serum level correlated only with TSS. Correlations between caspase 1 and lung dysfunction and sFas levels with joint and bone involvement in SSc patients were also observed. The obtained results revealed that disturbances of apoptosis might play a role in SSc pathogenesis. Caspase 1 and sFas serum levels correlate with the skin involvement severity, lung dysfunction, joint and bone changes.

A preliminary study of the submandibular gland of the rat after long-term cadmium intoxication.

The aim of the study was to establish to what degree the 24-week exposure of a rat to 5 and 50 mg Cd/dm3 affects the proliferating activity of cells with PCNA and Ki-67 nuclear immunoreactivity in the submandibular gland cells. The control animals received only redistilled water to drink. The group I rats were given 5 mg Cd/dm3, while the group II animals were given 50 mg Cd/dm3. The highest concentration of cadmium was observed in group II, with a concomitant increase in the number of PCNA-positive cells. In group I, cadmium concentration was significantly less compared to group II, and there were fewer PCNA-positive cells. The reaction for Ki-67 in both experimental groups was negative.

Serum levels of tissue inhibitor of metalloproteinases 2 in systemic sclerosis: a preliminary study.

BACKGROUND: The etiopathogenesis of systemic sclerosis (SSc)--an autoimmunological disease characterized by excess collagen and other connective tissue components in skin and internal organs--remains unclear. Given the potential role of matrix enzymes, metalloproteinases, and their tissue inhibitors in pulmonary fibrosis, we assayed the serum concentration of tissue inhibitor of metalloproteinases 2 (TIMP-2) in patients with SSc and explored its possible correlation with the duration of Raynaud's phenomenon, the intensity of sclerosis of the skin, and pulmonary lesions. MATERIAL/METHODS: We studied 18 SSc patients, 8 with limited SSc and ten with diffuse. The degree of sclerosis of the skin was measured using the Total Skin Scale. Chest x-rays and spirometric examinations were also performed. The presence and type of antinuclear antibodies (ANA) were determined by indirect immunofluorescence and double immunodiffusion in agar gelatin. Serum TIMP-2 concentration was measured by ELISA. RESULTS: The average serum TIMP-2 concentration was not significantly higher in SSc patients than in controls. No statistically significant difference was found in this respect between limited and diffuse SSc. Despite the lack of correlation between the duration of Raynaud's phenomenon, the degree of sclerosis of the skin, and the serum concentration of TIMP-2, the latter concentration was higher in patients with restrictive pulmonary dysfunction in spirometric testing. CONCLUSIONS: Our research, though involving a small group of patients, points to the probable role of TIMP-2 in the development of pulmonary fibrosis.
