Eosinophilic gastroenteritis presenting as acute abdomen.

Many causes of acute abdomen are diagnosed on computed tomography (CT). We present two cases of eosinophilic gastroenteritis that showed mural thickening of the stomach, duodenum, and/or proximal jejunum on CT. The correct diagnosis was made by checking the eosinophil count, and, in one case, unnecessary surgery was avoided.

CT findings for postsurgical blind pouch of small bowel.

OBJECTIVE: Our objective was to define the CT criteria for blind pouches formed after enteric anastomosis. CONCLUSION: Familiarity with the CT appearance of blind pouches avoids the mistaking of these entities for bowel obstruction or abscesses.

CT of acute biliopancreatic limb obstruction.

OBJECTIVE: Our objective was to report the CT features of biliopancreatic limb (afferent loop) obstruction. CONCLUSION: Acute biliopancreatic limb obstruction has typical CT features. Given its high morbidity and rate of reoperation, it is useful to make this specific diagnosis instead of reporting the findings as postoperative small bowel obstruction.

Gastrointestinal stromal tumors: CT and MRI findings.

The objective of this study was to report the CT and MRI appearances of primary and metastatic gastrointestinal stromal tumor (GIST). The clinical and imaging findings of 31 patients with histological and immunohistochemical diagnosis of GIST were reviewed. The CT and MRI findings were assessed independently for size, location, enhancement characteristics, and pattern of metastatic disease. The tumors were of enteric (n=13), gastric (n=12), duodenal (n=2), and rectal (n=3) origin. In one case the primary site was the mesentery, without involvement of bowel. Primary tumors were typically exophytic (79%), larger than 5 cm (84%), and inhomogeneously enhancing (84%). Central necrosis of all tumors (37%) and aneurysmal dilation of enteric tumors (33%) were less common. Metastases were most commonly to mesentery (26%) or liver (32%). Less common findings were ascites (7%) and omental caking (3%). Liver metastases were hypervascular in 92% of patients and rapidly became cystic following therapy with imatinib mesylate (Gleevec; Novartis, East Hanover, NJ, USA). Lung metastases, bowel obstruction, vascular invasion, and significant lymphadenopathy were not seen in any patient. GISTs have some specific CT findings which could help differentiate them from other gastrointestinal tumors. Liver metastases became cystic following therapy, mimicking simple cysts. MRI was better than single-phase CT for assessing liver metastases, while CT was more sensitive for mesenteric metastases.

Distinguishing gelatin bioabsorbable sponge and postoperative abdominal abscess on CT.

OBJECTIVE: The objective of our study was to differentiate the CT findings of gelatin bioabsorbable sponges used as hemostatic agents from postoperative abdominal abscess. CONCLUSION: Gelatin absorbable sponge may mimic a postoperative abscess on CT. Findings that may help differentiate the hemostatic agent from abscess include linear arrangement of tightly packed gas bubbles, fixed position of gas bubbles on subsequent examinations, shape, lack of air-fluid level, and lack of enhancing wall.

Gastrointestinal complications of bariatric Roux-en-Y gastric bypass surgery.

Obesity is rapidly becoming the most important public health issue in USA and Europe. Roux-en-Y gastric bypass is now established as the gold standard for treating intractable morbid or super obesity. We reviewed the imaging findings following this surgery in 234 patients. In this pictorial essay we present the CT and upper gastrointestinal contrast study appearances of the expected postoperative anatomy as well as a range of abdominal complications. The complications are classified into leaks, fistula and obstruction. Postoperative gastric outlet and small bowel obstruction can be caused by anastomotic stenosis, mesocolic tunnel stenosis, adhesions, stomal ulcer, obturation, intussusception and internal or external hernia. Small bowel obstruction may be of a simple, closed loop and/or strangulating type. The radiologist should be able to diagnose the type and possible cause of obstruction.

Primary epiploic appendagitis: CT diagnosis.

The purpose of this study was to analyze the CT signs of primary epiploic appendagitis. A retrospective search of the CT database over 12 months for this diagnosis revealed 11 cases. The clinical findings were recorded. Softcopy CT images were reviewed by two experienced abdominal radiologists (KS, DM) for location of lesion, size, shape, presence of central hyperdense focus, degree of bowel wall thickening, mass effect, and ancillary signs. Abdominal pain was the primary symptom in all patients. Preliminary diagnoses were appendicitis (n=2), diverticulitis (n=5), pancreatitis (n=1), ovarian lesion (n=1), or unknown (n=2). Abdominal examination and white blood cell count were uninformative. CT examination revealed a solitary (n=11), ovoid (n=9) fatty lesion with some soft tissue stranding adjacent to the left colon (n=6), transverse colon (n=3), or right colon (n=2). Central hyperdensity (n=5), mild bowel wall thickening (n=2), and parietal peritoneal thickening (n=4) were also seen. In 4 patients the lesions were not visible on follow-up CT examination performed 23-184 days later. Primary epiploic appendagitis can clinically mimic other, more serious inflammatory conditions. Knowledge of its findings on CT would help the radiologist make the diagnosis and allow a more conservative approach to patient care.

MR imaging and MR spectroscopic imaging of prostate cancer.

The primary indication for prostate MR and MR spectroscopic imaging is evaluating men with newly diagnosed prostate cancer who are deciding whether to undergo surgery or radiotherapy. Other applications of MR and MR spectroscopic imaging in prostate cancer are not defined fully. Areas of research include volumetric localization of prostate cancer, in vivo MR spectroscopic imaging at high field strength, in vitro MR spectroscopic imaging at very high field strength, novel spectroscopic markers of malignancy,and interventional MR guidance of biopsy and therapy. MR spectroscopic imaging remains a relatively novel technique, and successful implementation is demanding. Nonetheless, only MR and MR spectroscopic imaging allow structural and metabolic evaluation of prostate cancer location, aggressiveness, and stage, and MR imaging provides clinically and therapeutically relevant information on prostatic and periprostatic anatomy.

Fast fetal magnetic resonance imaging.

Fetal magnetic resonance imaging (MRI) can be used as a problem-solving tool when ultrasonic findings are equivocal. The role of fetal MRI has increased as obstetricians become aware of its potential and in utero therapy for anomalies becomes increasingly sophisticated. In this pictorial essay, we present a wide range of anomalies diagnosed or confirmed using MRI and discuss findings that help in the differential diagnosis.
