[Biliary abnormalities associated to portal Cavernoma. About 17 cases].

UNLABELLED: Portal biliopathy is a newly introduced term, describing the changes observed in the biliary ducts secondary to extrinsic compression by the portal cavernoma. It's a rare condition that is usually not diagnosed. OBJECTIVE: To explore the principles of diagnosis and treatment of portal biliopathy. METHODS: seventeen patients with extrahepatic portal vein obstruction were reviewed prospectively. RESULTS: symptomatic biliary obstruction was found in 82%. Endoscopic retrograde cholangiography revealed abnormality of the bile duct wall in all cases, with stricture in 11 patients. Endoscopic treatment was necessary in 10 patients, whether portosystemic shunting was done only in 2 cases. CONCLUSION: Portal biliopathy is frequent and must be diagnosed early.

[Endoscopic treatment of residual lithiasis. 661 cases].

AIM: Endoscopic sphincterotomy is a well-established procedure for treating choledocolithiasis and particularly residual lithiasis. The aim of this study is to expose our experience with this method and to evaluate his safety and efficacy. METHODOLOGY: It is a retrospectively collected series of 661 patients with residual choledocolithiasis. RESULTS: Endoscopic retrograde cholangio-pancreatographies were performed in 651 patients (98.5%). Standard sphincterotomy was realized in 633 patients (95%) and a pre-cut technique was done in 15 (2%). Bile duct clearance was achieved in 583 cases (91%). Overall, the complication rate of sphincterotomy was about 8.4%. CONCLUSION: Endoscopic sphincterotomy is a safe and effective procedure for patients with residual choledocolithiasis.

[A gastroenterology unit experience with adjuvant chemotherapy for non-metastatic colon cancer: results from 24 cases].

A prospective study was carried from January 1996 to December 2000 including patients that received adjuvant chemotherapy type FUFOL for colon carcinoma after curative surgery. Chemotherapy was recommended for all stage Dukes B2 end C. Adjuvant chemotherapy was administered to 24 patients (13 men and 11 women, mean age 56.7 +/- 11.5 years) with surgically resected stage B 2 in 7 cases (29%), C in 17 cases (71%). Treatment was completed in 15 patients (62%). With a median follow-up of 31.6 months, 11 patients had no recurrence, one patient had locoregional recurrence and 2 patients developed liver metastasis. Adjuvant FUFOL chemotherapy is actually the recommended adjuvant post-surgical treatment for colon cancer. This valided protocol is easily realised with a low toxicity and it can be done in gastro-enterology unit.

[Laparoscopic aspects of peritoneal tuberculosis. Report of 163 cases].

The aim of this study is to report the results, the complications and limits of laparoscopy in patients with tuberculous peritonitis. In a retrospective study of 163 laparoscopy realized from 1970 to 1998. All the patients had exsudative ascites with predominantly of lymphocytes. Miliary nodules were found in 87% of cases, adhesions between the peritoneum and organs were found in 69% of cases and congestion in 63% of cases. Laparoscopically guide peritoneal biopsies detected caseating granulomas in 87% of cases. Laparoscopic appearance of the peritoneum mimicking a carcinosis in 15% of cases. One patient had complication as a bowel perforation. Miliary nodules and adherences are the more frequent appearances into peritoneal cavity in tuberculous peritonitis. With peritoneal biopsies, laparoscopy is always the best method for definitively and rapidly diagnosis of tuberculous peritonitis.

[Pneumatic dilatation in esophageal achalasia. Report of 61 cases].

UNLABELLED: To assess the interest of pneumatic dilatation in patients with achalasia. MATERIAL AND METHODS: The work has interested 61 patients with achalasia who underwent pneumatic dilatation using polyethylene balloon (Rigiflex) dilators. 74 sessions of dilatation were performed and 13 patients underwent 2 sessions of dilatation (21%). One patient presented un esophageal perforation requiring a surgical treatment. The outcome was good on 88.9% and 83.3% of the patients after respectively one and six months. These results illustrate the interest of pneumatic dilatation in achalasia, which have to be proposed in the first line of the treatment.

[Gastric and colonic sarcoidosis. A case report].

Sarcoidosis is a disseminated disease defined by the presence of non-caseous granulomas. Digestive localizations are rare and the stomach is the mostly affected. The diagnosis is frequently difficult. We report the case of a digestive (gastric and colic) and hepatic sarcoidosis in a 38-year old woman. The clinical and endoscopic findings initially suggested linitis plastica. Subsequently the discovery of gastric granulomas led to the diagnosis of sarcoidosis. Coloscopy and abdominal scan were performed discovering a colic, hepatic and splenic localizations which were asymptomatic.

[Chronic pancreatitis in the ventral pancreas in pancreas divisum].

Pancreas divisum, partial or non fusion of the dorsal and ventral pancreatic ductal system, affects up to 10 percent of the population. Approximately 25 percent of patients with pancreas divisum will develop complications such as recurrent pancreatitis. Surgical and endoscopic therapy usually include minor papilla sphincterotomy or sphincteroplasty to facilitate drainage of the dorsal ductal system. The following case represents an unusual complication of the pancreas divisum with primarily ventral duct disease and pancreaticolithiasis.

[Esophageal prosthesis].

Curative surgery of oesophagus cancer is often challenged because of local and regional extension or metastasis or an impaired health in patients often elderly. Authors report result of palliative endoscopic treatment of dysphagia by prothesis in 11 patients. Improvement of dysphagia occurs in all patients. No death occurs by this technic. Global migration is observed in 44% because of low seat in 50% of cases, cardial seat in 25%, and finally important dilatation until 18 mm before putting prothesis. In conclusion, oesophagus prothesis is a palliative treatment that permit an alimentary comfort in patients with oesophagus cancer of 1/3 medium.

[Splenic angioma. Value of MRI: two case reports].

The splenic angioma is a rare tumour. Its aspects MRI are still badly known. The authors show that in the 2 brought back cases, there is a hypersignal on the 3 echoes of T2 with a catch of contrast slow and centripetal after Gadolinium injection. The hemorrhagic risk in connection with a spontaneous or traumatic rupture imposes the identification of this lesion.

[Ligation of oropharyngeal mucosa: a rare complication of ligation of esophageal varices. Two case reports].

The eradication of oesophageal varices by binding became the best traitment for the bleeding by rupture of the verices. She tends to substitute the sclerosis because of her fast delay of eradication and her fewer complications and gravity. The incidence of complications after the binding vary between 3.3 and 11%. The most frequent complication is the appearance of oesophageal ulcers that sometimes causes bleeding. A dysphagia can happen within a few hours after the binding. Infections are rare. We report two cases of pharyngeal mucosae binding occurring during the binding of the varices.
