A novel approach to the treatment of benign prostatic hyperplasia.

OBJECTIVE: To assess the clinical efficacy and safety of the combined alpha1- and postsynaptic alpha2-blocker GYKI-16084 compared to placebo during a 28-day active treatment of patients with benign prostatic hyperplasia (BPH). PATIENTS AND METHODS: After a 28-day placebo run-in phase, 7.5 and 15 mg GYKI-16084 or placebo were administered twice daily for 28 days to patients with BPH in a randomized single-blind Phase II study. Efficacy was primarily determined by changes in the American Urological Association (AUA) symptom scores and maximum urinary flow (Q(max)), while safety was assessed by orthostatic changes and adverse-event profile. A simplified International Index of Erectile Function questionnaire was used to assess effects on erectile function. RESULTS: Data from 63 patients were evaluated; the decrease in the AUA score during the active phase was greater in the 15 mg group (-6.05, -32.7%) than in the placebo (-4.3, 22.7%) or 7.5 mg (-3.55, -19.5%) groups. Q(max) improved in both active treatment groups (+3.3 and +2.16 mL for the 7.5 and 15 mg groups, respectively) compared to placebo (+1.29 mL). None of the drug-related adverse events associated with selective alpha1-blockers were reported. CONCLUSION: The combined alpha1- and postsynaptically selective alpha2-blocker GYKI-16084 significantly improved the AUA symptom scores and increased Q(max) in patients with BPH, without inducing any adverse reaction, orthostatic changes or erectile dysfunction.

Percutaneous nephrolithotomy in early pregnancy.

We report on an 11-weeks pregnant woman, who under went a percutaneous nephrolithotomy without the use of X-rays during the procedure due to 8-mm left upper ureteric stone. In the available literature, we didn't find any reported case about percutaneous stone removal without the use of X-rays.

Single-session laparoscopic radical and contralateral partial nephrectomy.

Renal tumor is likely to become one of the most important indications for laparoscopic surgery. We present an old woman, who underwent single-session laparoscopic nephrectomy and contralateral partial nephrectomy due to bilateral kidney tumor. The advantages of simultaneous bilateral intervention are reduced psychological stress, single anaesthesia, less medication, less blood loss, shorter hospital stay and convalescence, and considerable cost-effectiveness.

Renal cell cancer associated with sarcoid-like reaction.

An unusual granulomatous reaction within a conventional clear cell renal cancer in a 62 year-old woman is reported. Using immunohistochemical evaluation, cells of the granuloma were CD68 (Kp1), carboxypeptidase M and CD3 positive. No signs of sarcoidosis were found in other organs. According to the few publications that mention cancer associated sarcoid-like reaction, such lesions do not influence the prognosis. Our patient is still well for a 15 months follow-up.

Tumor grade-dependent alterations in the protein kinase C isoform pattern in urinary bladder carcinomas.

OBJECTIVES: Members of the protein kinase C (PKC) isoenzyme family play central role in the tumorigenesis of several tissues. In this study our goal was to determine the possible alterations in the protein kinase C (PKC) isoform pattern in relation with the different tumor grade in human urinary bladder carcinomas. METHODS: Western blot analysis, followed by quantitative densitometry, was performed to define the expression of PKC isoforms in the epithelial tissue of human urinary bladder carcinomas with various tumor grades and in control samples. RESULTS: The human urinary bladder epithelium expressed five PKC isoforms (PKC alpha, beta, delta, zeta), the levels of which differentially altered as a function of tumor grade. Namely, whereas the expressions of PKC beta and delta decreased with increasing grade of the carcinomas, the levels of PKC alpha, and zeta showed opposite patterns of changes. CONCLUSIONS: These grade-dependent alterations in the PKC isoform pattern strongly argue for the central yet antagonistic roles of certain members of the PKC system in malignant transformation of human urinary bladder epithelium.

Endopyelotomy in childhood: our experience with 37 patients.

PURPOSE: To evaluate our experience and results with endopyelotomy in the pediatric population. PATIENTS AND METHODS: Between 1990 and 2002, we performed percutaneous antegrade endopyelotomy under general anesthesia in 37 children because of ureteropelvic junction (UPJ) stricture. The youngest patient was 4.5 years and the oldest 17 years at the time of the procedure (mean age 11.5 years). One patient had bilateral stenosis; the two sides were operated on separately. After insertion of a 4F ureteral catheter and filling the collecting system with colored contrast material, a middle calix was punctured under fluoroscopic control. The tunnel was dilated to 26F by telescopic metal dilators. After insertion of a 0.035-inch gidewire through the UPJ, all its layers were cut by a cold knife in the dorsolateral direction so that the periureteral fatty tissue could be seen. Finally, the ureteral wound was stented by a 6F to 12F transrenal drain or a double-J catheter, which was removed after 6 weeks. RESULTS: Among the 37 patients, the procedure had to be repeated in 1 because the transrenal drain stenting the UPJ slid back to the renal pelvis. We had to perform open pyeloplasty or nephrectomy in two patients because of bleeding or failed procedure. The average postoperative hospital stay was 6 days. Comparison of the preoperative intravenous urograms with studies performed 1 year after endopyelotomy showed an overall success rate of 89%. All patients are without complaints at the moment. CONCLUSIONS: In experienced hands, endopyelotomy is a safe and effective method for the treatment of UPJ stricture, not only in the adult, but also in the pediatric, population.

Diode laser based photoacoustic water vapor detection system for atmospheric research.

A wavelength modulated, distributed feedback diode laser based photoacoustic water vapor mixing ratio measuring system for atmospheric research applications is presented. Laser modulation parameters were optimized either at 180 or 500 mbar total pressure to enhance the system's sensitivity for low or high pressures (upper troposphere/lower stratosphere or biosphere exchange layer), respectively. A wavelength locking method was developed that ensured sub-picometer absolute (5 x 10(-7) relative) wavelength stability of the laser while consuming minimum additional measurement time. At the calibration of the system, correction factors for the pressure- and temperature-dependence of the photoacoustic signal were determined, which were in turn applied to the calculation of the water vapor mixing ratio from the measured signal during the test operation of the system. The introduced features resulted in reliable, sub-ppm-level water vapor detection even under abrupt gas pressure or temperature variations typical in open atmospheric applications.
