Spontaneous posterior dislocation of both lenses in a patient with Marfan's syndrome: 17 years without complications.

In a twenty five-year-old female patient, we observed bilateral lens dislocation into the vitreous body that occurred spontaneously as a result of Marfan's syndrome and megalocornea, when she was 8 years old. Corneal diameters were 14 mm in the right eye and 13.5 mm in the left eye. Slit-lamp examination revealed miosis and iridodonesis. Fundus examination revealed bilateral inferior lens dislocation into the vitreous body. There was no lens induced posterior uveitis in either eye. This patient demonstrated that lens dislocation into the vitreous body can be present for a long time without causing any intraocular inflammation.

Abducens nerve palsy and optic neuritis as initial manifestation in brucellosis.

Cranial nerve involvement in brucellosis is rare. We present a case of brucellosis presenting with optic neuritis and abducens nerve palsy on the left side. Cerebrospinal fluid findings indicated Brucella meningitis with high protein count, low sugar level and pleocystosis. In addition, Brucella agglutination test (Wright test) was found to be 1/128 in cerebrospinal fluid. Serum agglutination test for Brucella was also positive at 1/1280. This case was diagnosed as brucellosis involving optic and abducens nerves. The patient was treated by ceftriaxone (intravenous), rifampicin (orally) and doxycycline (orally). Two months later the patient's vision acuity in the left eye had moderately improved and the patient's left abducens palsy had almost disappeared. In conclusion, cranial nerve involvement in brucellosis can have good prognosis if anti-Brucella treatment is undertaken early.

The frequency of combined target organ damage and the beneficial effect of ambulatory blood pressure monitoring in never treated mild-to-moderate hypertensive patients.

The aim of this study was to determine the frequency of target organ damage (TOD) and the beneficial properties of ambulatory blood pressure monitoring (ABPM) for detecting patients who are at high risk for TOD and cardiovascular disease in never treated mild-to-moderate hypertension. Sixty-seven patients (28 males and 39 females, mean age, 49.6 +/- 9.5 years) were divided into two groups, dippers (group I, n = 43) and nondippers (group II, n = 24), according to nocturnal blood pressure (BP) reduction of less than 10%. The groups were compared with respect to demographic and laboratory data and the signs of TOD (microalbuminuria, left ventricular hypertrophy, and retinopathy). We also tested the relationship between ABPM and clinic BP findings with TOD. Group I had significantly lower values than group II for serum fibrinogen (0.28 +/- 0.06 versus 0.32 +/- 0.06 g/L, P = 0.02), uric acid (0.18 +/- 0.05 versus 0.25 +/- 0.11 mmo/L, P = 0.01), urinary sodium excretion (133.7 +/- 45.2 versus 161.8 +/- 52.2 mmol/L, P = 0.02), urinary albumin excretion (17.5 +/- 14.2 versus 31.3 +/- 19.7 mg/24-h, P = 0.001), left ventricular mass index (111.8 +/- 31.0 versus 128.7 +/- 36.6 g/m(2), P = 0.05), and the prevalence of hypertensive retinopathy (51% versus 83%, P = 0.01). The frequency of the combination of all three signs of TOD (microalbuminuria, left ventricular hypertrophy, and hypertensive retinopathy) was higher in nondippers than in dippers (71.4% versus 30%, P = 0.04). We suggest ABPM may provide clinical information to detect patients prone to develop cardiovascular risks and TOD in newly diagnosed mild-to-moderate hypertension.

Topical cyclosporine in the management of shield ulcers.

PURPOSE: To report our experience with 0.05%-2% topical cyclosporine in the treatment of 4 cases of severe, corticosteroid-resistant, vernal keratoconjunctivitis with shield ulcers. METHODS: Four patients with shield ulcers not responding to medical treatment with topical steroids, antihistamines, and mast-cell stabilizers were treated with 0.05%-2% topical cyclosporine. The frequency of application was 4 times daily in all cases, and the concentration was adjusted according to the clinical status starting with 2%. RESULTS: Healing of ulcers with no epithelial defect occurred in 10 days with the 2% concentration in all cases. The patients experienced no side effects except for a burning sensation during administration. Recurrences occurred when the concentration was decreased to 0.05% in 3 cases. The recurrences were effectively treated with 1% concentration in all cases, and patients were stable with this concentration for at least 6 months. CONCLUSIONS: Topical cyclosporine is an effective treatment alternative in the management of shield ulcers in patients with vernal keratoconjunctivitis. The 1% concentration seems to be the minimal effective concentration in such cases in our experience, but further controlled studies are needed to support our findings.

Preventative effect of deferoxamine on degenerative changes in the optic nerve in experimental retrobulbar haematoma.

PURPOSE: Changes in the optic nerve due to the breakdown of blood elements and the effect of deferoxamine on these changes were evaluated. METHODS: The study group consisted of 12 rabbits. Three rabbits were used as control. In Group I, three rabbits had bilateral retrobulbar haematoma induced. In Group II, three rabbits had bilateral retrobulbar haematoma induced and were given systemic deferoxamine. In Group III, three rabbits were only given deferoxamine. At day 21, all the rabbits underwent bilateral enucleation and specimens from the orbital fat tissue were removed for light microscopic examination. The optic nerves were examined by light and electron microscopy. RESULTS: Light microscopic examination of the optic nerves did not reveal significant differences among the groups. Electron microscopy revealed ultrastructural changes in Group I. There were no significant pathological findings observed in Groups II or III. There was abundant iron pigment in the orbital fat tissue of Group I, but only a small amount in Group II. CONCLUSION: Systemic deferoxamine treatment is effective in preventing degenerative changes in retrobulbar haema-toma.

Ocular surface changes in keratoconjunctivitis sicca with silicone punctum plug occlusion.

PURPOSE: To evaluate histopathologic and clinical response to silicone plug insertion in dry eye patients. METHODS: Punctal plugs were placed in 32 eyes of 18 dry eye patients with aqueous deficiency who were on maximum medical therapy and who had Schirmer testing with topical anesthetic measuring less than 5 mm. Pre-treatment and post-treatment (6 weeks and 1 year) evaluations included temporal and inferior quadrant impression cytology examinations, Schirmer-1, BUT, corneal fluorescein and rose-bengal staining patterns and questionnaire scores. The cytology samples were graded according to the method described by Nelson. RESULTS: There was an improvement in subjective symptoms of patients 6 weeks after punctal occlusion. Corneal fluorescein and rose-bengal staining scores decreased from a mean of 5.33 +/- 1.74 to 1.37 +/- 1.48 and from 5.90 +/- 1.2 to 2.45 +/- 1.89 (p < 0.05) respectively. BUT values increased from 2.98 +/- 1.1 to 11.3 +/- 3.1 seconds (p < 0.05) at 6 weeks post-treatment. Increase in goblet cell density was observed after punctal occlusion at 6 weeks (p < 0.05) and at 1 year (p < 0.05) compared to the pre-treatment values. After occlusion, 3% of temporal specimens had Grade 0 squamous metaplasia, 63% had Grade 1, 34% had Grade 2; while 25% of inferior specimens had Grade 0, 56% had Grade 1, 19% had Grade 2 squamous metaplasia. CONCLUSIONS: Punctum plug occlusion in keratoconjunctivitis sicca provides improvement of tear film stability, ocular surface staining scores, conjunctival squamous metaplasia grades and goblet cell density. Increased ocular surface exposure to essential tear components with punctal occlusion may be important for the genesis of these changes.

The effect of hemodialysis on visual field test in patients with chronic renal failure.

PURPOSE: To investigate the visual field with FASTPAC 30-2 program before and after hemodialysis in patients with end stage renal disease. MATERIALS AND METHODS: Twenty eyes of 20 patients on regular hemodialysis were included in the study group. Twenty eyes of 20 healthy patients were chosen as control group. Intraocular pressures (IOP) were measured one hour before and one hour after the same hemodialysis session, and visual field was tested at the same times. RESULTS: When IOP was compared before and after hemodialysis, no statistically significant difference was found (p > 0.05), however Mean Deviation (MD) (p = 0.008) improved after hemodialysis. When we compared first global indices of the control group with pre-hemodialysis global indices, we noted significant difference in MD (p = 0.009). CONCLUSION: Visual field testing should be done after hemodialysis in patients who are on regular hemodialysis program.

Investigation of hemodynamic changes after strabismus surgery using color Doppler imaging.

PURPOSE: To investigate hemodynamic changes after strabismus surgery using color Doppler imaging (CDI). METHOD: Group 1 patients (n = 13) underwent surgery on a single horizontal rectus muscle, and group 2 (n = 13) underwent surgery on both horizontal rectus muscles. Subjects with no systemic or ophthalmologic disorders were selected as controls (n = 16). In group 1 and group 2, CDI of the ophthalmic artery (OA), the central retinal artery (CRA), and the nasal and temporal posterior ciliary arteries (PCAs) was performed before surgical correction and on postoperative days 1 and 7. For each vessel, peak systolic (V(max)), end diastolic (V(min)), and mean (V(mean)) blood flow velocities were measured, in addition to the resistivity index (RI) and pulsatility index (PI). RESULTS: V(max) in the OA on postoperative day 1 was higher in group 2 than in group 1 (P =.004). V(min) in the OA on postoperative day 1 was significantly higher in group 2 than in the controls (P =.001) and group 1 (P =.001). For group 2, comparison of the OA preoperative findings to the findings on postoperative day 1 showed that V(min) (P =.003) and V(mean) (P =.02) increased significantly, and the RI (P =.01) and PI (P =.006) decreased significantly. These significant differences were not observed on day 7 (P >.05). CONCLUSION: Surgery on both horizontal rectus muscles in an eye causes hemodynamic changes in the OA in the early postoperative period.

Non-concordance in amblyopia treatment: the effective use of 'smileys'.

PURPOSE: The success of any specific occlusion regimen is limited by the level of concordance, which is difficult to measure accurately in patching treatment. The aim of this study was to investigate prospectively the concordance rate of a group of children with strabismic and/or anisometropic amblyopia using a schematic diary based on completing 'smiley' images, filled in by the child under parental supervision, and to assess the effect of correlates such as initial visual acuity, age, and total and daily occlusion time on concordance. METHODS: We recruited 51 amblyopic children aged 13 months to 12 years (mean 6.57 +/- 2.82). The median duration of treatment was 16 weeks. Non-concordance was defined as occluding less than 75% of the prescribed time and was analyzed using a concordance index calculated from occlusion time recorded in the diary/prescribed occlusion time. RESULTS: Of the 51 patients followed prospectively, 12 (23.5%) failed to return their diaries (Group 1); 27 (52.9%) complied with occlusion (Group 2); and 12 (23.5%) failed to comply with occlusion completely (Group 3). The total proportion of non-concordance was 47.1% (Group 1 + Group 3). Concordance was not significantly related to initial visual acuity (r = -0.22, p = 0.19), patient age (F = 1.0787, p = 0.349) or total occlusion time (X2 = 2.779, p = 0.249), but the number of daily occlusion hours showed significant difference in Group 1 (X2 = 15.894, p = 0.000). When the three groups were compared for change in visual acuity, a significant difference was found between group 1 and group 2 (X2 = 6.125, p = 0.047). CONCLUSION: The substantial proportion of non-concordance in our study suggests that, although useful for recording purposes, parental diaries may not be stimulating enough to increase the overall level of concordance. Therefore, other forms of monitoring need to be explored.
