CT attenuation of colorectal polypoid lesions: evaluation of contrast enhancement in CT colonography.

The aim of this study was to calculate pre- and postcontrast CT attenuation values of benign colorectal polyp and carcinoma lesions detected by virtual colonoscopy, and to investigate whether contrast enhancement of these lesions can be potentially used for differentiation from residual fluid in the colon. Fifteen benign polyps and 21 colorectal carcinoma lesions detected by virtual colonoscopy in 18 patients were included in our study. All of the polyps and carcinoma lesions were confirmed by colonoscopic biopsy. Measurement of CT attenuation values was performed in precontrast (supine) and postcontrast (prone) scans for each polyp and carcinoma. The CT attenuation values of residual fluid in the colon was also measured from the same location before and after intravenous contrast administration. On unenhanced CT scan mean attenuation values of benign polyps and colorectal carcinomas were 32.4 and 42.6 HU, respectively. Following contrast enhancement, mean attenuation value increased to 78.9 HU for polyps and 90.7 HU for carcinomas. Increase in the CT attenuation values of these lesions was significant ( p<0.0001). Mean CT attenuation value of residual fluid before and after administration of IV contrast were 14.6 and 13.8 HU, respectively. The difference between CT attenuation value of residual fluid in the colon before and after contrast material was not significant ( p=0.29). Colorectal benign polyps and carcinomas demonstrate significant enhancement following contrast administration and use of intravenous contrast material during virtual colonoscopy may help in some cases in differentiating these solid lesions from residual colonic fluid that does not enhance.

Unusual imaging findings in Wilson's disease.

We present unusual imaging findings in the liver of a patient with Wilson's disease. On US, the liver contours were irregular and its echogenicity was increased with multiple, small hypo- and hyperechoic nodules. Prominent perihepatic fat layer was noted to be increased in thickness. On CT and MR, multiple, small, enhancing nodules were detected in the liver in the early arterial phase after administration of intravenous contrast material. Biopsy was consistent with chronic parenchymal liver pathology with dysplastic changes but without evidence of hepatocellular carcinoma. To our knowledge, dysplastic nodules enhancing at the arterial phase and thickened perihepatic fat layer have not been previously reported in patients with Wilson's disease.

Virtual endoscopy.

Virtual endoscopy is a new-generation technique which combines the features of endoscopic viewing and cross-sectional volumetric imaging. In the evaluation of gastrointestinal cancers, virtual endoscopy has been most commonly used in colorectal carcinomas and to a much lesser extent in gastric carcinomas. In this review, the current status of virtual colonoscopy was reviewed together with a brief discussion of virtual gastroscopy.

Cardiac tuberculosis with multiple intracardiac masses: a case report.

In this report, clinical, echocardiographic, and pathologic findings of a patient with multiple masses caused by tuberculosis both in the left and right side of the heart are presented. After antituberculosis treatment some of the masses disappeared and some became smaller. Although an intracardiac mass caused by tuberculosis is very rare, it should be considered in the list of masses detected by echocardiography.

Multifocal fibrosclerosis: a new case report and review of the literature.

A case of multifocal fibrosclerosis is presented with MR images. Bilateral sclerosing orbital pseudotumor invading cavernous sinuses were the presenting disorder. Magnetic resonance imaging showed involvement of paranasal sinuses bilaterally and multiple supratentorial dural masses. Retroperitoneal fibrosis was associated with the condition. Multifocal involvement should be considered in patients with sclerosing orbital pseudotumor and an imaging approach should be performed on the appropriate clinical condition to document possible coexistence of other disorders including retroperitoneal fibrosis, mediastinal fibrosis, sclerosing cholangitis, and Riedel's thyroiditis

Liver volume measurement by spiral CT: an in vitro study.

Liver volume measurement has many clinical and research uses. The purpose of the present study is to evaluate experimentally the accuracy and reproducibility of spiral computed tomography (CT) in measuring liver volume. To determine the accuracy of spiral CT system, nine fresh (non-formalin-fixed) sheep livers were scanned in vitro. The collimation was 7 mm. The CT sections were stored on floppy disks and the images were transferred to the workstation that contained a software for volume measurement. The mean volume is 912+/-497 ml and the actual reference volume is 921+/-527 ml. The correlation between the CT system and actual reference measurement of sheep liver volumes was very strong. The correlation coefficient r was.998 (P<.001) on 7-mm sections. The mean difference between liver volume measurements was 4.95+/-3.23%. As a result, CT scan could be used as a reference noninvasive method for liver volume measurement in clinics.

Adrenal adenoma presenting with torsade de pointes--a case report.

A case of primary hyperaldosteronism due to an adrenal adenoma with near syncope and torsade de pointes is described. A woman patient had a history of high blood pressure and severe hypokalemia that was the cause of her ventricular arrhythmia, which was controlled by administering potassium supplementation but no antiarrhythmic medication. Adrenal adenoma was identified on axial computerized tomography. This case report suggests that there may be a chance of complete cure from torsade de pointes if the underlying cause of QT prolongation can be identified.
