[Results of urolithiasis treatment in pilots with the use of a new domestic lithotripter "Kompakt" during remote lithotripsy].

Results of treating urolithiasis in pilots on new Russian lithotriptor Kompakt were analyzed. Length of the shock-wave pulse is 0.3-0.4 microsecond w/o the negative phase prevents damaging of the kidney tissue. Urolithiasis was treated in 45 pilots in 69 ESWL sessions: 6.2 concrements, mostly in calyces renales (95.6% of the patients), were crushed. In more than 30% of the patients concrements were no larger than 5 mm and in 66.7% of the patients they were X-ray negative (urate). Concrements were crushed in all 45 patients into fine-dyspersated fragments (< 3 mm). At no time after ESWL complications or secretory-excretory dysfunctions of kidneys were observed. In-hospital examination following 1.5-2 mos. since ESWL did not detect residual fragments in urinary tracts of the pilots. No renal or any other pathologies were found. All the patients were admitted to flight.

[The treatment of urolithiasis in the specialized urologic department].

The experience of urolithiasis treatment with the use of modern diagnostic methods (the computer tomography with concrement density detection, the DV-Med program which permits to obtain the calculus volumetric image and to study its physical characteristics), and therapy (distant lithotripsy with the use of different types of lithotriptors as well as retrograde contact lithotripsy with "Lithoclast") permits to minimize the percent of open surgical interventions. For the last 3 years 780 patients with different clinical disease forms were treated with favorable clinical outcome and only in 0.5% cases the open surgical intervention was conducted. These patients underwent the pyelolithotomy for the secondary nephrolithiasis conditioned by the pelvis-and-ureteral segment stricture when the endoscopic method failed to eliminate this problem.

[The lithotriptor "Compact" used in urinary bladder diseases].

The stroke-wave complex "Compact" is the first home-produced lithotriptor with electromagnetic system of stroke-wave generation and ultrasonic diagnosis. During its development it was possible to achieve the optimal parameters of focused waves, good technical characteristics and economic indices. The clinical use of lithotriptor has shown its efficiency in nephrolith and ureterolith remote destruction in the absence of complications connected with negative influence of stroke waves on kidneys and surrounding organs and tissues.

[Distant pulse-wave lithotripsy in the Central Clinical Air Force Hospital ].

With participation of the specialists from the 7th Central Military Clinical Aviation Hospital 4 pulse-wave complexes "Urat-P", "Urat-P2", "Ulitho", "Cholelithotriptor" were developed and introduced into clinical practice of the hospital. Distant pulse-wave lithotripsy was conducted 3035 patients (358 persons from flying staff). It is the main method of urolithiasis treatment, especially in the flying staff. Its efficiency is more than 96%. The modern models of lithotriptors are effective in destruction of biliary and urinary calculi and the applied parameters of wave impulses don't impact on the organs and tissues. The sphere of use of pulse-wave effect in the clinical practice was extended.

[Urolithiasis in flight personnel].

Our investigations shed the light on some clinical peculiarities of urolithiasis in flight personnel including development of the disease at young age, dependence of the risk of renal stones on length of flight career, prevalence of small increments in calices, asymptomatic course, comparatively more frequent uratate urolithiasis, and more rapid recurrence of the disease. The clinical profile of urolithiasis in flyers can be related with the morphofunctional state of kidneys as a result of stresses of the flight labour. There is necessity to search for and introduce modern scientific approaches to prevention and effective technologies of urolithiasis treatment in flight personnel.

[Diagnosis and treatment of nephroptosis in flying personnel].

Fifty human subjects of flying personnel and 25 human subjects of ground personnel suffered from nephroptosis mean age of which was 28.7 +/- 0.9 years have been under study. Dynamics observation of flying personnel for a period of 1-8 years revealed that the persons suffered from nephroptosis of the 1st stage are capable of performing the flying activity without limitations and need such observation and performance of the appropriate treatment- and--prophylactic measures. With the nephroptosis of stage II (27 pilots), the early operative treatment for eliminating the nephroptosis is indicated. The post-operative supervision of such patients is mastered. The nephropexy has been performed in 24 pilots; more recently, 23 persons were allowed to perform flying activities without restrictions, there were no nephroptosis recurrences. In the patients group of ground servicing personnel (26 persons) there have effectively been performed 20 nephropexies. Thus, it has been possible to overcome the nephroptosis and to improve the functional state of the kidneys. The operative treatment results allow one to recommend the early operative treatment of the stage II nephroptosis in the flying personnel even in the absence of the complicated nephroptosis and pronounced disorder of the kidney function.

[The work experience of the Central Military Aviation Research Hospital in the occupational rehabilitation of flight personnel after lithotripsy].

The evolution of the domestic-certified lithotriptors has made a complete professional rehabilitation of all pilots (100%) suffered from urolithiasis without operative intervention possible. Of 156 pilots who have been exposed to remote shock-wave lithotripsy (RSWL) in recent three years 120 individuals (93%) had the urate stones in the renal calyces. Before introducing the RSWL in clinical practice such concrement were recognized as inoperable and the urolithic pilots have no prospects for continuation of their flying activities. After the RSWL application there was a decrease in the secretory-excretory renal function due to the disturbed urodynamics by the migratory fragments of crushed stones in the renal system. The recovery of renal function occurred after complete passage of the crushed stone fragments. Depending on the composition of a crushed concrement, severity of inflammatory process taking place in the kidneys and urinary tracts, the special program of countermeasures to prevent relapsed lithogenesis was recommended for use by the patients. The relapse of renal stone formation after RSWL application was noted in 4 (3.2%) pilots. In these pilots, the concrements were recrushed by the mentioned method and professional qualification of above individuals was retained. Until recently, the treatment of cholelithic disease was recognized as a purely surgical problem--with the introduction of RSWL of renal stone into clinical practice there appeared real possibility to apply the similar approach to crushing the gallstones. To attain these ends, the experimental studies with the use of lithotriptor "URAT-P" have been performed. 140 gallstones of 0.5 to 2.0 cm in size were crushed. [Current problems in the diagnosis, treatment and prevention of cholelithiasis in flight personnel].

In 50% of afflicted pilots, cholelithiasis is asymptotic and develops with the minimum anatomicomorphological changes of the biliary system. An early diagnostics of cholelithiasis in the pilots will enable one to treat them timely with the aim of returning the pilots to flying activity in the future. The introduction of nonsurgical treatment methods into the clinical practice with an application of distance shock-wave cholelithotripsy will elevate the percentage of pilots returning to flying activities. A need for developing domestic special devices for cholelithotripsy is ripe.

[The characteristics or urolithiasis in flight personnel and the treatment results using a method of extracorporeal shockwave lithotripsy on the Russian lithotripter Urat-P2].

Urolithiasis occurs more frequently in the pilots than in the ground-service personnel. To decrease a damaging effect of the focused shock waves on the renal tissue while using remote lithotripsy technique, a new domestic-certified lithotriptor "URAT-P2" which shows a significantly low energy of shock wave with sufficient effectiveness of crushing the renal stones, is devised. Urolithiasis in the pilots is apparent at the early stages of its development and its complicated forms are less common than in the subjects of other professions. The stones are predominantly located in the renal calyces (73,3% of cases), in the left kidney and urethra in 63,3% of cases and there were urate stones in 40% of cases. The clinical use of the URAT-P2 complex in 30 pilots indicated that it holds advantages over the production domestic-certified lithotriptor. The stones are crushed in all the patients but the complications associated with a negative effect of the focused shock waves on the renal tissue are absent. The use of the new lithotriptor for treating urolithiasis in the pilots will allow one to improve the treatment response and to maintain professional worthiness of the pilots for a long time.

[The prospects for the use of a Soviet apparatus for the removal of gallstones with focused shock waves].

Clinico-experimental substantiation was conducted for extracorporal crushing of the stones in biliary ducts in patients who suffered from gallstone disease. The "Urat-II" national lithotriptor and "Urat-B" biostand were used to crush in experimental conditions 140 gallstones (0.5-2.5 cm in size) obtained during cholecystectomies. Experimental researches on 56 white rats were conducted to study the liver tissue and adjacent organs. Optimal regimes for the percussive wave generator were found out in which the best crushing effect was obtained (with the dimensions of fractions up to 1.6 mm) and at the same time the minimal disturbing factor was produced. The data obtained during these researches are used in clinical pilot testings on "Urat-II"--a prototype of the national cholelithotriptor.

[Effects of focused shock waves on children's kidney: experimental study].

The effects of focused shock waves produced by URAT-P2 unit on renal tissue was studied in 120 immature rats with 100-120 g b.w. Morphological material obtained after the procedure and 3 months after it was studied with reference to the following parameters of the impact: interelectrode spacing, capacitor capacitance, capacitor charging voltage, pressure in the therapeutic focus, number of impulses. It was found that one of the essential damaging factors of the shock waves is spontaneous widening of the interelectrode spacing in the course of the fragmentation which entails an increase in the pressure amplitude in the center of the therapeutic focus with resultant prolongation of the impulse.

[The characteristics of an acute infectious-inflammatory process in the single remaining kidney (experimental and clinical study)].

The paper is concerned with acute pyelonephritis pathogenesis in patients with a solitary kidney. Upon nephrectomy, the remaining kidney undergoes compensatory-adaptive processes giving rise to injury of the first barrier of the local nonspecific resistance. This results in more rapid and severe course of acute pyelonephritis, development of pyodestructive disease, protracted run. Active surgical policy in acute obturation pyelonephritis in patients with a single kidney, early recovery of adequate urine outflow can prevent progression to destructive pyelonephritis and advance treatment results.
