Communicating with individuals with dementia. The impaired person's perspective.

1 To better meet needs of individuals with dementia, caregivers should focus on receiving and interpreting verbal and nonverbal messages conveyed by the afflicted individuals. 2 Prior research has focused on enhancing communication from caregivers to individuals with dementia; however, the content and meaning of communication received from individuals with dementia remains largely unexplored. 3 This study demonstrates that individuals with dementia are able to transmit meaningful communication and this communication can be interpreted by others, making client-centered interventions possible with this population. 4 If care models based on communication interpretation techniques can be built, perhaps the needs of individuals with dementia can be better met, tensions reduced, problematic behaviors decreased, and quality of life enhanced.

Milk production and plasma gossypol of cows fed cottonseed and oilseed meals with or without rumen-undegradable protein.

Twenty-four multiparous Holstein cows were randomly assigned at calving to treatment diets using a modified split-plot design to determine the effects of protein source on milk production and composition. The treatment diets consisted of an 80:20 combination of corn and alfalfa silages and whole cottonseed at 12% of the dietary dry matter (DM). The treatment diets were formulated to contain 17% crude protein (CP) and 20% acid detergent fiber on a DM basis. One of the following sources of supplemental CP was included in each treatment diet: 1) cottonseed meal, 2) cottonseed meal plus a rumen-undegradable protein (RUP) supplement, 3) soybean meal, and 4) soybean meal plus an RUP supplement. Cows were fed the initial treatment diet for 6 wk and then were switched to the other oilseed meal source but continued to receive the same amount of RUP during the second period of the study. Milk production and composition were not affected by treatment diet. Cows fed treatment diets without RUP supplementation consumed more DM and thus more CP. Supplementation with RUP resulted in greater milk production efficiency per unit of DM consumed. Cows fed treatment diets containing cottonseed meal had higher plasma gossypol concentrations than did cows fed treatment diets containing soybean meal. Plasma gossypol concentrations for all cows in each group were below the recommended upper limit that is considered to be safe. Data suggest that cottonseed meal in the diet can be substituted for soybean meal, resulting in similar milk production and composition.

Explicating middle-range theory through methodological diversity.

Debates about methodological diversity and the most appropriate paradigm for scientific inquiry are still flourishing within the community of nurse scholars. Rather than continuing to debate these controversies, the authors of this article suggest that nurse researchers set aside methodological differences and move to discover the truths that will build the foundational base of nursing practice and, thus advance the discipline. This article discusses the multiple methodological perspectives used to explicate middle-range concepts from the theory and paradigm for nursing, Modeling and Role-Modeling. Each author will demonstrate how combining different approaches and methodologies can be helpful in exploring and validating middle-range concepts.

Effects of method of colostrum feeding and colostrum supplementation on concentrations of immunoglobulin G in the serum of neonatal calves.

Holstein heifer and hull calves (n = 52) at Ames Plantation (Grand Junction, TN) and Piedmont Research Station (Salisbury, NC) were blocked by sex and assigned randomly to receive 3.8 L of maternal colostrum in one feeding, 1.9 L in two feedings at a 10- to 12-h interval, or 1.9 L in two feedings at a 10- to 12-h interval plus 272 g of colostrum supplement at the first feeding. The colostrum supplement was mixed with 0.95 L of warm water and fed immediately following colostrum. Serum immunoglobulin G (IgG) concentrations were unaffected by the number of feedings and averaged 20.0 and 16.6 g/L at 24 and 48 h, respectively. Calves that were fed the colostrum supplement at the first feeding had lower serum IgG concentrations at 24 h (16.0 g/L) than did calves that were fed two colostrum feedings without supplementation (21.0 g/L); however, serum IgG concentrations at 48 h did not differ among treatments. Dry matter intake and body weight gain were unaffected by feeding method. Calves may be fed high quality colostrum in one or two feedings without affecting IgG absorption.

Effects of the method of calf starter delivery and effects of weaning age on starter intake and growth of Holstein calves fed milk once daily.

Fifty-six Holstein calves were assigned randomly at birth to one of four treatments in a study that evaluated two methods of calf starter delivery (specially designed nipple bottle versus an open container) and weaning age (28 vs. 56 d) on starter intake and growth. Calves were fed 3.8 L of whole milk once daily through nipple bottles until weaning. A pelleted commercial calf starter mix (18.3% crude protein and 19.9% acid detergent fiber) was fed from birth through 56 d. Calves fed starter using the nipple bottle were changed to an open container at weaning. Calves were fed a total mixed diet containing 13.0% crude protein and 25.6% acid detergent fiber from d 57 through 180 for ad libitum intake through a self-feeder. Calves that were weaned at 28 d consumed more calf starter from d 28 to 56 regardless of the method of starter delivery. At 90 d of age, calves that had been weaned at 28 d had similar body weights (BW) and wither heights as did calves that had been weaned at 56 d. Either method of calf starter delivery resulted in similar overall BW, wither height, and average daily gain. Feeding 3.8 L of milk once daily, weaning at 28 d, the use of either method of starter delivery, and feeding a total mixed diet from d 57 through 180 allowed calves to attain BW that exceeded acceptable reported means at 6 mo of age.

Effects of intraperitoneal L-leucine, L-isoleucine, L-valine, and L-arginine on milk fat depression in early lactation cows.

Eight Holstein cows were assigned following calving to two groups, balanced for parity, using a continuous completely randomized block design. Cows were fed a diet with 13.5% CP and 22.4% ADF from 35 to 55 DIM and then 13.8% CP and 15% ADF from 56 to 92 DIM. Alfalfa grass hay was the forage source, and concentrate mixtures contained primarily corn and soybean meal. Cows were given daily intraperitoneal infusions of a solution of L-Leu (46.1 g, 84.2 mM), L-Ile (31.4 g, 57.3 mM), L-Val (38.3 g, 78.2 mM), and L-Arg (25.0 g, 34.4 mM) or physiological saline following the a.m. milking from 42 through 84 DIM. Infusion of AA significantly increased plasma concentrations of Leu, Ile, Val, and Arg. Effects of AA infusion occurred during the low fiber period. Cows receiving AA maintained daily milk fat yield, increased p.m. milk fat yield, decreased less in p.m. milk fat percentage, and increased daily and p.m. yields of C16 fatty acids in milk. During the posttreatment period, cows previously receiving AA declined in daily milk fat yield, milk fat percentage, and total daily C4 to C16 milk fatty acid yield. Results suggest that the infused AA may have increased de novo synthesis of C16 milk fatty acids.

Effect of total and rumen undegradable protein on the performance of cows fed low fiber diets.

Twelve multiparous and 18 primiparous Holstein cows were fed a 17.3% CP, 21.0% ADF diet during wk 2 through 6 postpartum. Cows then were assigned from wk 7 through 14 to one of three low fiber (10.7% ADF) dietary treatments containing either 14.4 or 18.7% CP, the latter with or without a soybean meal enhanced with rumen undegradable protein. Treatments had no effect on milk yield or composition in multiparous cows, although milk fat percentage was not depressed in multiparous cows receiving the low fiber diets. The soybean meal diet enhanced with rumen undegradable protein increased yields of milk, 4% FCM, fat, protein, and DMI compared with the 14.4% CP diet in primiparous cows; it also increased yields of 4% FCM and fat versus the 18.7% CP, untreated diet in primiparous cows. Blood urea N concentrations were greater for high CP diets than for the low CP diet in both parity groups. Rumen acetate: propionate ratios were higher for both high CP diets than for the low CP diet in multiparous cows. Soybean meal enhanced with rumen undegradable protein improved yields of milk and its components in primiparous cows fed low fiber diets, even when high protein diets were fed.

Influence of dietary protein and supplemental niacin on lactational performance of cows fed normal and low fiber diets.

Forty-seven cows (24 primiparous) were assigned to one of four normal (20.5%) ADF diets for wk 2 to 5 postpartum. Dietary treatments in a 2 x 2 factorial design were diets of 13.8 versus 18.8% CP and 0 versus 12 g/d of niacin per cow. During wk 6 to 13 postpartum, cows were fed low (11.8%) ADF diets while maintaining CP and niacin treatments. Low CP diets contained solvent-extracted soybean meal; rumen soybean meal with enhanced undegradable protein was used in high CP diets. High CP diets increased milk protein percentage in multiparous cows and yields of milk, 4% FCM, fat, protein, and SNF in primiparous cows during the normal fiber period. High dietary CP also increased yields of 4% FCM, fat, protein, and SNF in primiparous cows fed normal fiber diets. When switched to low fiber diets, primiparous cows fed high CP diets decreased more in 4% FCM and fat yields than those fed low CP. Primiparous cows fed niacin decreased more in 4% FCM than controls. High dietary CP increased DMI in primiparous cows fed normal fiber diets, but those fed low CP diets increased more in DMI when switched to low fiber diets. Supplemental niacin appeared to interact with dietary CP in multiparous cows, increasing blood glucose and decreasing blood beta-hydroxybutyrate and NEFA concentrations with the high CP, normal fiber diet. Increased dietary CP improved yields of milk and milk components in primiparous cows.

Theory-testing research: building the science.

Lack of emphasis on the empirical validation of nursing models has hindered the development of nursing science. This article explores issues concerning theory development in nursing and criteria for evaluating theory-testing research. From a comprehensive review of the literature on theory testing, 15 criteria are identified as essential for evaluating theory-testing studies. The criteria are applied to selected research reports, and recommendations for future use of criteria are suggested.

The cryopreserved homograft valve in the pulmonary position: mid-term results and technical considerations.

Since September 1985, 60 cryopreserved homograft valves (12 aortic and 48 pulmonary) have been implanted in the pulmonary position in 56 patients. There were 34 primary operations and 26 reoperations. The ages ranged from 15 days to 22 years; the follow-up ranged from 1 month to 5 1/2 years. The operative mortality was 3.3% (2/56). There were two late deaths (not valve related). Five patients underwent replacement of the homograft; in one patient the homograft was replaced with a heterograft valve. Pathological analysis of the explanted homografts revealed calcification of the wall with satisfactory leaflet function. The leaflets were relatively acellular. No evidence of inflammation or rejection was detected. Four explants were performed for nonvalve-related indications. There are 51 long-term survivors with a homograft in place. Forty-eight are in excellent condition; three are mildly symptomatic (not valve related). Benign pulmonary insufficiency murmurs are present in 29 patients. The presence of trivial or mild insufficiency was independent of the technique of implantation. In general, cryopreserved homograft valves function well in the right ventricular outflow tract. Longer term follow-up is necessary to confirm the superiority of the cryopreserved homograft when compared to the porcine heterograft in the right ventricular outflow tract.

Effects of protein level and forage source on milk production and composition in early lactation dairy cows.

Twenty multiparous and 12 primiparous Holstein cows were assigned at calving to one of three grass hay-based diets containing either 14, 18, or 22% CP or an alfalfa hay-based diet containing 22% CP to examine the effect of protein level and forage source on milk yield and composition. The diets contained 23% ADF during wk 1 to 4 postpartum, which was lowered to 11% for wk 5 to 12 postpartum. Cows fed the 18 and 22% CP grass-based diets produced higher yields of milk, 4% FCM, fat, protein, and SNF than those fed the 14% CP diet during the high fiber period. In addition, cows fed the 22% CP grass-based diet had higher milk fat tests than those fed the 14% CP diet during the high fiber period, due primarily to an increase in short-chain fatty acid synthesis. Milk fat depression was more severe when cows were changed to low fiber diets while fed the 22% CP alfalfa-based diet than when fed the 22% CP grass-based diet. Depression in milk fat content was 15.0, 17.0, 15.6, and 27.0% for 14, 18, and 22% CP grass-based and 22% CP alfalfa-based diets, respectively. Cows receiving the 18 and 22% CP grass-based diets exhibited higher blood NEFA during the high fiber feeding period than those fed the 14% CP diet. After fiber was lowered, changes in rumen acetate:propionate ratios were unaffected by treatment. Lowering fiber level resulted in an increased milk CP percentage regardless of treatment. Grass hay appeared to be more effective than alfalfa hay in preventing depression in milk fat test upon the change to a low fiber diet.

A survey of infectious diseases in wild turkeys (Meleagridis gallopavo silvestris) from Arkansas.

Wild turkeys (Meleagridis gallopavo silvestris) trapped as part of a relocation program by the Arkansas Game and Fish Commission were tested for selected infectious diseases and parasites. The 45 birds were trapped at four locations in Pope, Scott, and Montgomery counties (Arkansas, USA). Forty-four blood samples for serology, 27 blood smears and 12 fecal samples were collected. Of the serum samples tested, 20 of 44 (45%) were positive for Pasteurella multocida by enzyme-linked immunosorbent assay (ELISA), 42 of 44 (95%) were positive for Bordetella avium by ELISA, and 15 of 44 (34%) were positive for Newcastle disease virus antibody by the hemagglutination inhibition test. All serum samples were negative for Mycoplasma gallisepticum, Mycoplasma synoviae, avian paramyxovirus 3, avian influenza, hemorrhagic enteritis, Marek's disease, avian encephalomyelitis, laryngotracheitis, Salmonella pullorum and Salmonella gallinarum. Haemoproteus meleagridis was found in eight of 27 (30%) and Leucocytozoon smithi in nine of 27 (33%) blood smears; all smears were negative for Plasmodium hermani. Enteric parasites included Ascaridia dissimilis, Heterakis gallinarum, Eimeria dispersa and Raillietina spp. This study was an attempt to document the health status and disease exposure of wild turkeys in Arkansas to aid in managing and preventing the spread of disease agents to wild turkeys and other species of birds.

Increased left lung consolidation in turkey cholera related to larger left pulmonary artery.

After necropsy of the surviving turkeys in a comparative vaccine study for fowl cholera, in the turkeys with only one lung lobe consolidated, a significantly (P < 0.01, chi square test) higher number of turkeys were found with a consolidation of the left lung lobe than with the right lung lobe. When the circumferences of the left and right pulmonary arteries 0.5 cm rostral from the bifurcation were measured and converted to cross-sectional area, the left pulmonary artery had an average area of 29.6 +/- 2.8 mm2, and the right pulmonary artery had an average area of only 22.9 +/- 2.8 mm2, which was nearly one-fourth smaller. This finding suggests that the left lung lobe receives more blood than the right lobe and that, during an acute Pasteurella multocida septicemia, it would receive more of this organism than the right lung lobe.

High mortality of domestic turkeys associated with Ascaridia dissimilis.

Third- and fourth-stage Ascaridia dissimilis larvae were isolated from commercial white turkey intestinal scrapings from two farms that were experiencing high mortality. Lesions consisted of a necrotic-like enteritis that was most severe in the jejunum. Subsequent bacteriological isolation yielded heavy growth of Escherichia coli and Clostridium perfringens. The rate of mortality declined rapidly when the turkeys were administered 18 ppm fenbendazole for 7 days.

Development of an enzyme-linked immunosorbent assay for Bordetella avium.

A Bordetella avium enzyme-linked immunosorbent assay (ELISA) was developed to detect serum antibodies in 1-day-old poults, experimentally infected turkeys, and naturally infected turkeys. The optimized procedure included use of a suspension of whole bacteria coated onto plastic microtiter plates, a 1:200 serum dilution, a 1:3200 dilution of commercially available goat anti-turkey IgG (heavy and light chain) conjugated with horseradish peroxidase, and 0.04% orthophenylenediamine as substrate. A sample/negative (S/N) ratio method of analysis was used to estimate antibody titer from absorbance values. The regression equation used to estimate antibody titers was derived from the testing of naturally infected turkey sera. The equation was derived by plotting the log10 titer of the sera against the S/N ratio at a 1:200 serum dilution. The ELISA was an effective method for detecting antibody to B. avium, and the procedure should prove useful for laboratories equipped for high-volume ELISA testing.

Hemoglobinuric nephrosis in a rhea (Rhea americana).

An eighteen-month-old female rhea (Rhea americana) was presented dead for necropsy. The owner reported having observed blood in the droppings. Gross examination revealed a rhea in good body condition with a copious amount of frank blood in the cloaca. Large masses of matted fescue grass (Festuca spp.) distended the ventriculus and jejunum. No hemorrhage was evident in the digestive tract, which was otherwise void of ingesta. The kidneys were dark brown in color. Renal histopathology revealed a severe accumulation of eosinophilic pigment in the tubular epithelial cells of the proximal convoluted tubules, with tubular nephrosis and eosinophilic casts in the collecting tubules. It was concluded that the renal pigment was hemoglobin and not myoglobin, based on lack of evidence of muscle injury and the severe erythrophagocytosis evident in hepatic macrophages. The renal pigment also stained positive with the hemoglobin-specific Okajima stain. Hemoglobinuric nephrosis has not previously been reported in an avian species.

Comparison of live avirulent PM-1 and CU fowl cholera vaccines in turkeys.

The live avirulent PM-1 Pasteurella multocida vaccine, grown in brain-heart infusion broth, was evaluated and compared in two experiments with the Clemson University (CU) vaccine, which had been shown to be effective in preventing fowl cholera in turkeys. Experiment 1 was performed during warm environmental temperatures and Expt. 2 during cooler environmental temperatures. The PM-1 vaccine was comparable with the CU vaccine in protecting turkeys against challenge with virulent P. multocida but was considered no less virulent than the CU because turkeys died after vaccination with both the PM-1 and the CU vaccines. A significantly (P < 0.05) higher percentage of unvaccinated turkeys challenged during the cooler environmental temperatures died than did unvaccinated turkeys challenged during the warmer temperatures. A microtiter agglutination test demonstrated a significant (P < 0.01) correlation between the level of serum anti-P. multocida antibody found 1 wk after vaccination and survival after challenge with virulent P. multocida in Expt. 1 and a significant (P < 0.05) correlation between these parameters in Expt. 2. However, there was a significant (P < 0.01) negative correlation between serum anti-P. multocida antibody titer 1 wk after vaccination and body weight gained 4 wk after vaccination, but before challenge, in Expt. 1, suggesting that vaccination with the live vaccines may have had a negative effect on body weight gain. At 4 wk after challenge or 8 wk after vaccination in Expt. 2, there was also a highly significant (P < 0.001) negative correlation between these parameters in the surviving turkeys.

Differentiating turkey postvaccination isolants of Pasteurella multocida using arbitrarily primed polymerase chain reaction.

The chromosomal DNA of 29 field isolants of Pasteurella multocida from commercial turkey farms in Missouri and the avirulent Clemson University (CU) and M-9 vaccine strains of P. multocida were tested using the arbitrarily primed polymerase chain reaction (AP-PCR) in combination with 32P-labeled deoxycytidine triphosphate (dCTP) and high-resolution gel electrophoresis. The 29 field isolants of P. multocida were isolated from outbreaks of fowl cholera in turkey flocks in which vaccination with the CU vaccine had been performed within 2 weeks of the isolation, and it was suspected that the outbreak could have been due to the use of the live CU vaccine. The results of this study showed that: 1) the use of the live CU vaccine can lead to the isolation of the vaccine strain if the outbreak occurs within 2 weeks of vaccination; 2) a higher proportion of field isolants collected during 1983 and 1984, when the usage of the CU vaccine strain was highest on Missouri turkey farms, had PCR-amplified product profiles similar or identical to those of the CU vaccine strain compared with the period between 1987 and 1992, when its use was less; and 3) there was no relationship between the PCR-amplified product profiles and the serotype.
