Pseudokobusine.

The title compound, hetisan-6,11beta,15beta-triol, C20H27NO3, is a hetisane-type diterpenoid alkaloid. It consists of six six-membered rings and two five-membered rings. The fused-ring system contains three chair, two boat, one distorted boat and two envelope conformations. Intramolecular and intermolecular hydrogen bonds are present between the O atoms, with O...O separations of 3.006 (3) and 2.743 (3) A, as well as an O...N intermolecular interaction of 2.887 (3) A.

Distraction method for chronic dorsal fracture dislocation of the proximal interphalangeal joint.

The authors used gradual ligamentous distraction for closed reduction of nine irreducible dorsal fracture dislocations of the PIP joint. The volar fragment size was equal to or less than 40% in five and more than 50% in four. The average follow up was 18 months. The sequence of treatment required distraction, translation, and joint flexion. Overdistraction by approximately 2 mm was necessary to get sufficient soft tissue lengthening. This allowed adequate joint flexion for concentric joint reduction. Distraction and simultaneous mobilization restored the final range of motion earlier than distraction and subsequent mobilization. The final range of motion of the PIP and DIP joints was from 12 degrees to 91 degrees and 0 to 45 degrees respectively.

A salvage procedure in the treatment of the boutonnière deformity caused by contact burn and friction injury.

Contact burns and friction injuries to the dorsum of the finger denude it of full thickness of skin and central extensor tendon tissue, rendering the primary repair of the resultant boutonnière deformity difficult. A salvage procedure that offers both functional restoration and cosmesis is described. Here the unaffected lateral band attachments are transposed en-masses, as a composite flap, to the dorsum of the finger to establish active extension of the proximal interphalangeal joint.

Neural repair for sensory restoration in a groin flap.

A preliminary study of the recovery of sensation in groin flaps, after suture of the lateral cutaneous branches of the twelfth thoracic nerve supplying the groin flap with those of the hand, is described. This appears to be a worthwhile procedure.

Percutaneous internal fixation of fractures of the proximal phalanges.

A rational methods for management of the fractures of the proximal phalanges is presented. By this technique fracture healing and recovery of movement take place concurrently rather than consecutively thereby the morbidity is greatly reduced. It leads to early and complete rehabilitation of the patient in a short period.
