Duration of morning stiffness in rheumatic patients after medication with enteric-coated and plain naproxen tablets.
Twenty-nine adult rheumatic patients who were taking 500 mg naproxen at bedtime and had a certain degree of morning stiffness despite this medication took part in a randomized double-blind cross-over study in which the duration of morning stiffness after evening doses of 500 mg enteric-coated naproxen tablets was compared with that after identical doses of plain naproxen tablets. The duration of morning stiffness was significantly shorter after taking enteric-coated tablets (p less than 0.01), and the mean plasma naproxen morning concentration was 34% higher (p = 0.01). Since the results were unambiguous in such a small group of patients, they are judged to be of considerable clinical value.
Pyometra in the dog--a pathophysiological investigation. IV. Functional derangement of extra-genital organs.
Results on haemoglobin, PCV, serum proteins, blood urea nitrogen, creatinine, cholesterol, bilirubin and some serum enzymes in bitches with clinical pyometra were compared with the degree of polydipsia, anorexia and dehydration in the same patients, and with their plasma progesterone levels. The findings indicate that the pyometra syndrome includes a liver derangement--i.e. cholestasis--in addition to changes in bone marrow activity and renal function as shown previously. Autopsic results also indicate the presence of an endotoxic influence upon the lung function.

An investigation on the pathophysiological mechanisms underlying the hypoalbuminaemia and the hypercholesterolaemia of the pyometra syndrome in dogs was performed on 15 patients. The control material consisted of blood from 15 and urine from 6 healthy dogs. The presence of intrahepatic cholestasis in most of the dogs was indicated by an increase of serum alkaline phosphatases and phosphodiesterase I, and by the morphological demonstration of bile pigment retention in liver biopsy material. Serum cholesterol (total- and free-) was positively correlated with serum phospholipids, alkaline phosphatases and phosphodiesterase I, as regularly seen in cholestatic syndromes. Fibrinogen and alpha 2-globulins were increased in most patients. Albumin was inversely related to these parameters, and the main cause of the hypoalbuminaemia was probably the presence of an "acute phase reaction". The levels of coagulation factors in plasma were not lowered. Enzyme markers of hepatocellular injury (ALAT, OCT) were not increased, but on the contrary significantly lowered. The possible presence of enzyme inhibitors as the cause of the decrease of ALAT and OCT is discussed.
Pyometra in the dog. III.--A pathophysiological investigation. III. Plasma progesterone levels and ovarian morphology.
The peripheral plasma progesterone levels in bitches with spontaneously developed pyometra did not differ significantly from those in normally cycling or pregnant bitches. The macroscopic ovarian morphology gave no indication of an increased corpus luteum activity. Ovarian follicular cysts were found in 4 out of 31 cases. There was no detectable difference of results between 14 bitches that had received hormonal medication 10 years to 1 week previous to the pyometra diagnosis, and the other patients.
Pyometra in the dog. II.--A pathophysiological investigation. II. Anamnestic, clinical and reproductive aspects.
The article deals with the anamnesis and clinical status of pyometra patients at the time of admission for treatment. The average age of 119 pyometra bitches was 7.8 years. About 80 patients were examined for clinical signs of dehydration and hypoperfusion. Such signs were present in up to 80% of the patients, although only about 1/3 seemed weak and in a poor condition. Previous treatment with oral gestagens did not seem to influence the pyometra incidence. However, 14% of the bitches had been given oestrogens (stillbestrol) during the pyometra-associated season.

Arterial pH, oxygen and carbon dioxide tensions, and standard bicarbonate, as well as serum electrolytes (Na, Cl, K, inorganic P, Ca and anion gap) have been investigated in bitches with pyometra. A respiratory alkalosis was a common finding in the patients, and a smaller group showed a metabolic acidosis of mixed origin superimposed on the respiratory alkalosis. In this group 4 out of 9 patients died. In addition, the patients showed moderate electrolyte disturbances indicating a "sick cell syndrome" and a mild to severe renal dysfunction.
