An unusual complication of paracentesis.

Paracentesis is an important and commonly performed procedure in patients with ascites. It is a safe procedure when carried out in the midline below the umbilicus, with a complication rate of less than 1%. We report an instance in which a large midline varix was entered during paracentesis. The utility of different imaging techniques in detecting such anomalies in the portal hypertensive patient with portal hypertension and ascites is discussed. The approach and management of this complication are outlined.

Sonographic detection of foreign bodies of the extremities.

A prospective study to assess the accuracy of ultrasound in the detection and localization of foreign bodies of the extremities was undertaken. Forty-five patients with clinical suspicion of foreign bodies were included in the study. Among 20 patients with radio-opaque foreign bodies, ultrasonic detection of foreign bodies was achieved in 19 patients. In 25 patients, with no radiographic evidence of foreign body, ultrasonic detection of foreign bodies was made in seven patients and among these foreign bodies were confirmed and removed at surgery in five patients. Accurate pre-operative ultrasonic localization of foreign bodies was carried out in three patients.

Ultrasound diagnosis of horseshoe kidney.

Ultrasound diagnosis of horseshoe kidneys depends on the demonstration of an isthmus or band of renal tissue across the midline of abdomen connecting the lower poles of the kidneys. In a number of cases this band of renal tissue may evade ultrasonic detection. In a series of 11 patients with horseshoe kidneys, sonographic detection of renal isthmus was achieved in seven patients, but in all 11 cases there was demonstrable alteration of renal outlines. Instead of normal reniform shape, the kidneys appeared "inverted pyriform" or "inverted triangular". Detection of such an altered renal outline should raise suspicion of horseshoe kidney.

Identification and partial characterization of diagnostically relevant antigens of Aspergillus fumigatus.

Metabolic antigens of Aspergillus fumigatus, soil strain 2605 and sputum isolate, were evaluated for their diagnostic applicability using hyperimmune sera and sera of adults and pediatric patients of allergic bronchopulmonary aspergillosis. An indirect ELISA was standardised by using 2-10 micrograms/ml of coating antigen for detection of specific IgG and IgE antibodies in the sera of patients. The ratios of absorbance for specific IgE and IgG antibodies by ELISA (normal to patients) were observed to be in the range of 1:2 to 1:3 to 1:8 respectively. These antigenic preparations were further analyzed to identify and characterize the individual components by immunoblotting. This analysis indicated the presence of allergenic and antigenic determinants in the antigens of molecular weights 70, 34, and 28 Kd. The utility of the antigens of soil strain for diagnostic purpose is suggested.

Effects of metoclopramide and ranitidine on gastric fluid volume and its acidity.

The effects of metoclopramide 10 mg and ranitidine 150 mg on gastric fluid volume and its pH are studied when given orally either alone or in combination, 3 hours before induction of anaesthesia in adult surgical patients. The study shows that metoclopramide causes reduction of gastric fluid volume but does not elevate its pH. Ranitidine elevates the pH of gastric fluid satisfactorily in all cases, but reduces the gastric volume below the critical level only in 50% cases. A combination of the said two drugs offered most satisfactory protection against acid aspiration syndrome almost in all cases. This regime seems to be superior to either ranitidine or metoclopramide alone in order to reduce the risk factors for aspiration of gastric contents.

A study of some blood genetic characteristics of Bedouin and non-Bedouin Arabs of Jordan.

Blood genetic markers were investigated in a sample of 111 Bedouin and 412 non-Bedouin male Arabs of Jordan. The frequencies of the ABO alleles were similar in these two groups, with a relatively high frequency of the O gene (0.63; 0.61). The frequencies of Rh genes were slightly different in these two groups, with a higher Ro and a lower r in the Bedouins. A sub-sample of a smaller size was investigated for the distribution of haptoglobin types, Tf subtypes and red-cell G6PD (glucose 6-phosphate dehydrogenase) phenotypes. There was no significant difference in any of the systems between the two populations. No haemoglobin variant was detected. The study confirms the Arab characteristics of Jordanians with a small degree of African admixture.

Energy balance study in Singapore medical students.

Energy cost of rest and various common daily activities was measured in 7 male and 16 female medical students by using a Max-Planck respirometer and a Lloyd's gas analysis apparatus. The average weight of male and female subjects during the course of the study was 67.3 +/- 14.2 and 51.2 +/- 5.1 kg, respectively. The average energy expenditure per day was estimated from a diary of activities for a period of 7 days. The average daily intake of energy was computed from food tables and based on weighed amounts of raw materials of food consumed over the same 7-day period when energy expenditure was determined. The average energy expenditure of rest and different activities was found to be significantly lower than the figures published for western subjects. The daily energy intake and expenditure were 2,138 +/- 217 kcal (9.0 +/- 0.9 MJ) and 1,894 +/- 168 kcal (8.0 +/- 0.7 MJ), respectively, in male medical students, giving a positive balance of 244 kcal (1.0 MJ). The values of the same in female medical students were 1,711 +/- 292 kcal (7.2 +/- 1.2 MJ) and 1,474 +/- 211 kcal (6.2 +/- 0.9 MJ) with a balance of 237 kcal (1.0 MJ).

Sonography as an aid to diagnosis of caecal carcinoma in the elderly.

Many elderly patients with suspected colonic neoplasm are referred to radiology departments for barium enema. Despite routine use of laxatives and colonic washouts and employment of antispasmodic drugs, problems occur with faecal residue and poor retention of barium. This may lead to inadequate demonstration of the caecum. Over a 6-month period sonography following the barium enema allowed a confident diagnosis of a caecal mass lesion to be made in five such patients, whose barium enema had been suspicious but suboptimal. It is suggested that in some patients sonography, where positive, may shorten and facilitate investigation.

Energy cost of some common physical activities of Chinese schoolboys.

14 Chinese schoolboys aged 12-14 years, resident in Singapore coming from affluent homes, were tested for the determination of energy cost, pulmonary ventilation (PV) and oxygen (O2) consumption at rest and during some common physical activities by using a Max-Planck respirometer and Lloyds gas analysis apparatus. The study was undertaken for comparison with the results of a similar investigation of Indian schoolchildren of the same age-group living in a hostel under hostel discipline and diet in Singapore reported earlier. The energy cost (kcal/min, kJ/min) in these Chinese children was found to be significantly higher, but the energy cost per kilogram body weight per hour was found to be significantly lower than in the Indian children. PV in liters per minute was significantly higher in Chinese schoolboys during all physical activities except lying at rest, sitting and running. O2 consumption in liters per minute was also significantly higher during all activities except lying at rest and sitting.

A genetic study of the Jordanians.

A total of 543 random samples of blood collected from unrelated male blood donors of Amman, Jordan, were analysed for ABO and Rh(D) blood groups, serum haptoglobin, transferrin and albumin; glucose-6-phosphate dehydrogenase, 6-phosphogluconate dehydrogenase, lactate dehydrogenase; and haemoglobin phenotypes. The results of the study establish the Arab characteristics of the population with no or minimal presence of African genes.

Energy balance study in pregnant Asian women.

Twenty-four pregnant Asian women resident in Singapore were tested for the determination of energy cost of rest and various common daily activities. The average daily energy expenditure was estimated from a diary of activities of seven days. The average daily energy intake was computed from Food Tables on the basis of consumption of measured dressed raw materials of food over seven days in the same period of measurement of energy expenditure. The average daily energy intake and expenditure in this group of pregnant women were found to be 2020 and 1810 kcal (8.5 and 7.6 MJ) giving a balance of +210 kcal (0.9 MJ) per day.
