Hospital records as a data source for occupational disease surveillance: a feasibility study.

To assess the feasibility of using hospital records for occupational disease surveillance and to evaluate the quality of the industry/occupation (I/O) information available in these records, the computer file of all discharge diagnoses from a large health maintenance organization during 1985 was reviewed. The frequencies of discharge diagnoses previously listed as Sentinel Health Events (Occupational), or SHE (O), were calculated and three possible SHE(O) diagnoses--lung cancer, bladder cancer, and toxic hepatitis--were selected for further review. Outpatient charts of patients discharged for each diagnosis were abstracted with regard to I/O information and the discharged patients were interviewed by telephone to obtain a lifetime occupational history. The accuracy of the I/O information obtained from the hospital chart was compared to that obtained by patient interview by number of digits matched on standard classification codes. The frequencies of matches for occupation and industry were greater for "usual" than for "last" categories with both cancer diagnoses, but were similar for "usual" and "last" categories with toxic hepatitis. To assess the proportion of each possible SHE(O) diagnosis that was related to workplace exposures, the I/O information obtained by interview was rated in a blinded fashion by an experienced occupational medicine physician. The highest probability ratings for work-relatedness were noted for lung cancer, primarily due to asbestos exposure. The results of this study suggest that hospital records can be used to identify possible SHE(O); if adequate I/O information is available, then work-relatedness can be assessed. However, the accuracy of I/O obtained from hospital charts is relatively low. The efficient and accurate collection of I/O information from hospital records will require the use of a simple, easily coded instrument to be routinely administered on admission.

Selective synthetic ligands for human nuclear retinoic acid receptors.

From a series of naphthalene and benzoic acid derivatives we have identified synthetic retinoic acid analogues exhibiting high selectivity for the nuclear retinoic acid receptors RAR alpha (Am 580), RAR beta (CD 2019) and RAR gamma (CD 437) as well as ligands sharing high affinities for all RAR subtypes (CD 367). The compounds were evaluated in two complementary screening systems: (1) binding to nuclear proteins extracted from COS-7 cells after transfection with the appropriate expression vectors, and (2) induction of plasminogen activator in the embryonic mouse teratocarcinoma cell line F9. All compounds behaved as retinoic acid agonists in the F9 test.

Development of enzymes of glycolysis and gluconeogenesis in human fetal liver.

The activities of two key enzymes of glycolysis and two key enzymes of gluconeogenesis were measured in liver samples from 44 human fetuses ranging in gestational age from 20 weeks to term, from infants to 10 years and from adults from 21 to 58 years. Specific activities of both gluconeogenic enzymes, fructose-1,6-biphosphatase and phosphoenolpyruvate carboxykinase, increased throughout the period of fetal development examined, and rose to near adult levels after birth. The activities of both glycolytic enzymes, phosphofructokinase 1 and pyruvate kinase, were lower in fetal than in pediatric and adult samples. For both of these enzymes, there was a significant reduction in activity of livers from fetuses of 34-37 weeks' gestation. Both enzymes showed hyperbolic kinetics at 24 weeks' gestation, but this changed to sigmoid kinetics during the 34-37 weeks' period of low activity. The data indicate that during the last weeks of gestation, inhibition of the activities of these two glycolytic enzymes, coupled with the rise in the two gluconeogenic enzymes, may reflect a change in liver from a primarily glycolytic role in the first two trimesters to a gluconeogenic role shortly before birth.

[Severe psychiatric disorders associated with severe acute colitis].

Acute colitis occurring in patients suffering from psychiatric illnesses is believed to be linked to the intake of psychotropes. From 1983 to 1989, the authors observed, in three Hepato-gastroenterology units, 10 cases of acute colitis in patients suffering from serious psychiatric disorders, most of them inpatients of mental hospitals. The detailed study of 7 of these cases emphasized a certain number of common features: there was no previous history of digestive disease, the psychiatric illness was serious and longstanding, acute colitis was severe, and there was no recurrence during clinical and endoscopic follow-up averaging 4.3 years. Of these 7 patients, 2 were not taking psychotropes at the time of colitis or after, 2 had discontinued their treatment for a few days, and 2 had not stopped taking psychotropes. One patient died. The short-term and long-term evolution in these cases was not influenced by the intake or not of psychotropes. The pathogeny of this colitis is yet to be determined: infection is the most likely origin.

Spontaneous dialytic ultrafiltration with intraperitoneal reinfusion of the concentrate versus large paracentesis in cirrhotic patients with intractable ascites: a randomized study.

Dialytic ultrafiltration of ascites through a hemofilter associated with peritoneal reinfusion (DUF) of the concentrate has been proposed for the treatment of refractory ascites. In five cirrhotic patients, 18 ascites evacuation procedures were randomized either to DUF (n = 8) or to large paracenteses (LP) (n = 10). The effects of these two methods on hemodynamic and renal function were assessed. After DUF, the protein concentration in ascites increased transiently from 28 +/- 7 g/l to 64.8 +/- 8 g/l (p less than 0.04); urinary output increased from day 1 to day 4 (1000 +/- 100) VS 1430 +/- 140 ml/24h; p less than 0.02). After LP, ascitic protein concentration and urinary output were unchanged. No side effects were observed with the two methods. The mean amount of albumin infused was 20 +/- 15 g after DUF and 15 +/- 5 after LP (ns).

Spontaneous dialytic ultrafiltration with intraperitoneal reinfusion of the concentrate in 15 cirrhotic patients with intractable ascites.

The clinical efficacy and tolerance of dialytic ultrafiltration of ascites through a hemofilter (DUF) with peritoneal reinfusion of the concentrate was evaluated in 15 cirrhotic patients with intractable ascites. All together, 51 DUF procedures were carried out. An average of 8.6 was ultrafiltered during 12 h with no significant change in blood pressure, hemoglobin, coagulation parameters or plasma creatinine. A significant increase in ascitic protein concentration was observed immediately after the procedure and a slight but significant increase in 24 h urinary output. A controlled evaluation of DUF compared to large paracenteses seems to be justified by these preliminary results.

Transition from cytosolic to mitochondrial thymidine kinase during development in human fetal tissues.

The transition from cytosolic ("fetal") to mitochondrial ("adult") thymidine kinase, as detected by electrophoresis, was examined in six human fetal tissues of gestational ages 11-40 weeks. In all tissues there was an early period during development in which only the fetal form was detected, followed by a transitional period in which both fetal and adults forms were present, followed by a later period in which only the adult enzyme occurred. Transitional periods were 23-25 wk. gestational age for colon, 13-15 wk. for kidney, 18-20 wk. for liver, 14-18 wk. for lung, 34-36 wk. for serum, and 25-28 wk. for thyroid. In all cases, only the adult form was present by the time of birth and persisted during the first 18 months of extrauterine life. The adult form, but not the fetal form, was inhibited by dCTP.

Blood pressure and blood lead concentration in bus drivers.

San Francisco bus drivers have an increased prevalence of hypertension. This study examined relationships between blood lead concentration and blood pressure in 342 drivers. The analysis reported in this study was limited to subjects not on treatment for hypertension (n = 288). Systolic and diastolic pressures varied from 102 to 173 mm Hg and from 61 to 105 mm Hg, respectively. The blood lead concentration varied from 2 to 15 micrograms/dL. The relationship between blood pressure and the logarithm of blood lead concentration was examined using multiple regression analysis. Covariates included age, body mass index, sex, race, and caffeine intake. The largest regression coefficient relating systolic blood pressure and blood lead concentration was 1.8 mm Hg/ln (micrograms/dL) [90% C. I., -1.6, 5.3]. The coefficient for diastolic blood pressure was 2.5 mm Hg/ln (micrograms/dL) [90% C. I., 0.1, 4.9]. These findings suggest effects of lead exposure at lower blood lead concentrations than those concentrations that have previously been linked with increases in blood pressure.

[Cholesterol, arterial pressure and cigarette smoking in the Swiss population: the MONICA project].

Elevated plasma cholesterol, high blood pressure and cigarette smoking are three major risk factors for coronary heart disease. Within the framework of Switzerland's participation in the multicenter study MONICA (MONItoring of trends and determinants in CArdiovascular disease), proposed by the WHO, a first risk factor survey was conducted in a representative sample of the population (25-74 years) of two reporting units (cantons of Vaud and Fribourg, canton of Tessin). A high blood cholesterol level (greater than 6,7 mmol/l) is the most common risk factor for coronary heart disease among the studied population. Among men, about 13% have elevated blood pressure, the proportion being about one in ten among women; these proportions increase with age and are slightly above these values in Tessin. Cigarette smoking is still a common behavior; between 25 and 45 years one third of the population (male and female) regularly smoke cigarettes.

[Body weight, food habits and physical activity in the Swiss population: the project MONICA].

Within the framework of the first risk factor survey among the two reporting units of MONICA-Switzerland (cantons of Vaud and Fribourg; canton of Tessin), body mass index (BMI) was also analysed. One in four adults (25-74 years) presents with overweight, this proportion being even 1 in 3 among males of the canton of Tessin. However, this health risk can probably be changed, as every third female and every fifth male indicated to have modified their nutritional habits in a more healthy sense within the last 12 months preceeding the survey. On the other side, less than half of the population engages in a degree of physical activity conducive to cardiovascular prevention and reduction of overweight. In order to increase the level of physical activity in the population, it is primarily leisure time sports activities that have to be developed as only very few adults experience a heavy physical work load within their profession.
