Early diagnosis of acute rupture of the quadriceps tendon by arthrography.

Arthrographic study of the knee was performed in six patients for diagnosis of acute rupture of the quadriceps tendon. All of the patients gave a history of attempting to recover from a fall. Pain and tenderness over the suprapatellar area were common findings in all patients. Four patients had a positive arthrogram demonstrating extravasation of the radiopaque material outside the knee along the quadriceps tendon. Of these four patients, two had complete loss of quadriceps function, but the other two were able to partially extend the knee when the gravity was eliminated. Two patients had negative arthrograms; both had loss of active extension of the knee against gravity but were able to extend the knee when the gravity was eliminated. All four patients with a positive arthrogram had rupture of the quadriceps tendon, which was demonstrated at the time of surgery. In one patient with a negative arthrogram, exploration of the knee revealed an intact extensor mechanism. Arthrography of the knee is an aid to the diagnosis of rupture of the quadriceps tendon and when indicated can be performed in the emergency room.

Patellofemoral replacement.

From an analysis of the clinical evaluation of 85 cases and subsequent revisionary procedures, patients for isolated patellofemoral joint replacements should be carefully selected. At the present time, the primary indications are distortion or severe degeneration of the femoral groove.

Retained posterior horn of the medial meniscus.

The incidence of a symptomatic retained posterior fragment of the medial meniscus in 2 different groups of patients with medial meniscectomy is compared. If the original surgery was done through one capsular incision, there was a 9.5% incidence of symptomatic retained posterior fragment as compared to 0.2% incidence if the original surgery was performed through 2 capsular incisions. For total excision of the meniscus medial meniscectomy should be performed through 2 capsular incisions.

The Synovial shelf syndrome.

A syndrome complex mimicking a tear of the anterior horn of the medial meniscus consists of absence of effusion with negative arthrographic and arthroscopic examinations. Because of a thickened band of the medial synovial shelf this entity is termed the "medial synovial shelf syndrome."

Extra-articular stabilization of the knee joint for anterior instability.

The results after one year or longer were evaluated in 76 patients with extra-articular reconstruction for functionally disabling anterior instability of the knee joint. The reconstruction was performed by a modification of the Ellison procedure for treatment of anterolateral rotatory instability. The various superimposed components of knee instability were managed by a variety of methods. Postoperatively,objective stability correlated well with subjective improvement. Patients with 2+ or greater anterior instability preoperatively had disappointing results in both subjective symptoms and objective signs. although extra-articular reconstruction may be sufficient for lesser grades of functionally disabling instability, knees with grade 2-3+ instability may require direct anterior cruciate ligament reconstruction in addition to the extra-articular surgical procedure.
