Catatonia with high CSF lactate in a case of diabetes with associated conditions.

A case of reversible catatonia in a well controlled insulin-dependent diabetic is described. The course of catatonia was characterized by very high CSF lactate values (serial semiautomatic determinations) during more than one month, beyond the clinical recovery. The CSF lactate elevations seem to reflect cerebral hypoxia. The uncommon coincidence of diabetes with cerebral atrophy, mental weakness, and perceptive deafness migh suggest the classification of this case of diabetes in the group 'associated with certain conditions and syndromes'.

Arterial thrombosis of the arm in infant of diabetic mother.

A case of multiple arterial thromboses of the arm in the infant of a diabetic mother is reported. A hemocoagulative pattern of intravascular disseminated coagulation was present. Conservative thrombolytic treatment with Urokinase and Calciparin proved successful.

ACTH 1-17 effects in intrinsic bronchial asthma.

We have studied the effect of Synchrodyn 1-17 (100 micrograms/day/15 days, intramuscularly) on 12 patients with intrinsic asthma, presenting a circadian pattern in several functional parameters related to the bronchial patency (and/or with bouts of nocturnal asthma). Treatment with ACTH 1-17 had two main effects: the pathological rhythms of the bronchial tone disappeared and there was a clear general improvement in the indices of bronchial patency. The drug was well tolerated. Euphoria and water retention were the side effects: they were noticed in a variable number of subjects and disappeared after discontinuation of treatment.

[Evaluation of diabetic glycemic control in hospital and extra-hospital regimen (author's transl)].

It is suggested an evaluation of the degree of diabetic control in insulin-dependent "difficult" patient, during the period following the discharge from hospital. In 13 cases, the quality of glycemic regulation was compared in intra- and extra-hospital therapeutic regimen by means of 3 new indices of the diurnal blood sugar profile: the Mbs, delta, and Mdelta values. Mbs value describescribes the mean level of the blood sugar curve; delta indicates the amplitidue of the glycemic variability field; Mdelta reports both maximal and minimal extremes of the curve. A combination of the first two parameters proved adequate for a clinical assessment of diabetic control. In the majority of the cases, blood glucose patterns were found similar or even improve in extra-hospital conditions. The insulin dosage ordered during hospital staying was confirmed in all, but one, patients. Psychological approach is probably essential for such good results. The procedure seems to be suitable in order to establish a more correct starting point for an effective treatment of the diabetic out-patient.
