Lasers and skin disease. What works and what doesn't.

Lasers have been used in surgery for over 20 years. Their application has expanded enormously due to a better understanding of laser/tissue interactions and the development of sophisticated delivery systems. Resurfacing techniques using a carbon dioxide laser in the treatment of acne scarring and wrinkles has proved to be a very effective tool. Certain cutaneous vascular malformations, benign and malignant skin lesions, as well as pigmented skin lesions and decorative and traumatic tattoos are now successfully treated with lasers with minimal or no scarring.

Upper lip reconstruction: use of the free superficial temporal artery hair-bearing flap.

The use of the superficial temporal artery hair-bearing flap applied to the repair of burn injuries to the upper lip of two male patients is described. It is a relatively simple method of providing tissue to the injured lip with advantages over traditional methods in that it is single-staged and provides the possibility for hair growth in the form of a camouflaging moustache. Donor site morbidity is minimal. When combined with additional reconstructive procedures in the multiply-injured male face, it provides a reasonable expectation for the patient of an acceptable appearance.

Replantation of an excised segment of radial forearm tissue.

A report on a case of successful replantation of an excised segment of forearm tissue is presented. An 18 year old man was admitted to hospital with a completely excised segment of the radial side of his forearm and wrist as a result of a circular saw injury. The best reconstructive option was replantation of the excised radial forearm tissue thus providing both soft tissue cover and bony stabilization.

Vascularized innervated transfer of the clavicular head of the pectoralis major muscle in established facial paralysis.

Twenty cadaver dissections of the clavicular head of the pectoralis major muscle were carried out to determine the detailed anatomical arrangement of the vascular and nerve supply, with a view to using this muscle as an independent neuromyovascular unit in the treatment of established facial palsy. In all dissections, the muscle was found to have an independent nerve and vascular supply. A clinical case is reported with anatomical description and essential details of the operative technique. The ideal neuromyovascular unit is briefly described and the advantages of the pectoralis major for this use are illustrated. It is emphasized that limiting the goals of free muscle transfer to reanimation of the lower facial musculature will produce the most satisfactory results.

Digital avulsion injuries: the shish kebab effect of the fibrous flexor sheath.

On debriding a hand from which four fingers with all their flexor tendons had been avulsed, a sizeable quantity of 'minced' muscle was found in the palm. Further cadaver investigations showed that the avulsed flexor tendons disrupted proximal to the musculo-tendon junction, and that a significant amount of devitalized forearm muscle tissue was left in the palm as the tendons passed through the proximal fibrous flexor sheath. A case of early development of carpal tunnel syndrome following avulsion injury of the little finger is illustrated in which later surgical exploration of the palm revealed detached muscle tissue compromising the median nerve. It is suggested that exploration of the palm is indicated in all cases of proximal avulsions of the flexor tendons.

Male pseudohermaphrodite reared as a female.

A 31-year-old male pseudohermaphrodite has been brought up as a female. The parents had told their child not to discuss the problem of ambiguous genitalia with anyone. As a teenager masculinization had occurred. Chromosome analysis of both peripheral blood and gonadal tissue was 46XY. The internal gonads were testes surrounded by a rim of ovarian stroma. A rudimentary cervix opened into a small vagina, as did the urethra in a valve-like fashion. The phallus was amputated during surgical reconstruction of the external genitalia to create a phenotypic female and the vagina was enlarged. Oestrogen therapy resulted in breast development.

Classification and evaluation of the functional results of replanted parts of the hand at the Prince of Wales Hospital and the Prince of Wales Children's Hospital: 1984 to 1988.

This paper describes a simple method of classification and evaluation of the functional results of replanted and revascularized parts in the hand. The results are presented in graphic form and have been analyzed to correlate various factors: injured part, cause, and zone (level) of injury. The type of injury, ischemic time and age have been studied in more detail to determine their influence of the final functional result. The series contains 187 amputated and devascularized parts of the hand in 119 patients who have undergone surgery at the Prince of Wales Hospital from 1984 through 1988. The length of cold or warm ischemic times, up to 16 hours in this series, while not affecting survival of the amputated part, does adversely affect the functional result. The survival rate of replanted parts in children was significantly less favorable than in adults, but the functional results were uniformly superior.
