The effect of written reassuring information on out-patients complaining of tinnitus.

The study investigated the effect of a two-page leaflet designed to reassure patients complaining of tinnitus. One half of new attenders at a tinnitus clinic received the leaflet while the other half did not. Clinic staff were blind to the condition to which patients were assigned. Tinnitus distress was assessed at the second appointment by means of a standardized questionnaire. Results indicated that questionnaire scores for the informed and non-informed subjects did not differ. However the informed group reported having significantly fewer 'unfulfilled needs'. They were less likely to want a medical cure or professional guidance. The level of general emotional distress as measured by the General Health Questionnaire did not moderate these effects.

The influence of protein binding upon tissue fluid levels of six beta-lactam antibiotics.

The effect of protein binding upon the penetration of six-beta-lactam (three penicillins and three cephalosporins) antibiotics into tissue fluid was studied in humans. A cantharides blister technique was used. It was found that there was a linear relationship between the percentage of protein binding and the penetration into the blister fluid of the antibiotic as measured by the area under the curve of the protein-free fraction. This finding is further evidence that protein binding may have important influence upon the likely efficacy of an antimicrobial agent.

Functional significance of antibody formation after long-term salmon calcitonin therapy.

Twenty patients with Paget's disease were challenged with porcine (PCT) and salmon (SCT) calcitonin before and after 6 months SCT therapy. Acute hypocalcaemia was clearly related to the prevailing rate of bone turnover but responses to PCT showed that treatment had not disturbed this relationship. Although antibodies to SCT developed in eight patients during the period of treatment the acute hypocalcaemic response to SCT was not diminished in relation to bone turnover. It is suggested that antibodies which develop during the course of SCT therapy do not necessarily exert a functionally effective neutralizing action.

Pharmacokinetics of Bay k 4999, a new broad-spectrum penicillin.

The pharmacokinetics of the broad-spectrum penicillin Bay k 4999 were studied in six healthy male volunteers. A 2-g dose was given by the intravenous route. The tissue penetration of the antibiotic was studied by both dermabrasion and blister techniques. A total of 26.4% of the drug was recovered in the urine in 24 h, 79% of this being excreted in the first 2 h. The elimination half-life in serum was 1.3 h. The dermabrasion levels of Bay k 4999 were generally similar to those in serum, but after 1 h the blister fluid levels of antibiotic were greater than those in serum. Different drug levels obtained by blister and dermabrasion techniques may be due to the different composition of the two fluids.
