[Cystic kidney disease in the adult. Update].

Genetic changes on the extracellular matrix of nephron basal lamina seem to underlie the pathogenesis of renal cystic disease, as also confirmed by experimental results. The cysts behave as dynamic structures and repeat some physiological features of the nephron segment which gave origin to them. These properties reflect, for instance, on the pharmacokinetics of antibacterial drugs. New imaging techniques detect more and more asymptomatic cystic formations of very little volume. Moreover, a better definition of some complications, such as haemorrhage and infections, is afforded by the RX- and RM-CT and the use of specific radiopharmaceuticals (67Ga-citrate, 99mTc-sulfur-colloids, 111In-labeled leucocytes).

[Immunoscintigraphy in the diagnosis of choroid melanoma].

This study was aimed at evaluating the role of 99mTc-labelled anti-melanoma antibodies as in vivo tumor markers in the diagnosis of uveal melanomas. Usually, neither conventional semeiological techniques nor more recent diagnostic imaging methods allow a differential diagnosis to be made of benign and malignant intra-ocular tumors. Immunoscintigraphy was employed to examine 9 patients with monolateral reduction of sight, and photopsy. All patients were previously submitted to funduscopy, fluorescein angiography, and US. Four hours after iv injection of 370 MBq of 99mTc F(ab1)2, we obtained 500,000-count images of skull, abdomen, and thorax, with a computerized gamma camera equipped with a general-purpose collimator. A SPET of the head followed the conventional examination. Our first results seem promising enough for further study to be carried out to assess immunoscintigraphy specificity in the diagnosis of uveal melanoma.

[Tumors of the adrenal medulla: nosologic profile and diagnostic technics].

A nosologic re-arrangement of medullo-adrenal neoplasms has been looked for in this article, with particular regard to their common relationship to the complex histogenic system descending from the neuroectoderma-neural crista. The morpho-biologic features of these clinical entities are also reviewed, as well as the up to date laboratory and imaging techniques, with a stress on the high sensibility and specificity levels afforded, e.g., for some of these tumours by selected radio-isotopic examinations, due to their histogenetic, thereby biologic peculiarities.

A study of gastric kinetics (comparison between different isotope carriers).

From the various investigations of gastric emptying described in the literature, radionuclide labelling of meals appears to provide the most suitable technique. The emptying patterns of different 99mTc-DTPA carriers were studied in healthy volunteers, compared with patterns found after ingestion of a fried egg incorporating 99mTc-serum albumin. The results are described and discussed. They seen to suggest that there are different modalities of gastric emptying when different meals and markers are used.

[Conservative treatment of splenic injuries in childhood].

From 1978 to 1983, 11 children with splenic traumas were observed at the Pediatric Surgery of Parma. The diagnosis was confirmed by splenic scintigraphy with 99m Technetium. Those patients whose condition was stable with intravenous fluids or blood transfusions were not operated on. Emergency laparotomy was necessary only in 3 cases of 11 patients, when the general conditions were unstable.
