Timolol versus pilocarpine separately or combined with acetazolamide-effects on intraocular pressure.

Fifty-eight patients with intraocular hypertension or primary open angle glaucoma participated in a double masked randomized study. Timolol in concentrations 0.25% and 0.5% was compared with 1, 2, or 4% pilocarpine Acetazolamide (250 mg x 3) was added if intraocular pressure (IOP) was uncontrolled with the highest concentrations tested. No statistical difference was found in hypotensive effect between pilocarpine and timolol neither on ocular hypertensions nor glaucomas. The additive hypotensive effect of acetazolamide was the same for both substances. Once a day by administration of timolol was sufficient in 17 of 20 cases controlled merely by topical administration.

Ocular complications in recipients of kidney transplants.

104 renal transplant recipients have been examined regarding ocular complications. A high incidence of posterior subcapsular cataracts has been found, increasing with time after operation. Only 2 cases of open angle glaucoma were found, indicating a seemingly small risk for developing a corticosteroid-induced glaucoma in such patients. A high frequency of treatment-resistant herpes simplex keratitis, probably due to reduced corneal resistance, was found.

Melanotropic drugs and retinal functions. II. Effects of phenothiazine and rifampicin on the sheep ERG.

The acute effects of chlorpromazine, promethazine and rifampicin on the a-, b- and c-waves of the conventional electroretinogram (ERG) were studied in sheep. Iv administration of chlorpromazine and promethazine resulted in a b-wave amplitude decrease and an initial c-wave amplitude decrease, followed by cyclic amplitude changes resembling damped oscillations. Iv injections of rifampicin, however, resulted in cyclic changes of the c-wave amplitude without initial concomitant b-wave changes. The results of the present study indicate that rifampicin has a selective influence on the pigment epithelial cells, while chlorpromazine and promethazine seem to have more generalized retinal effects on both the neuroretina and the pigment epithelial cells.

Absorption of ocular timolol.

The absorption of timolol after topical administration to the eye was studied in 11 healthy volunteers and in 5 patients with bilateral open angle glaucoma. The volunteers received 2 drops of 0.5% timolol ophthalmic solution in each eye, equivalent to 0.8 mg of timolol maleate. Overflow was absorbed by paper tissue. The patients received the same dose twice daily for 2 weeks and were then investigated. In the paper tissue, 12 to 88% of the dose administered was recovered. Plasma concentrations were never above 5 ng/ml and not always detectable (detection limit 1 to 2 ng/ml). However, timolol was absorbed as the drug was found in the urine samples from all volunteers and glaucomatous patients.

Reduction of hepatic arterial flow by degradable microspheres in patients with liver tumor.

Ten patients with primary or secondary liver tumor were subjected to preoperative angiographic mapping of the hepatic circulation, combined with infusion of microspheres (Spherex Infusion). During laparotomy, 22 electromagnetic measurements of hepatic arterial flow were made and 2-40 ml of microspheres were infused at a rate of 2-16 ml per min over 1-4 min. Liver biopsies were taken. Hepatic arterial flow was reduced in a dose-dependent manner by a mean maximum of 67% following 8 ml at a rate of 2 ml per min. The reduction had a mean maximum duration of 10.4 min from end of infusion. At microscopy of liver specimens taken during maximum flow reduction, microspheres under degradation were seen. Angiography was of very limited value for estimating flow reduction. One patient experienced severe side-effects due to shunting. It appears rational to use microspheres together with cytostatics, provided that these are incorporated into hepatic tumor cells during a five min reduction by 50% of hepatic arterial blood flow.
