Vitamin B-12 induced acnes.

A type of acne induced by vitamin B-12 deserves a special place among acneiform eruptions. The eruption is monomorphic and of a particular type. It consists of voluminous folliculitis lesions which develop acutely after the first injections of vitamin B-12 and disappear rapidly when treatment is discontinued. The etiologic and pathogenic mechanisms of the disease are not know.

Acne Vulgaris

[Acne-like eruption induced by pseudo-xanthoma elasticum (author's transl)].

Pseudoxanthoma elasticum can induce acne-like eruption. The lesions are located on the latero-cervical parts and consist of comedones, cysts and inflammatory nodules. Histologically, there are no transepithelial eliminations or follicultitis, but a very important granulomatosus tissue with epithelioid cells and a giganto-cellular reaction with elastotic fibres phagocytosis. These findings are different from the cases with association of P.X.E. and elastosis perforans, but they are closely related to it. We think that these features are of a very great interest for the dermatological practice.

Chequered localized pseudoxanthoma elasticum: a variety of Christensen's exogenous pseudoxanthoma elasticum?

We report a very curious case of a condition which has never been described before. Its features are comparable but not quite identical with those of Christensen's saltpetre-induced PXE which the author considers to be an exogenous variety of pseudoxanthoma elasticum. Since our case appeared spontaneously without any accidental episode we prefer to designate it under the name 'localized PXE'. The chequered appearance of the lesions seems to be the characteristic feature of the disease.

Cockade-like vitiligo and linear vitiligo a variant of fitzpatrick's trichrome vitiligo.

The authors report 2 cases of atypical vitiligo in which they observed 1) "cockade-like" lesions resembling those of "trichrome" vitiligo (from the centre to the periphery, achromic area, hypochromic ring, normal or hyperchromic border), 2) numerous linear achromic lesions corresponding to former excoriations (Koebner's phenomenon, isomorphic phenomenon). Related affections are discussed with reference to these cases; the trichrome vitiligo described by Lerner and Fitzpatrick, and the primary leukomelanodermas described in black patients by Basset and by Sarrat and Nouhouayi.

[Vertical striated sand-papered twenty-nail dystrophy (author's transl)].

Vertical striated sand-papered twenty-nail dystrophy, an entity observed in both children and adults, is suggestive of an expression of alopecia areata. When the vertical striated sand-papered effect is not uniformly distributed over the full surface of the nail plates the disorder may be caused by lichen planus.

[Acquired fibro-keratoma. Report of 8 cases].

The acquired fibro-keratoma is a benign fibro-epithelial tumor of acquired nature. I was set apart as a distinct entity in 1968 by Bart et al. who named it "acquired digital fibro-keratoma". It is predominantly, although not exclusively, located on the fingers and toes, near the phalangeal joints. The authors report 8 personal cases and make a short general review of the disease. The lesion is slightly prominent, well defined and surrounded by a hyperkeratotic collar. It protrudes on the surface of the skin much like a hernia. Histologically, it is a dermo-epidermal tumor. There is a proliferation of connective tissue under the papillomatous, hyperacanthotic and orthokeratotic epidermis. The mature collagen bundles, voluminous fibroblasts and numerous enlarged capillaries are parallel to the vertical axis of the tumor. The acquired fibrokeratoma must be differentiated from other cutaneous tumors, particularly fibromas and Koënen's tumors. The etiology is unknown. However, it was found in some cases that a trauma had contributed to the development of the tumor. The only possible treatment is surgical excision.

[Hyperkeratosis punctata of the palmar creases].

The authors have collected 6 cases (one personal case and 5 cases from the literature) of hyperkeratosis punctata of the palmar creases, an affection which seems to be a new type of keratosis punctata. It is characterized by the development of small horny plugs in small cup-like depressions which are electively and exclusively found in the palmar flexion creases of the fingers. Histological examination shows a plug-like hyperkeratosis penetrating, more or less deeply, the depressed, funnel-like Malpighi's mucous layer which sometimes consists of one layer of cells. This affection might be compared to Kyrle's disease (and the hyperkeratotic plugs of mechanical origin). The histological evidence of non-rupture of the dermis by the horny plugs may not be considered as a sufficient factor of differentiation: however, other clinical criteria differentiate these various affections. In the absence of a known etiology we must conclude that this type of keratosis punctata confined to the palmar creases is an autonomous disease.

Inflammatory linear verrucose epidermal nevus. A pathologic study.

We saw three patients with inflammatory linear verrucose epidermal nevus (ILVEN). The purpose of this study is to better delineate the histopathologic features of this type of nevus. In fact, in all three cases very particular, specific histologic lesions have been noted that permit us to better distinguish this recently outlined entity. The histologic features were depressed, cup-like areas of hyper-granulosis with overlying orthokeratotic hyperkeratosis, alternating (with sharp dermacation) with raised, level areas of agranulosis with overlying parakeratotic hyperkeratosis.
