[3D-CT and angiography of cast preparations of pelvic vessels: demonstration of arterial blood supply of the acetabulum].

Anatomical studies of the arterial blood supply of the acetabulum were performed utilizing conventional angiography and 3D-CT on 27 hip specimens. In 16 cases the arterial arborization was filled with undiluted barium-sulfate via the internal artery and in 11 cases with a solution of resin and lead power. 3 specimens out of 30 were excluded because of extravasation using barium-sulfate in one case and incomplete filling with resin in 3 cases. Conventional angiograms were performed in all specimens and in preparations with resin/lead power additional three-dimensional reconstructions of the surface of the acetabulum and arterial cast were done with 3D-CT. 3D-CT shows precise relationship of the arteries to the acetabulum in any spatial plane and definition of vascular supply without elaborate preparation. 3D-CT is superior to non-selective conventional angiography because of better definition of arterial arborization. Filling with resin and lead powder is optimal for anatomical studies because of its reliability and stability. It permits comparative 3D-CT and conventional x-ray examination and subsequent preparation of the vessels in corrosion technique. The advantages of 3D-CT in vessels of cadaver specimens should encourage research on its viability in patients.

Acetabulum

Effects of human alpha-interferon on granulocyte-macrophage progenitor cells (CFU-GM) in vitro.

Two preparations of human interferon (IFN)-alpha were assessed for their influence on granulocyte-macrophage progenitor cells (CFU-GM) in vitro. Both highly purified human IFN-alpha Ly and recombinant IFN-alpha 2a suppressed CFU-GM colony formation in a dose-dependent manner using low-density bone-marrow target cells. Suppression of CFU-GM colony formation was accompanied by an increase in clusters. However, depletion of monocytes, T lymphocytes and B lymphocytes from low-density bone-marrow cells resulted in insensitivity of progenitor cells to IFN-alpha. These results demonstrate that the effects of human IFN-alpha on myeloid progenitor cells (CFU-GM) are mediated by accessory cells within the bone marrow.

[Operative long-term results in delayed radius head dislocation in childhood].

Only some cases and no long term follow-ups are reported in literature concerning the problem of delayed luxations of the radial head in children. 21 cases (19 acquired and 2 inherited) were treated in our hospital by operative procedure 12.5 months after the accident on the average. Disregarding one early and one late resection of the radial head we reconstructed the joint reposition and annular ligament plasty and if necessary correction osteotomy of the ulnar or shortening of the radius. Follow-up examination took place between 2 and 16 years after the operation, 7.9 years on the average. A reluxation was observed in 2 cases, a subluxation in 1 case. 2 patients showed a wrong axis and 4 patients signs of a beginning radio-humeral arthrosis, but most results were good or satisfactory. Therefore a reconstruction of a luxation of the radial head in children is recommended in delayed cases (sometimes in combination with a correction osteotomy of the ulna or radius) in contrast to other authors.

Experimental animal investigation of bone regeneration with collagen-apatite.

Multicentric osteoregeneration is a new principle of osteoinduction [31a] which can be realized with the combination of collagen and fine dispersed hydroxyapatite ceramic particles (Collapat). This was histomorphometrically proved in the present study involving 14 rabbits with 28 6-mm-wide bore holes in the distal femoral condyles. With Collapat, an average of 5 times more new bone was regenerated than in the control defects without implant. The Wilcoxon matched-pairs signed-ranks test proved the positive effect of the implant Collapat on osteoregeneration with a significance of P less than 0.0001. At sites of Collapat insertion bone regeneration begins in the 2nd week, reaches its climax in the 3rd week, and is completed after the 4th week. in the course of development the histological picture is typified by a lively bone remodeling. Bone formation can be recognized in direct contact to the apatite granules. Foreign body or allergic reactions are not observed. Remodeling of the bone is not hindered by the slowly resorbable apatite granules. About 250 human implantations have been performed in our hospital since 1979, with good results and no negative effects.

[Fractures of the spine - long-term results of functional treatment (author's transl)].

644 fractures of the spine without paraplegia treated at the "Bergmannsheil" Hospital in Bochum between 1963 and 1977 are reported. Indications and long-term results of functional treatment are discussed. More than 80 per cent of the anterior wedge fractures had a compression rate of less than one third. Even in early mobilization there is normally no increase in deformity. 8.3 per cent with an increase in deformity had an osteoporosis or a compound fracture. Unstable fractures are stabilized by springs and plates. In cases with great deformity surgical treatment has to be discussed, too.

Adult

[The posttraumatic "psychogenic club-foot" (author's transl)].

Two cases of posttraumatic "psychogenic clubfoot" deformity are reported. Case I. A woman, aged 44, with foot deformity after infantile paralysis on the right side sustained an inversion injury to the left ankle in April 1979. Two weeks later she was admitted to our hospital. She was unable to walk and examination showed a drop foot with supination contracture of the left foot. Neurological examination was normal inclusive electromyogram. Ligaments of the ankle and peroneal tendons were reviewed by operative procedure under suspicion of rupture. There were no pathological findings, no hematomas or other traumatic signs. The neurological control examination was normal and psychogenic contracture was diagnosed. The personal history showed a corresponding problematical psychic background. Case II. A man, aged 24, was admitted to our hospital 3 months after an inversion injury to the right ankle. Examination showed a drop foot with supination contracture (Fig. 1 a, b). He walked with two crutches. Roentgenograms were normal, the ankle had normal stability, and neurological examination showed no pathological findings. There were no signs of an organic lesion. Psychogenic club-foot was diagnosed. Both patients were treated functionally with active exercises and psychological care. The follow-up control 1 resp. 1/2 year after injury showed normal muscles and free active mobility in both cases (Fig. 1 c, d).
