Immunohistochemistry: a useful adjunct in the evaluation of malignant cutaneous spindle cell tumors.

We used immunohistochemistry to evaluate four cytologically malignant cutaneous neoplasms on the face or neck of elderly individuals. All four lesions were composed of a dermal proliferation of spindle and pleomorphic giant cells. Differential diagnosis included spindle cell carcinoma, atypical fibroxanthoma, malignant melanoma, leiomyosarcoma, and angiosarcoma. All four neoplasms were strongly immunoreactive for vimentin and negative for cytokeratin, S100 protein, desmin, and factor-VIII-related antigen. Focal immunoreactivity for lysozyme and/or a1-antichymotrypsin was seen in the giant cells of each lesion. These results supported the diagnosis of atypical fibroxanthoma in each instance. Immunohistochemical staining can provide useful information for distinguishing among malignant cutaneous spindle cell tumors.

Acral mucinous syringometaplasia. A benign cutaneous lesion associated with verrucous hyperplasia.

We describe herein a case of acral mucinous syringometaplasia that presented clinically as a verruca plantaris. Microscopically, verrucous hyperplasia of the epidermis and central, dilated eccrine ducts lined by a mixture of squamous and goblet cells were present. Mucinous syringometaplasia is a benign entity that should be distinguished from other mucin-containing cutaneous lesions.

Serum enzyme alterations in polymyositis. Possible pitfalls in diagnosis.

We report serum enzyme patterns in three patients with polymyositis or dermatomyositis whose cases posed potential problems in distinguishing malignancy, hepatocellular damage, and myocardial infarction from myositis. The alanine aminotransferase showed five- to 16-fold elevations. The creatine kinase MB isoenzyme and a predominance of lactate dehydrogenase isoenzymes 2 and 3 were present in each of the three patients. However, none of the patients showed any evidence of hepatocellular damage, myocardial infarction, or malignancy. We suggest that, although the individual occurrences are indicative of other processes, the concurrence of elevated alanine aminotransferase, creatine kinase MB, and elevated lactate dehydrogenase isoenzymes 2 and 3 is consistent with the diagnosis of polymyositis.

Pappenheimer bodies. An additional cause for a spurious platelet count.

A 67-year-old woman who had sideroblastic anemia had a spuriously elevated platelet count due primarily to Pappenheimer bodies. The platelet count on an electrooptical instrument was falsely elevated. The true platelet count was determined by phase microscopy using lysed whole blood and by the peripheral blood smear. There appear to have been no previous reports of falsely elevated platelet counts due to Pappenheimer bodies. Other causes of spuriously elevated platelet counts are reviewed. The reasons for the spurious counts are discussed.

Bartholinitis--an unusual etiologic agent.

Streptococcus pneumoniae type 4 was isolated in pure culture from a Bartholin's gland abscess in a 26-year-old woman, gravida 3. To the authors' knowledge, this is the first report describing this agent as a cause of bartholinitis.
