[Mental disorders in pathological gamblers].

The study aimed at investigating mental disorders in pathological gamblers. A sample included 73 pathological gamblers (PG) and 77 age- and sex-matched controls. Severity of mental disorders was assessed using the SCL-90-R questionnaire. Most of PG were male (89%), mean age 26,8 +/- 6,3 years. Divorced were 27,4% and never married--20,5%. Most of PG (71,1%) had secondary school education. Alcohol-related problems were observed in 35,6% of PG and 52,1% had positive family history of alcohol-dependence. Mental disorders, mostly of anxiety-depressive and asthenia character, as well as obsessive-compulsive, paranoiac and phobic symptoms were found in 66 (90,4%) PG. In 92,4% cases, severity of disturbances reached the level of symptomatic stress disorder.

[Efficacy and tolerability of coaxil (tianeptine) in the therapy of posttraumatic stress disorder].

The results of a multicenter study of coaxil in the treatment of 90 patients with posttraumatic stress disorder (PTSD) are summarized. Two dosages of the drug, 75 mg daily (44 patients) and 37.5 mg daily (46 patients) have been compared. Coaxil efficacy was estimated with the Clinician Administered PTSD Scale (CAPS) and Hamilton depression scale (HAM-D). High efficacy of coaxil was shown, with 76.7 and 75.6% responders, respectively, for the dosages used. The optimum one proved to be 37.5 mg daily, which is congruent to the standard dose in the treatment of depression and provides not only the efficacy but tolerability of coaxil. The coaxil efficacy depended on a PTSD type: the drug was more effective in chronic PTSD that, in the authors' opinion, might be related to its antidepressant action.

[Mental disorders in patients with arterial hypertension with metabolic disturbances].

A battery of psychological tests has been used to study the psychic state and quality of life in 75 patients with mild and moderate arterial hypertension (AH) with or without (controls) different metabolic disturbances (diabetes mellitus, dyslipidaemia, obesity). Comparing to controls, the AH patients with metabolic disturbances suffered more frequently from different psychic disorders of neurotic register and their quality of life, general condition, activity and mood proved to be significantly worse. The authors conclude that psychological testing may be an important diagnostic tool in general hospital. It allows identification of the borderline mental disorders features, the correction of which appears to be of great importance for basic disease course and its therapy.

[Psychophysiological health aspects of population living in radiation-contaminated territories].

A combined psychophysiologic study of 100 individuals living in territories contaminated by radionuclides and 60 people from ecologically clean region. There were no significant differences in the incidence of neurotic disorders between people from these regions (61.2 and 65.3%, respectively). The highest sensitivity to small radiation doses was observed at rather young age (20-40 years). It was found that a considerable role in the development of borderline psychic disorders belongs to both somatic disturbances due to low-dose radiation and psychogenic effects. A combined action of psychogenic and somatogenic factors aggravated symptoms of neurotic disorders and promoted widening of a range of psychic disturbances. Some part of the population from the contaminated territories tended to the development of cerebro-vascular psychoorganic pathology resultant from a breakdown of the reserves of the adaptation. Now the stress factor of radionuclides exposure is not so important as social and economic problems.

[Effectiveness of electroconvulsive therapy in the light of current approaches to the treatment of febrile conditions in schizophrenia].

The authors review febrile attacks of schizophrenia and the febrile period of the malignant neuroleptic syndrome in schizophrenia. Twenty-five schizophrenia patients were observed to establish the effect of the ECT on their status. The ECT was found to be highly effective in combination with intensive infusion therapy aimed at the correction of homeostasis. It was revealed that the prognosis and efficacy of the use of the ECT is determined by the stage of the development of febrile condition, the leading syndrome and the gravity of somatovegetative disorders. The use of the ECT was ineffective in cases where the development of a febrile attack reaches the level of amential dull consciousness.

[Clinico-pathogenetic patterns in the development of febrile attacks in schizophrenia].

Patients with febrile attacks of schizophrenia showed clinical and laboratory correlations between the severity of the clinical picture on the one hand and parameters of the leukocyte index of intoxication, ESR, the NaK ratio and 17-CS secretion, on the other. The authors consider the possible causes of elevated levels of dopamine in the urine and of beta-lipoproteins and creatine phosphokinase in the cerebrospinal fluid. The study has revealed anti-brain antibodies and sensitization of leukocytes to neuroleptics. A possible role of neural viral infection in the etiology and pathogenesis of the studies conditions is discussed.

[Clinical picture and course of the neuroleptic malignant syndrome (acute febrile neuroleptic encephalopathy)].

On the basis of their observations and management of 23 cases of the neuroleptic malignant syndrome (MNS) the authors draw a conclusion that this syndrome is similar to extrapyramidal-psychotic side effects of neuroleptic therapy. Different variants of the course of this complication of neuroleptic therapy attended by hyperthermia are considered. The authors propose the term 'acute febrile neuroleptic therapy' which incorporates all hyperthermic variants of the course of this complication of neuroleptic therapy, varying from mild transitory one to the MNS. Criteria of the early diagnosis are presented.

[Therapeutic approaches to the treatment of febrile schizophrenia].

Analysis of the literature data on the use of various therapeutic approaches to the treatment of febrile schizophrenia has shown that so far psychiatry does not possess such methods of treatment which could allow the complete prevention of lethal outcomes in this disease. The article describes a new original approach to the treatment of patients with febrile schizophrenia, aimed at the correction of homeostatic disturbances with the complete refusal from neuroleptics in the febrile period of an attack. This therapeutic approach proved highly effective in 24 out of 26 patients and enabled preventing lethal outcomes in all cases. Study of the characteristics of febrile schizophrenia based on the authors' own observations and case reports made it possible to identify three variants of the time-course of the febrile attack. The article also considers the specificities of therapy with respect to the identified variants of the time-course of febrile attacks.

[Modified variant of simultaneous discontinuation of psychotropic drugs (given in combination with diuretics) as a method of abrupt termination therapy for protracted episodes of schizophrenia].

A total of 100 schizophrenic patients with protracted attacks and refractory to psychopharmacotherapy were studied. In order to detect the efficacy of different treatment methods directed towards a break-off of protracted attacks, a comparative study of the intensity of psychopharmacotherapy, a simultaneous discontinuance of psychotropic drugs and their combinations with diuretics was accomplished. The latter method appeared to be the most effective with the least amount of side effects.

[Biochemical characteristics of blood from patients with febrile attacks of schizophrenia].

The data of detailed biochemical analysis of the schizophrenic patients with febrile psychoses suggest the differences in the blood enzymatic spectrum between different types of this disease (spontaneous febrile schizophrenia, malignant neuroleptic syndrome, generalised allergic reaction). The activity of the plasma enzymes is correlated with febrile syndrome and clinical manifestation of the disease, its highest level is observed in patients with malignant neuroleptic syndrome. The role of the cytolysis in clinical manifestation different types of psychoses is discussed.
