[Diagnosis and treatment of aneurysms of the lower segment of the vertebrobasilar system].

The authors present 20 patients with aneurysms of the vertebral artery and its branches, treated operatively. The symptomatology and the diagnostic methods in this group of rare vascular abnormalities are discussed. The authors are of the opinion, that there are no specific symptoms typical of this localization. All aneurysms of this group can be treated by suboccipital craniectomy. The results of the operation were good; in 75% the results were good or very good. 20% patients died.

[Immediate and late results of surgical treatment of hydrocephalus after subarachnoid hemorrhage].

During the period from January 1st 1983 up to the end of April 1989 in the Neurology Department and the Neurosurgery Department 1417 patients were treated due to subarachnoidal hemorrhage. Among these patients in 275 cases the inner hydrocephalus was diagnosed using computer tomography (19.4%). In 17 cases (6.2%) hydrocephalus was treated surgically. In all patients the ventricle-atrial Pudenz valve was implanted; in 15 patients the valve was implanted for low pressure and in 2 for moderate pressure. The diagnostics was based on the computed tomography and angiography of cerebral vessels. In all cases large hydrocephalus was diagnosed (the anterior horn index in the computed tomography was below 3.0). The most frequent clinical symptoms were: urine incontinence (15 cases), disturbances of orientation (8 cases), akinetic mutism (8 cases) and paresis of extremities (7 cases). Above symptoms together with radiologic image presented indication for the valve implantation. Very good and good results were obtained in 13 patients, bad results in 2 cases, 2 patients died. Longer observation showed that 6 patients remain in satisfactory state, 3 patients are in bad state and 3 others died.

[Case of double aneurysm located at the bifurcation of the basilar artery].

The authors report a case of two aneurysms at the bifurcation of the basilar artery in a 47-year old housewife. The technique for the exclusion of the aneurysms from the cerebral circulation using the operating microscope is described. The course of the illness and the patient's condition several months after procedure are described.

[Early complications following intracranial surgery].

The authors discuss early non-inflammatory postoperative complications developing in the first week after operations. The reported material comprised 97 patients from among 1 815 cases treated surgically in the years 1971--1973 at the Departments of Neurosurgery and Neurotraumatology, Medical Academy in Cracow. The most frequent causes of reoperations were brain oedema, epidural haematoma and haematoma in the empty space left after evacuation of lesion. In the group of reoperated patients the percent of fatal cases increased to 56,7%.

[Subarachnoid hemorrhage from ruptured aneurysm in the course of pregnancy].

The authors report a case of subarachnoid haemorrhage from a ruptured aneurysm of the left internal carotid artery at the site of branching-off of the ophthalmic artery in a patient with advanced pregnancy. Indications and contraindications to surgical treatment in pregnancy are discussed. The case and follow-up are described.

[Our experience with using nimodipine in the treatment of ischemic complications after subarachnoid hemorrhage].

On the basis of 50 patients treated for subarachnoid haemorrhage the authors report own experiences with the use of Nimotop for control of vasospasm caused by cerebral ischemia. All these patients were in grades III and IV of the Hunt-Hess scale. The studied patients with vasospasm and cerebral ischaemia were compared with patients similarly in grades III IV of this scale who were not given Nimotop. The analysis showed that patients treated with Nimotop were operated on earlier and had less evident neurological defect on discharge from the hospital.

[Clinical-radiological correlations of cerebral vasospasm and cerebral infarction in patients after subarachnoid hemorrhage].

The correlation was studied between angiographically demonstrated vasospasm, ischaemic focus demonstrated by computed tomography and the clinical condition in 100 patients within 2 weeks after spontaneous subarachnoid haemorrhage. The control group comprised 50 patients after subarachnoid haemorrhage without vasospasm. The clinical condition of the patients with vasospasm was more serious than in the control group. The proportion of patients with clinical grade I was 24% in the group of vasospasm and 50% in the control group. Ischaemic foci were found in computed tomograms four times as frequently in the group with vasospasm than in the control group. Neurological deficit was noted in 41% of patients with vasospasm and in 26% in the control group. The study confirmed correlations between vasospasm, cerebral infarction and neurological deficit but these correlations were not strong.

[Hydrocephalus following subarachnoid hemorrhage].

The authors present 56 patients with subarachnoid haemorrhages in whom CT of the head demonstrated internal hydrocephalus. This complication was observed most frequently in the age group 51-60 years. In 39% of cases hydrocephalus was low grade, in 36% it was moderately severe, and in 25% high grade. In 33 patients (59%) a syndrome of clinical symptoms and signs was observed which could be related, in part at least, to hydrocephalus. In 11% of cases. Pudenz valve had to be implanted. No significant differences were found in the frequency of hydrocephalus in late period after subarachnoid haemorrhage in relation to sex, number of haemorrhages, clinical state, vasospasm, treatment with epsilon-aminocaproic acid and dexamethasone.

[Topographical and radiological correlations in aneurysms of the middle cerebral artery].

The authors demonstrated topographic-radiological correlations in cases of middle cerebral artery aneurysms verified surgically. In an own material of 91 cases treated in the Department of Neurosurgery, Medical Academy in Cracow in the years 1980-1983 the relationship of the aneurysm to the neighbouring vessels and the direction of the dome of the aneurysm were analysed. In the presented material aneurysms of the middle cerebral artery accounted for 18% of all intracranial aneurysms. Most frequently they were situated at the main bifurcation of the artery (86.8%). The dome of the aneurysms of the main bifurcation was lying usually in the line prolonging the main trunk of the artery cephalad and laterally. The results of surgical treatment are presented in 81 patients with aneurysms of the middle cerebral artery treated surgically.
