[Male breast cancer in Africa, Apropos of 5 cases at the Ouagadougou University Teaching Hospital (Burkina Faso)].

A retrospective study of male breast cancer was undertaken at Ouagadougou University Teaching Hospital over a 3 year period (1993-1996). Authors report 5 cases representing 4.16% of all breast cancers. The patients' mean age was 61 years. The average duration of signs and symptoms before the diagnosis was 13 months. Clinically all the 5 cases presented advanced cancers (4 T4N2M0, 1 T4N2M1 according to UICC TNM System) with size ranging from 5.5, to 11.5 cm. Histology found: 2 medullary infiltrating carcinoma, 1 canalar infiltrating carcinoma, 1 colloid mucous carcinoma and 1 lobular infiltrating carcinoma. All patients had mastectomy associated with axillary clearance in 4 cases. Radiotherapy, chemotherapy and hormonotherapy were not associated because unavailable in Burkina Faso. Three patients died: the first, 10 days after surgical treatment and the 2 others respectively after 14 and 17 months. We have lost sight 1 patients. The last one is still alive. Authors find that to get better prognosis, it is important to improve medical and technical means, to increase information and to promote early detection.

Voluntary female sterilisation via minilaparotomy: report from Burkina Faso.

We present the first study of voluntary female sterilisation in Burkina Faso. The average woman undergoing tubal ligation was a 37 year old, married, house wife para 8 with five living children. The main reasons for TL were: achieved desired family size (45.9%) and medical reason (29.5%). The TL was usually performed (77.8%) in the postpartum, using the Pomeroy technique. With a follow up of three to fifteen months, no pregnancy has been reported and no request for reversal expressed. The authors make some suggestions to increase the prevalence of TL in Burkina Faso.

[Female sterilization in Burkina Faso. Report of 142 cases].

Through a transversal study over a twelve month period, the authors report 142 cases of female sterilization (FS) in Burkina Faso. The aims of this study were to determine the profile of women undergoing FS and to precise the indications and the immediate outcome. The study took place in the two main gynecologic and obstetric units of the country. Various data about the socio-demographic characteristics of the women, their reproductive history, the indications for the FS and the short term follow up were recorded. The woman undergoing tubal ligation (TL) is 36 years old, gravida 7 para 7 with 5 children (3 boys for 2 girls). Her occupation is housewifery, she is married and is illiterate in 84.4% of the cases. This woman has no prior history of contraceptive use in 71% of the cases. FS was performed for medical reasons in 50% of the cases whereas economic reason was invoked by only 5% of the couples. Prior to the TL oral informed consent was obtained from both the woman and her husband. The TL was performed during caesarean section in 55.6%, in the postpartum period in 34.5% of the cases. Two wound abscesses occurred. With a follow up ranging from 2 to 14 months, no pregnancy has been reported. To give a chance to FS to be popular in Burkina Faso, the authors made some suggestions.

[Measles and pregnancy. Apropos of 16 cases in Burkina Faso].

We report an epidemic of 16 cases of measles during pregnancy. The risk factors for such an association and the materno-fetal outcomes are presented. The mean age of the patients was 20.6 years, with a mean gravidity and parity of 2.1 and 1.1 respectively. The mean clinical features were: conjunctivitis, hyperthermia and cutaneous rash. Nine maternal complications occurred: 6 laryngitis and 3 pneumopathies. All patients were HIV negative. The outcomes of the pregnancy were the following: 2 abortions, 3 stillbirths, 1 preterm delivery and 2 full term births. Eight patients with ongoing pregnancies were lost to follow-up after their discharge from the hospital. We conclude on the need for a systematic prevention in exposed pregnant women by immunotherapy and in children by immunization.

[Eighty cases of uterine rupture at the maternity service of the National Hospital Center of Ouagadougou , Burkina Faso].

In this retrospective study, we recall the frequency of uterine ruptures within a national health center of West Africa and propose solutions in order to reduce the incidence. This very high frequency was 23/1000, i.e. one uterine rupture for 44 deliveries. The feto-maternal prognosis was poor with a high stillbirth rate and maternal mortality. Maternal mortality which was 35% of cases represented 22.7% of the causes of maternal mortality observed within the unit during the survey. The stillbirth rate was 95% representing 145/1000 of the hospital stillbirth rate recorded during the survey. Maternal morbidity was also high because 14% of those who survived presented a parietal suppuration with the release of suture during the immediate postsurgical period. The seriousness of the feto-maternal prognosis is linked to a lack of adequate patient management. The importance of uterine ruptures in the obstetrical activity of the unit needs a joint and urgent action of all intervening parties within the sanitary system in order to contend with this scourge, which is the sign of poor quality health care in Obstetrics.

[Adnexal torsion during pregnancy. Diagnostic and therapeutic problems in Burkina Faso].

The authors report a case of adnexal torsion during early pregnancy. Because of a late diagnosis they had to practise an unilateral adnexectomy with a conservative medical treatment of the pregnancy. They recommend an early surgical treatment, that will save the adnexa and permit a normal evolution of the pregnancy. In a young patient, the loss of an ovary may be a tragic event.

Anti-implantation activity of antiestrogens and mifepristone.

To develop a better postcoital contraceptive, the following antiestrogens were tested for their anti-implantation activity in the rat: anordrin, anordiol, tamoxifen, ICI 182,780, and RU 39411. The compounds were administered orally or subcutaneously (s.c.) to female rats on days 1, 2, and 3 of pregnancy. All the antiestrogens tested were 100% effective in preventing blastocyst implantation. The lowest effective doses when administered orally were 10, 1.25, 0.062, 6.0 (partially effective), and 0.01 mg/kg/day, respectively. The estimated median effective doses (ED50) were 5.60, 0.40, 0.035, 5.40, and 0.0074 mg/kg/day, respectively. When administered s.c., the minimum effective doses in preventing blastocyst implantation in all animals were 2.0, 0.1, 0.1, 0.1, and 0.01 mg/kg/day, respectively. Anordrin, anordiol, and ICI 182,780 were more potent when administered s.c.; whereas tamoxifen and RU 39411 were effective at similar doses when administered parenterally or orally. RU 39411 was the most potent among the antiestrogens tested and should be evaluated as a potential postcoital contraceptive. The administration of mifepristone, an antiprogestin, at a dose of 8 mg/kg/day blocked blastocyst implantation in all treated animals; whereas at a dose of 4 mg/kg/day or lower, the drug was ineffective. These findings confirm that estradiol and progesterone are essential for blastocyst implantation in the rat. The capacity of mifepristone to potentiate the anti-implantation activity of the antiestrogens was also determined. The combination of a non-effective dose of each of the antiestrogens (anordrin, anordiol, and tamoxifen), and RU 39411, with mifepristone at a non-effective dose, prevented pregnancy, demonstration that an antiprogestin and antiestrogen act synergistically in blocking blastocyst implantation in the rat. The antiestrogen compounds whose anti-implantation activities were potentiated by mifepristone were found to possess significant estrogenic activity, when assayed by measuring the increase in the uterine weights of ovariectomized rats. The only exception was ICI 182,780, which showed no estrogenic activity in the uterine weight bioassay and did not act synergistically with mifepristone in blocking blastocyst implantation. Estradiol was effective in preventing pregnancy at a dose of 1 microgram/kg/day. The combination of non-effective doses of estradiol and mifepristone did not prevent pregnancy. The findings that mifepristone potentiates the anti-implantation activity suggests that the synergistic effect may be a unique property of this class of antiestrogens.

[Vaginal injuries during coitus: 98 cases].

UNLABELLED: We have undertaken a prospective study of vaginal injuries due to coitus with the aim of evaluating their incidence, risk factors and anatomic lesions. Over a period of 39 months, 98 cases were found, giving a monthly incidence of 2.5. The patients had a mean age of 25 years, were paucipara in more than 50%. Sexual abstience was found in 72 cases with a mean duration of 6 months. This injury occurred more often in gynecological period than in the postpartum (77 cases versus 22). During sexual intercourse the most harmful position was when the patient was in dorsal decubitus with maximum flexion and abduction of the limbs. The vaginal injury was located in the posterior cul de sac in 39 cases and was of 5 cm of length in 56%. Surgical suture was required in 92 out of 98 patients, in combination with local and general antibiotics. CONCLUSION: Vaginal injuries due to coitus are frequent in our practice and they create serious anatomic lesions. So there is a need to precise the etiopathogenicity (hormonal deficiency ?, infections ?, role of the male partner) and to evaluate the long term sequellas.

Synthesis and secretion of an estrus stage-specific protein by rat uterus.

A specific protein with an estimated molecular weight of 260 kDa was found to be synthesized and secreted into the incubation medium by rat uterus only during the estrus stage of the cycle. This secreted uterine protein was designated as estrus stage-specific protein (ESP). ESP was not produced by pregnant, lactating or immature pup rat uteri. Estradiol administered to ovariectomized rats induced production of ESP which was blocked by the antiestrogen, ICI 182, 780. The present results show that the synthesis and secretion of ESP is regulated by estradiol and this protein maybe involved in blastocyst implantation.

Low ventricular performance and high resistance in established hypertension in adults.

OBJECTIVE: To investigate the relationship between left ventricular performance and sympathetic nervous activity. DESIGN: We studied the alpha- and beta-adrenergic responsiveness of the subjects and assessed their left ventricular performance. METHODS: Fifty-four adult established hypertensive patients, all with apparent left ventricular hypertrophy, and 36 age-matched normotensive controls were studied. Thirty-four of the hypertensive patients were within the +/- 2SD confidence area of fractional shortening/end-systolic stress relation of the normotensive controls and are denoted subgroup A; 19 patients were below the lower limit and are denoted subgroup B. Isoproterenol and neosynephrine injection tests were used to assess beta- and alpha-adrenergic responsiveness, respectively. Intravenous infusion tests using regitine and isoproterenol were performed in 16 patients to assess the effects of sympatho-adrenergic responsiveness on changes in left ventricular performance. RESULTS: Afterload and left ventricular mass were similar in the two subgroups. Left ventricular performance and beta-adrenergic responsiveness in subgroup A were comparable with the corresponding levels in the normotensives, whereas in subgroup B both were markedly decreased. The regitine infusion test induced a fall of 25% in peripheral resistance from baseline, but no significant improvement in left ventricular performance. In contrast, isoproterenol infusion test resulted in striking improvements: left ventricular performance increased by 60%, afterload decreased by 48% and peripheral resistance fell by 50% from baseline. CONCLUSION: The diminished ventricular performance and high resistance observed in adult established hypertension may be due to synergic effects of significantly reduced beta-adrenergic responsiveness coupled with enhanced alpha-adrenergic responsiveness.

[Antithrombin ii in eclampsia: estimation of predictive value].

We have observed in our study that antithrombin III activity decreases very significatively in eclampsia (p < 0.0001). A level of 90% was defined as a threshold. All the rates which are under or equal to 90% have 78.3% as a positive predictive value and those over 90% have a 98.7% as a negative predictive value for the overcoming of eclampsia. We have concluded that the 90% antithrombin III activity represents the alarm level for over coming eclamptic crises. The determination of the antithrombin III activity must be systematically done in every hypertensive pregnancy with proteinuria.

[Conjugal sterility at Dakar CHU: epidemiological profile and evaluation limits apropos of 429 cases].

The writers draw up the outcome of the medical management of infertiles couples at Dakar's teaching hospital over a period of 7 years (1983-1989). Restrictives factors (low rate incomes, social and psychological obstacles) make that médical management difficult. The responsibilities are shared among the couple with sex ratio of 1 man for 3 women. Among the women the etiologies dominated are the sterility of the Phaloppe tubes (81%) among men with the oligo and the azoospermy (32 and 27%). Sterility within married couples is a problem of public health that becomes more acute every day. A better handling of this problem through preventive medicine and elementary sanitary training is necessary.

[Perinatal mortality and eclampsia at C.H.U. of Dakar. Apropos of 49 cases].

From 1/04/1985 to 12/02/1990 the authors have treated 49 pregnant women with eclampsia symptoms. Keeping in mind the foetal neonatal results, the following has been noted: 26 live births including on case involving twins, 20 still-births, two foetal abortion and two patients who had never returned. 90.47% of the deliveries were effected vaginally against 09.53% by means of cesarean; patients of whom we lost track are not accounted for, together with 05 foetuses who were extracted during the maternal autopsy. Among the live births, 52.84% weighed less than, 2500 grams and had an APGAR score lower than 07 during the first minute in 69.23% of the cases observed. Six newborns died during the neonatal period because of infection and/or premature birth. Perinatal mortality of this short sample is of 63.07%. This leads the authors to conclude that the improvement of the foetal prognosis of the eclampsia is necessary. This improvement will necessarily go through the broadening of the indication of cesarean.

[Hydatid cyst of the uterus (apropos of a case seen at the gynecologic and obstetrical clinic of the C.H.U. of Dakar)].

Hydatic Cysts, or Hydatidosis, is the result of an intermediary host to VON SIEBOLD's Taenia Echinococcus larva turning into a cyst within the organism. Hydatic sickness combines all the toxic and allergic symptoms produced by the hydatic cyst. Echinococcosis is the anthropozoonosis of parasitic origin responsible for repercussions on the organism, development of the hydatic cyst and the hydatic sickness. It can come from dogs, sheep or cattle or, exceptionally, humans. Based on a case of hydatic cyst in the uterus, the authors underline gyneco-obstetrical aspects of the affliction. They recall diagnostic difficulties which may occur outside the endemic zones this very rare localization, and propose a practical solution based on echography.

Effects of anordiol, an antiestrogen, on the reproductive organs of the male rat.

Anordiol, an antiestrogen, was administered at doses of 2.5 and 10 mg/kg day-1 for 30 and 60 days to adult male rats. A significant decrease in serum LH, FSH, and testosterone levels occurred in the treated animals as compared to controls. Serum testosterone and LH levels decreased to about 15-35% of control value on day 15 of treatment and reached undetected levels thereafter. Serum FSH level also decreased to about 25-50% of control value by day 15 of treatment and remained at this level. Significant decrease in the weights of the tests and accessory reproductive organs occurred following anordiol treatment, while the body weights remained at pretreatment level. Histological examination of the tests revealed significant decrease in the diameter of the seminiferous tubules and in the number of germ cell elements. Spermatogenesis was arrested at the secondary spermatocyte stage. Leydig cells appeared atrophic and contained pyknotic nuclei. The epididymis, prostate, and seminal vesicle showed degenerative changes. The secretory activity of the glandular epithelium of the prostate gland appeared to be markedly reduced. In conclusion, anordiol acts by blocking gonadotropin production and/or release by the pituitary; thereby testosterone production by Leydig cells is not stimulated, causing spermatogenesis arrest.

[Maternal HIV infection and the anthropometric characteristics of children at birth in Burkina Faso. DITRAME Study Group].

The aim of this study was to investigate the relationship between maternal HIV status and the anthropometric characteristics of children at birth. A cross-sectional study was conducted in Bobo-Dioulasso from January 1995 to May 1996. We included all mother-child pairs seen postpartum for whom the HIV status of the mother had been determined at an antenatal visit and anthropometric measurements had been taken for the child at birth. Birth weights below 2,500 g were classed as "low", body lengths of less than 47 cm at birth were described as "small birth size" and head circumferences of less than 33 cm were classed as "small". We included 956 mothers and 956 children in the survey. Low birth-weights were recorded more frequently among children born to HIV-infected mothers than among those born to uninfected mothers (23. 37% versus 15.6%; p = 0.006). Mean birth-weight, birth size and head circumference did not differ significantly between the children of HIV-infected and uninfected mothers. In multivariate analysis, HIV infection and primiparity were independently associated with low birth-weight. Maternal HIV infection was the only factor associated with small birth size. There was no relationship between head circumference and maternal HIV status. Thus, maternal HIV infection appears to be associated with low birth-weight and small birth size.
