Prediction of treatment response to rivastigmine in Alzheimer's dementia.

OBJECTIVES: To predict the treatment response to rivastigmine in patients with Alzheimer's dementia using neuropsychological and EEG data. METHODS: A neuropsychological examination and a quantitative EEG study were done in 20 patients with Alzheimer's dementia before initiating treatment with rivastigmine. After one week of treatment a second EEG examination was done. Therapeutic efficacy was determined six months after treatment initiation. Treatment response was defined as improvement in short term memory after six months of rivastigmine treatment. RESULTS: For the group of patients as a whole, there was a significant improvement in short term memory and orientation during rivastigmine treatment. The mini-mental state score improved from 20.2 to 21.7 (NS). In the EEG, theta power decreased significantly after one week of treatment. Treatment responders had a greater decrease in theta power after one week of treatment and a better short term memory at baseline than non-responders. Decrease in theta power during rivastigmine treatment and baseline short term memory were good predictors of treatment response. CONCLUSIONS: Generally available neuropsychological and EEG data may be useful for predicting response to rivastigmine in patients with Alzheimer's disease.

Trans-Antarctic expedition: the second 100 days.

On July 27, 1989, six men and 39 sled dogs embarked on the first trans-Antarctic dog, sled, and ski crossing. Data sheets were developed and a review of their day-by-day reports was carried out. The period of time covered is November 3, 1989, to February 10, 1990. Miles traveled were recorded for each day and varied from no progress to 31 miles for a single day. A running account of the total miles traveled was also maintained in relationship to the number of days traveled. Radio contact was maintained when conditions permitted. They were able to make calls home and as far away as Paris, France. The time traveled was summarized where recorded and weather conditions particularly were reviewed and maintained. Land conditions, visibility, and cloud formations were discussed. The average temperature varied from -8 degrees C to -48 degrees C (-54 degrees F). Wind conditions and land locations were recorded, as were difficulties encountered on each day. High-calorie foods were eaten. One dog ate as many as 12,000 calories per day. Following a review of appropriate topics, a summary of the conditions is given.

Improving clinical practices can boost the bottom line.

An investment in improving quality has the potential to yield a higher rate of return than any other investment a healthcare organization can make. Payers and employers in the future will contract with those healthcare organizations that not only charge appropriate amounts for delivering care but also can demonstrate the quality--i.e., value--of the care they are providing. Improving the quality of care can improve both a hospital's fiscal performance and its market share.

Studies on platelet function during different modes of administration of PgF2alpha in obstetrics and gynecology.

Platelet function has been studied during intravenous, intraamniotic, and extraamniotic administration of prostaglandin F2alpha (PgF2alpha) for termination of missed abortion and missed labor, for therapeutic abortion, and for induction of term labor. The controls received oxytocin i.v. (missed labor and term labor). Our investigations have shown that there was a normalization of the increased spontaneous platelet aggregation and a significant reduction of ADP- and collagen-induced platelet aggregation in the groups given PgF2alpha i.v. The desaggregation in these groups was increased. The other groups given PgF2alpha showed no significant changes in platelet function. Inducing labor by oxytocin we found a tendency to increased platelet aggregation and decreased desaggregation. The clinical importance of these findings and the consequences for hemostasis are discussed.
