Anatomoclinical significance of splenic artery anastomotic bridges.

The authors analyzed the frequency of anastomotic bridges between the terminal branches of the splenic artery, their location related to the splenic surface, length and caliber on a total of 102 autopsy specimens of human spleen and pancreatic tail. The methods applied were corrosion casting and postmortem arteriography. The incidence of cases with the splenic artery anastomotic bridges was 33.3%. They were located as follows: intrasplenic 25.5%, extrasplenic 4.9%, both intra- and extrasplenic 2.9%. The remaining two-thirds of spleens were free of anastomoses and thus hypothetically acceptable for partial and segmental splenectomy, as arterial bridges can jeopardize such surgical procedures. Intrasplenic arterial anastomotic bridges were of small caliber and ruled negligible, while the extrasplenic arterial anastomoses are of large caliber being of surgical importance.

New aspects in laparoscopic cystic artery anatomy.

While performing laparoscopic cholecystectomy we noticed lateral branches of the cystic artery as well as arterial bleeding from the gallbladder bed. In order to establish the nature of these blood vessels we undertook a corrosion cast study. The features analyzed were their frequency of occurrence, caliber and length. The research was performed on 50 human autopsy specimens of the liver with the hepatoduodenal ligament. Lateral branches of the cystic artery that irrigate a part of the anteromedial segment of the right liver lobe were observed in 10% cases. Anastomotic bridges between the cystic artery and the arterial system of the liver were observed in 12% cases.

[Sarcoidosis activity markers].

Sarcoidosis is a granulomatous multisystemic disorder, of unknown origin, that commonly affects young adults between 20-40 years of age. The disease usually manifests itself with changes in the chest which are radiologically visible in 90% of the patients in the form of bilateral hilar lymphadenopathy to interstitial infiltrates in the lungs and bronchi. Once the diagnosis of sarcoidosis has been established, the next step is to evaluate the activity and stadium of the disease. These activity markers include clinical, biochemical and immunological parameters. The clinical criteria include symptoms which indicate the clinical manifestation of sarcoidosis, as well as symptoms which are of prognostic importance for the further course of the disease: dry cough, dyspnea, erythema nodosum, posterior uveitis, polyarthralgia, myopathy, cardiac, renal or nervous system involvement, lymphadenopathy, skin lesions, splenomegaly, enlarged parotid and lacrimal glands, changes in chest x-ray and changes in pulmonary function tests. Biological criteria: biochemical markers in serum which are related to: macrophage and epithelioid cell activity, to lymphocyte activity, to granuloma activity and to collagen metabolism alterations; isotopic markers--67 gallium scan and cellular and soluble components in bronchoalveolar lavage fluid (BAL).

[Malignant disease of the appendix].

The authors shows case report 81 old patient, underwent surgical treatment on Surgical Clinic KBC DR Dragisa Misović because of malignant appendix. Primary symptoms of illness was acute abdomen. Histology confirm nature of illness. The authors discusses about different types of malignant appendix and ways of their treatment, and propose operative procedures which are need to be done in these cases.

[Pulmonary adiaspiromycosis].

A 54-yr old male patient with a history of dyspnea and cough is presented. Due to the clinical course of disease and the radiological changes in the chest a diagnosis of sarcoidosis was established. However, the open lung biopsy revealed the true nature of the pulmonary disease: pulmonary adiaspiromycosis, only secondary to asbestosis, siderosis and silicosis as due to the well known occupational exposure to asbestos and other dusts.

[Primary pulmonary cancer in women the the Doboj Region].

The aim of this work is primary lung cancer in female population of the Doboj district in the period 1975-88. incidence research as well as age structure, localisation, histological types, stage and diseases associated with lung cancer. A group of 114 female patients was investigated and as comparative group male patients with lung cancer in the same period was used. The results show that incidence rate increase is significant and in the future that trend will be continued. Especially serious is often microcellular and adenocarcinoma appearance as well as female non-smoking patients (42%) and patients with diabetes mellitus (18%) in the investigated group high percentage. These phenomena lead to conclusion that risk factors are partly different and they should be better investigated. Females with diabetes mellitus require special treatment and further research should be focused on diabetes--lung cancer relationship.
