Silicone gel sheeting for stabilization of skin grafts.

BACKGROUND: Success of skin grafts depends on sufficient immobilization and early intervention for hematoma, seroma, or infection. OBJECTIVE: To stabilize and cover skin grafts with a tie-over technique using translucent silicone gel sheet. METHODS: Twenty-seven skin defects were resurfaced with skin grafts. A sterile silicone gel sheet was placed over the skin graft. Gel was fixed to the wound edges with skin staplers. RESULTS: All grafts healed without any complication. CONCLUSION: Using silicone gel sheeting on 27 skin grafts, we found that it is an effective method for stabilization and allows direct visualization of the graft in order to inspect hematoma-like complications.

Possibly new multiple congenital anomaly syndrome: cranio-fronto-nasal dysplasia with Poland anomaly.

We present a possibly new multiple congenital anomaly syndrome of craniosynostosis, Poland anomaly, cranio-fronto-nasal "dysplasia," and genital and breast anomalies. A similar pattern of anomalies was observed in two previous cases from the literature; however, some peculiar findings suggest that these cases might represent a new multiple congenital anomaly syndrome.

Multiple occurrence of pilomatrixoma.

Multiple occurrences of pilomatrixoma are rare. Although it has been known as a benign lesion, there are reports of some tumors that have histopathologic features of malignancy. A case of a 68-year-old Caucasian women with multiple pilomatrixoma is presented. She was operated on to remove three other pilomatrixomas on the arms 3 years after the initial excision from the face. Two years after excision of the lesions on the arms, no recurrences were evident.

Bilaterally involved Tessier No. 4 cleft: case report.

Craniofacial clefts are rare among facial anomalies, with an incidence of 1.5 to 5 per 100,000 births, and 1 per 100 cases of cleft lip and palate. The Tessier No. 4 oro-ocular cleft is one of the rarest, with 33 unilateral and bilateral clefts reported in the literature. Among the bilateral clefts only 3 of 9 involved Tessier No. 4 cleft bilaterally. We report a case of bilateral Tessier No. 4 craniofacial cleft and our approach to surgical correction.

Alteration by Iloprost of the vasospastic effects of endothelin-1 in rabbit cerebral vessels.

The reversal of endothelin-1 induced cerebral vasospasm with Iloprost was studied in the rabbit. Vasospasm in the basilar artery was evaluated by angiography; cerebral ischaemia by 'red-neurone-count' on light microscopy and morphological changes by electron microscopy. A potent antagonistic effect of Iloprost against ET-1 was observed in each of the parameters measured.

[Chemical sterilization of gutta-percha cones with sodium hypochlorite, sporocidin and PVP-I].

This study was undertaken to determine the chairside disinfection of gutta-percha cones prior to obturation of the root canal, during the endodontic therapy. Several contaminated gutta-percha cones are immersed in 2.5% aqueous sodium hypochlorite, Sporocidin (2% glutaraldehyde) and 10% polyvinylpyrrolidone-iodine (PVP-I) solutions for 10, 20 and 30 seconds. A 10 second exposure of contaminated gutta-percha cones to the 2.5% aqueous sodium hypochlorite solution found to be the most effective in the surface disinfecting of points in this study.

Multiple atrophies following tooth extraction.

Following a molar tooth extraction, the patient developed a depression and atrophy of the right ear, the right half of the mentum, and the tongue. These atrophy deformities were successfully corrected with dermografts.

Use of beta blockers for increasing survival of free fat grafts.

Fat tissue is abundant in the body and preferred in soft tissue augmentation. Since resorption is its greatest disadvantage, many agents have been tried in an attempt to increase fat graft survival in various studies. Yet selective beta1 blocker had not been used before. The effect of selective beta1 blockers on resorption following auto-transplantation of free fat graft was examined in rats. The effect of selective beta1 blockers was compared with those of insulin and saline solution as a control. For this comparison, the weight of fat grafts was measured both during and nine months after operation using the "liquid overflow method" in which obtained material was put into 5 cc syringe filled with saline solution and the increase in liquid level was recorded as graft volume. Histological examination of grafts and statistical analyses of the results were also made. Our in vivo study demonstrated that selective beta1 blocker significantly increased free fat graft survival. It is our conclusion that selective beta1 blocker is a promising agent for decreasing fat graft resorption.

Application of external fixators in major foot contractures.

Extremity contractures are one of the most common deformities faced by the reconstructive surgeon. Major deformities with ankylosis of the related joint have to be managed by both soft tissue and bone operations. Contractures existing for a long period of time cannot be brought into anatomic position easily because of shortened tendons, nerves, and vessels or ankylosed joints. Although tendons can be lengthened in one operation, this is not the case for neural and vascular structures. The authors use external fixators associated with soft tissue procedures to gradually correct major foot dorsiflexion contractures.

A case with some clinical findings overlapping to Rubinstein-Taybi, Rubinstein-Taybi-like syndrome or multiple pterygium syndrome: coincidental findings or a new entity?

We report a case with broad, deviated thumb and big, duplicated, deviated toes resembling Rubinstein-Taybi syndrome. But the patient did not have severe mental retardation as in Rubinstein-Taybi syndrome and had no microdeletions on chromosome 16 by FISH-based assay. This patient had mild webbing as seen in multiple pterygium syndrome, but broad-deviated thumb has not been reported in this syndrome. We discuss whether these are coincidental or overlapping findings or whether this is a possible new clinical entity.
