[The diagnosis and treatment of postvagotomy disorders].

From experience with various types of vagotomy in 3,102 patients and study of the late-term results, it was established that recurrent ulcer developed in 4.7%, dysphagia in 2.9%, incompetence of the esophagogastric junction in 9.7%, reflux esophagitis in 3.8%, dyskinesia of the duodenum in 2.6%, duodenostasis in 5.3%, the dumping syndrome in 5.4%, diarrhea in 6.1%, and hiatal hernia in 0.3% of cases. The surgical correction of disorders after vagotomy is marked by specific techniques which must be borne in mind to improve treatment.

Duodenal Ulcer

[Features of the functional status of the pancreas and liver after vagotomy].

A study of 578 patients with different kinds of postvagotomy disorders revealed disorders of the incretory and excretory pancreatic function as well as of the absorptive function of the polygonal liver cells. It was established that an increase of the concentration of immunoreactive insulin and C-peptide with simultaneous reduction of immunoreactive trypsin and accumulation of the radioactive agent in the liver may be used as a diagnostic test of incomplete vagotomy.

C-Peptide

[Characteristics of immunologic status in patients with post-vagotomy disorders].

The results of examination of 578 patients with different postvagotomy disorders are presented. The patients with complete, incomplete and inadequate vagotomy were marked out. Among the postvagotomy disorders, the functional, organic and mixed ones were distinguished. The immunologic status was assessed before and after surgical correction in 206 patients by relative and absolute number of the E-RFC, EAC-RFC, EA gamma-RFC and by the level of A, M and G immunoglobulins. It was established that different postvagotomy disorders were associated with the development of the signs of secondary immunologic deficiency. After corrective operations, no restoration of the immunologic indices studied were noted.
