Actuarial analysis of late results after closed mitral valvotomy.

The long-term results of closed mitral valvotomy performed between 1978 and 1985 in 198 patients with noncalcific mitral stenosis were analyzed. Follow-up data were available on 185 patients (93%); 1 patient died in the postoperative period, and 12 foreign patients were lost to follow-up. At the 4-year and 8-year intervals, 91% and 80% of patients, respectively, were event free (not in need of further operative procedures). By multivariate analysis, the factor preoperative mild mitral regurgitation showed a tendency to influence the event-free period. By univariate analysis, postoperative mitral regurgitation significantly reduced the event-free period. Twenty-one patients subsequently underwent mitral valve replacement; 8 for mitral regurgitation, 10 for mitral stenosis, and 3 for mixed mitral regurgitation and stenosis. By multivariate analysis, the reason for reoperation significantly influenced the length of the event-free period. The patients with mitral regurgitation required mitral valve replacement sooner than those with mitral stenosis. Advanced age, sex, previous valvotomy, preoperative New York Heart Association Functional Class, low mitral valve leaflet excursion, and pulmonary hypertension had no influence on the long-term result.

Treatment with oral piperazine oestrone sulphate for genuine stress incontinence in postmenopausal women.

The use of oestrogens in the treatment of genuine stress incontinence was assessed by a double-blind prospective trial in 36 postmenopausal women with genuine stress incontinence who received 3 months of cyclical treatment with either piperazine oestrone sulphate or a matching placebo. Patients were assessed subjectively and objectively before and after treatment by 7-day bladder charts, urethral pressure profiles (UPP), the Urilos nappy test, vaginal cytology and hormone assays (plasma oestrogens and gonadotrophins). There was no statistical difference in the subjective response to treatment between the two groups. After 6 weeks of treatment there was a greater reduction in the number of pad changes/24 h in the oestrogen-treated patients that approached statistical significance but, because of a marked response in the placebo group, this difference was not significant after 3 months of treatment. There were also no significant differences between the two groups with respect to the UPP or Urilos measurements but the vaginal cytology and hormone profiles were significantly affected by oestrogens. In view of the possible risks of oestrogen therapy its use in genuine stress incontinence is limited.

The evaluation of a hostel ward. A controlled study using modified cost-benefit analysis.

A controlled modified cost-benefit evaluation of a hostel ward caring for new long-stay patients is described and results are presented for the first two years. In some respects the residents of the hostel ward had fewer psychotic impairments than those remaining on the wards of the district general hospital, mainly because the latter seem to continue to acquire such defects, while the former have remained relatively unchanged. The hostel ward residents also develop superior domestic skills, use more facilities in the community, and are more likely to be engaged in constructive activities than controls. These advantages were not purchased at a price, since the cost of providing this form of care for these patients has cost less than care provided by the district general hospital.

Nuclear diameter in parathyroid disease.

The nuclear diameter of chief cells was measured in 17 cases of parathyroid adenomas, four cases of secondary hyperplasia, five cases of primary hyperplasia and six cases of tertiary hyperparathyroidism. All the cases with secondary hyperplasia and tertiary hyperparathyroidism were associated with chronic renal failure. The nuclear diameter in both the adenomatous and hyperplastic areas of tertiary hyperparathyroidism were measured. The adenomatous areas of tertiary hyperparathyroidism contained nuclei of a larger diameter than those in the hyperplastic foci of the same gland. The nuclear diameter in adenomatous foci of tertiary hyperparathyroidism was similar to that in adenomas from primary hyperparathyroidism. These findings lend support to the concept of formation of autonomous adenomas against a background of reactive parathyroid hyperplasia in cases of tertiary hyperparathyroidism. Using statistical methods there were differences between the nuclear diameter in cases of primary adenomata, and cases of primary and secondary hyperplasia. Primary parathyroid hyperplasia stood out as a distinct group. The significance of these findings is discussed.

Measurement of alcohol withdrawal in detoxification centre patients.

The pattern and severity of alcohol withdrawal in 49 alcohol dependent subjects admitted to the Manchester Detoxification Centre are described. Assessment over 10 days involved the use of the Selected Severity Assessment Scale (SSA) and allocation of patients to subgroups of the alcohol withdrawal syndrome. There were associations between tremor, clouding of sensorium, hallucinations and convulsions (items of the SSA) and the four pre-delirious clinical sub-groups identified. We concluded that clinical assessment of the pre-delirious stage took into account these four variables and that there was little value in the routine use of the SSA on a unit such as ours.

Verb schema use and input dependence in 5-year-old children with Specific Language Impairment (SLI).

BACKGROUND: It has been argued that children with Specific Language Impairment (SLI) use language in a conservative manner. For example, they are reluctant to produce word-plus-frame combinations that they have not heard in the input. In addition, there is evidence to suggest that their utterances replicate lexical and syntactic material from the immediate language environment. AIMS: This study investigated further the linguistic conservatism of children with SLI. It addressed the hypothesis that these children have difficulties using syntactic schemas in a verb-general manner. It employed a technique pioneered by Childers and Tomasello in 2001 that aims to develop verb-general representations through the presentation of appropriate exemplars. In addition, it tested the hypothesis that children with SLI present with input-dependent language use. METHODS & PROCEDURES: Twenty-four children with SLI (mean age 5;6 years) were matched to 23 typically developing children (mean age 3;5 years) on overall language abilities. The children experienced play sessions designed to simulate exemplar-based learning. Facilitative exemplars consisted of sentences that alternated between nouns and pronouns in the argument slots, e.g. 'Pooh's churning the plate. See, he's churning it'. The children were divided evenly into two groups. One group experienced facilitative exemplars, while the other group heard exemplars with no variation in the argument positions. The training effect was assessed with a procedure in which the children heard novel verbs in a non-transitive frame and were required to produce them in a transitive frame. In addition, the argument types of the children's responses (nouns versus pronouns) were analysed to investigate the issue of input dependence. OUTCOMES & RESULTS: Few of the children produced novel verbs in the transitive. This result may indicate difficulties using the transitive, although a number of methodological difficulties may be responsible for this finding. As hypothesized, the children with SLI demonstrated greater input dependence in terms of the kind of arguments they used in responses during the training sessions. CONCLUSIONS: Potential reasons for the low response rate of both groups during the testing stage are explored and a number of methodological adjustments proposed. It is argued that the greater input dependence of the children with SLI may reflect a tendency to reduce processing load within a limited capacity system. It is proposed that input dependence could be a fruitful new area for SLI research and an important issue in the design and delivery of therapy programmes.
