Diet, alcohol, smoking, serum beta-carotene, and vitamin A in male nonmelanocytic skin cancer patients and controls.

A case-control study was conducted in Melbourne, Australia of 88 consecutive males admitted for the surgical removal of a nonmelanocytic skin cancer (histologically confirmed basal cell carcinoma and squamous cell carcinoma) and of 88 male control patients admitted for small elective surgical procedures. In both cases and controls, previous diet, alcohol consumption, and smoking habit were investigated and serum beta-carotene and vitamin A levels were measured. A statistically significant inverse relationship was found between the risk of skin cancer and a high intake of fish (p = 0.05); vegetables in general (p < 0.001); beans, lentils, or peas (p < 0.001), carrots, silverbeet (Swiss chard), or pumpkin (p < 0.001); cruciferous vegetables (cabbage, brussel sprouts, or broccoli) (p < 0.001); and beta-carotene- and vitamin C-containing foods (p = 0.004). Cases had a lower mean serum level of beta-carotene (p < 0.001) and vitamin A (p = 0.02) than controls. The incidence of skin cancer in the study was inversely related to the level of serum beta-carotene (p < 0.0001). The correlation coefficient between dietary beta-carotene/vitamin C and serum beta-carotene was 0.22 (p = 0.04). Smoking and alcohol consumption showed no statistically significant association with the risk of nonmelanocytic skin cancer. The results were similar for both cell types. A high intake of vegetables including cruciferous vegetables, beta-carotene- and vitamin C-containing foods, and fish appears to be protective for nonmelanocytic skin cancer, and this deserves further study, as does the possible etiologic relevance of the low serum levels of beta-carotene and vitamin A.

Inhibition of hepatic drug metabolism by norethindrone.

In order to assess the effect of norethindrone on hepatic drug metabolism in man, hepatic N-demethylation of aminopyrine was studied by means of the aminopyrine breath test (ABT) in 7 healthy women during two menstrual cycles. Aminopyrine metabolic clearance rates were also studied in 3 women. The women were examined at the ends of the first, second, and third weeks before starting progestogen therapy and at the same times during a second menstrual cycle during which they took norethindrone, 350 microgram/day. The ABT was 5.1 +/- 1.9% (mean +/- SD) during the three control weeks and lower (p less than 0.001) during the three weeks on norethindrone, 3.9 +/- 0.9%. Aminopyrine metabolic clearance rate also fell during norethindrone therapy. The data suggest that progestogens inhibit hepatic microsomal function.

Surveillance of malformations in Sydney in 1976.

The results of a pilot study of malformations in newborn infants in two regions of Sydney in 1976 showed that the incidence of major malformations was 11.31 per 1000 live births and stillbirths, and the incidence of minor malformations was 7.33 per 1000. There were 131 stillbirths among the 12816 births in the study. There were geographical and seasonal variation and "time place clustering" of cases. There was a higher incidence of congenital heart disease and chromosomal abnormalities than in the study carried out in 1966 by Stevenson. The need for accurate diagnosis and for the provision of support and genetic counseling for the parents soon after the birth of a baby with an abnormality is stressed; these should be an integral part of total patient care.

Human fibroblast conditioned media contains growth-promoting activities for low density cells.

Normal diploid human fibroblasts, cultured at high density (1-2 X 10(5) cells per cm2) release two growth promoting activities into the culture medium. The fibroblast proliferation activity-conditioned medium facilitates the attachment of low density cells to the substrate. The activity resides in a non-dialyzable material that is sensitive to proteolytic inactivation. A second activity is dialyzable and can be recovered in the dialysate. In the presence of serum it stimulates cell growth. After 168 hours of incubation conditioned medium cultures contain five times more cells than are present in comparable cultures without conditioned medium. A reproducible biological assay for each activity is described.

Antenatal diagnosis of neural tube defects.

One hundred women with pregnancies at risk for the recurrence of neural tube defects have entered our monitoring programme of echography and alphafetoprotein estimation (AFP) on amniotic fluid and maternal serum during the second trimester. The results of these pregnancies, the application of the programme, sources of error, and the reliability of alphafetoprotein levels are discussed. Conclusions regarding the value of the tests and routine antenatal screening are made.

Control of rat mammary-gland pyruvate dehydrogenase by insulin and prolactin.

Withdrawal of prolactin or of insulin from the circulation of lactating rats leads, within 3h, to increased inactivation by phosphorylation of mammary-gland pyruvate dehydrogenase. Prolactin may act by priming the tissue to respond directly to normal concentrations of circulating insulin and by this means be responsible for the increased activation of the enzyme during the course of normal lactation.
