[Carbohydrate tolerance and intracellular sodium concentration in rabbits with dietary hypercholesterolemia].

The transport of sodium ions and glucose is a specialized membrane function. Variations in the cholesterol content of the cell membrane influence these functions. The influence of a hypercholesterolemia upon the intracellular sodium concentration and the glucose tolerance was investigated in cholesterol/rape oil and in cholesterol fed rabbits. The cholesterol/rape oil fed rabbits had lower KG-values and higher peripheral insulin levels than the control animals. The Na+Ery was decreased after 12 weeks of hypercholesterolemia. The na+Muscle was not changed. A negative correlation was found between the KG-value and the Na+Ery (Na+Muscle) in normal rabbits. In the hypercholesterolemic rabbits existed a negative correlation between the KG-value and the Na+Muscle and a positive correlation between the KG-value and the Na+Ery. It was postulated that membrane functions determined these correlations.

[Effect of interoceptive stimulation of the stomach on photically evoked potentials of the visual cortex with reference to blood pressure and heart rate in the cat].

In ten chloralose narcotized, relaxed and constantly oxygenated cats the stomachs were dilated with two different pressures in acute experiments. Photically evoked potentials (PEP) were derived from the visual cortex of the animals. The aim of the work was to find out in which respect the processing of an exteroreceptive stimulus (light stimulus of only 200 ms) in the visual analyzer can be influenced by an interoceptive stimulation. Besides, it had to be established to which extent the visceral stimulation leads to a change of other vegetative parameters. Qualitative alterations of different PEP-parameters, blood pressure and heart rate of the experimental animals were examined at different stomach pressures. Changes of the positive and negative amplitudes and the time of the negative component of the primary complex of the evoked potential have been observed. Moreover, there could be registered an increase in blood pressure depending on the height of stomach pressure. The heart rate was not influenced. Further studies will have to clarify whether the changes of the PEP are produced by the interoreceptive stimulus or by the changes of blood pressure or respiration.

[Behavior of intracellular electrolytes in the diabetic metabolic state].

By animal experiments and examinations of test persons with healthy metabolism could be confirmed that a high intracellular content of sodium coincides with a low glucose assimilation coefficient. Moreover, the intracellular sodium concentration seems to have influence also on the peripheral insulin content after glucose load as it was shown by the negative correlation of the quotient Na(Plasma)/Na(Ery) with the insulin concentration surface in the rabbits. In maturity-onset diabetics with and without hypertension compared with a control group the intracellular sodium concentration was increased. This intracellular shift of electrolytes might have an influence of the utilisation of glucose in maturity-onset diabetes.

Open-heart surgery: somatic predictors of postoperative psychopathology.

This study reports on some relationships between somatic conditions and postoperative psychopathological disturbances in 102 subjects from 20 to 65 years of age, undergoing open heart surgery. The overall incidence of postoperative psychiatric complications was 40%. Only two variables showed a significant correlation with the psychiatric criteria we used, independent of the different diagnoses: body weight and preoperative urea-N levels in serum. Some of the somatic predictors reported in the literature could be found to be specific for mitral or aortic valve disease. Predictors for psychiatric complications in patients with aortic valve replacement were: age, preoperative protein and urea-N levels in serum, and daytime sedation. The predictor in mitral valve replacement was the decrease in venous oxygenation (pulmonary artery) under physical strain.

Open-heart surgery from the psychological point of view and resulting therapeutic considerations.

A total of 168 patients underwent thorough psychological investigation before and after open-heart surgery. The pre-operative psychological status and the psychosocial situation of the patients were studied by means of questionnaires and semi-standardized interviews. The post-operative psychic status was documented daily on the first four post-operative days and once after discharge from the intensive care unit, by means of a slightly modified version of the AMP rating scale. We found significant correlations between psychosocial problems, the patients' ability to cope with their illness, the surgical stress and the environmental stress of the intense care unit, and post-operative psychic disturbances. The results lead to deliberations on therapeutic consequences.

[Problems in the classification of psychopathological disorders following cardiac surgery with extracorporeal circulation].

Classifications of postoperative psychopathological disorders after heart surgery according to the literature are very different as the results are dependent upon the accuracy of the observation. The accuracy and choice of classification are important as the base for identifying causal factors. We studied 214 patients using three alternative methods of classification and discuss problems of classification of the postoperative psychopathological dysfunctions.

[Some psychological predictors for psychosis after open heart surgery (author's transl)].

In a study of postoperative psychosis after open heart surgery three psychopathological syndromes were identified which had different psychological predictors. Predictors of postoperative emotional disturbances are family problems and the lack of plans for the future, whereas patients with postoperative disorientations seem to have difficulties in their jobs and therefore feel distressed preoperatively. Predictors of the paranoid syndrome after the operation are: a high degree of fear in awaiting the operation and little confidence in the doctors. The social surroundings of these patients are often unstable and although they have no precise plans for the future they object to start working again after operation.

[Changes in the excitation contraction coupling in the myocardium of rabbits fed a cholesterol-containing diet].

It was of interest whether a hypercholesterolemia after feeding a diet containing 2 g cholesterol in sunflower oil per day during 12 weeks induces changes of the myocardiac cells of rabbits which must not necessarily be the consequence of a diminished bloodstream on account of coronary stenosis or the consequence of a lesion of the wall of the vessels. The contractile force and the indirectly measured shortening velocity of the isometrically working papillary muscle out of the left ventricle were not reliably changed. As a result of the feeding the isometric force rose after paired pulse stimulation and after a pause during the rhythmic action, although a considerable deposition of fat within the cells and disseminated necrosis in the myocardium could be observed. Because of the increasing loss of sodium into sodium-free solution in parallel with an increasing sodium content of the myocardium it has been concluded that the permeability of the cell membrane for this ion should be elevated. Furthermore it is assumed that the activation of the ionic pumps is elevated which is recognizable from a higher positive inotropic action of both the application of strophanthine and the substitution of NaCl by LiCl. But it seems that one elevation could not completely compensate the other one. The mean value of the velocity of relaxation related to the isometric force was increased. It has been postulated that after the diet rich in cholesterol and sunflower oil the cardiac cell shows a retarded restitution of the contraction during the interval between beats and elevated content of calcium. This may be one of the causes for the observed necrosis besides the ischemic component of the injurant feeding.

[Classification of psychopathological disorders after open-heart surgery (author's transl)].

A better discrimination of phenomenology and severeness of psychopathological disorders after open-heart surgery has been obtained: a) There is no equal distribution of degrees of severeness, a high correlation of severeness with duration of the disorders, a low correlation with age and no dependance of sex. b) Disorientation, clouding of consciousness and disorders of awareness arise earlier, delusions, hallucinations and paranoid ideas tentatively later after surgery. c) Three psychopathological syndromes have been identified. They appear in different frequency, intensity and can be combined.

[Some somatic and psychological predictors of psychopathological disorders after cardiac surgery (author's transl)].

Some new predictors of postoperative psychosis in open-heart surgery have been identified in a multifactorial study of somatic and psychological factors: Somatic predictors are lower body weight, higher blood levels of urea nitrogen and creatinine, preoperative use of tranquilizers, especially in patients with double valve replacement or congenital heart disease and preoperative cerebral embolism in mitral valve disease. Bypass-time has not proved to be a valid predictor. Psychological predictors are distress provoked by severity and duration of illness, repeated delay of surgery as well as the manner of psychological coping with the illness.

[The function of innervated kidneys during stimulation of the carotid chemoreceptors under constant renal perfusion pressure].

The carotid chemoreceptors of narcotized, vagotomized and spontaneously breathing hydropenic cats in hypertonic mannite diuresis were stimulated by perfusion with venous blood penic cats in hypertonic mannite diuresis were stimulated by perfusion with venous blood for 70 min. Elevation of blood pressure at the innervated kidneys was prevented by an automatically controlled balloon located within the aorta. Stimulation of the chemoreceptors intensified respiration and raised the arterial systemic pressure. With the renal arteries at constant pressure, the effective renal plasma flow and the glomerular filtration rate significantly declined. The filtration fraction remained unchanged. The absolute urinary and sodium excretion did not change significantly, whereas the fractional time-volume, fractional sodium excretion, and the fractional osmotic excretion significantly increased. The fractional tubular reabsorption of osmotically free water was significantly enhanced. These reactions subsided during subsequent perfusion of the glomerula carotici with arterial blood. The results suggest that tubular sodium reabsorption is inhibited by stimulation of the carotid chemoreceptors, although re-adjustment of renal perfusion and filtrate volume cannot be excluded.
