Combination chemotherapy in advanced carcinoma of the cervix.

Two combinations of Adriamycin, cyclophosphamide, and vincristine, one with and one without 5-fluorouracil, were used to treat 27 patients with advanced cervical cancer. One complete remission (3.7%) and one partial remission (3.7%) were observed; both patients are leading a normal life with no evidence of disease after more than 28 months. Thirteen patients (48.1%) had stable disease for a median duration of 3.0 months but only two showed an improvement in the performance status. No significant difference in the survival rate was noted between those with stable disease and those with progressive disease as analyzed by the life table technique. Despite moderate toxicity, the regimens cannot be recommended for recurrent and advanced carcinoma of the uterine cervix because of the low response rate.

Effect of oral medroxyprogesterone acetate on menopausal symptoms in patients with endometrial carcinoma.

A double-blind cross-over study was performed on 21 patients treated for endometrial carcinomas who had severe menopausal symptoms. The patients were randomized into two groups and received medroxyprogesterone acetate (MPA) 100 mg twice daily per os for 12 weeks and a placebo for 12 weeks. A significantly better effect on hot flushes and sweating was obtained with MPA than with the placebo. On average the maximum effect was achieved by MPA after 4-6 weeks. Six patients had a weight gain of more than 3 kg during the MPA administration, compared with none during the placebo administration. No significant difference was found in the blood pressure increase above 160/90 mmHg between MPA and placebo groups. Patients with endometrial carcinoma may risk exacerbation of their disease by undergoing therapy with exogenous estrogen. In contrast, MPA has been found of value in the treatment of disseminated endometrial carcinomas. In this study oral MPA was effective in the treatment of vasomotor menopausal symptoms and may be an alternative in women for whom estrogens might be hazardous.

Importance of histologic grading in the prognosis of epithelial ovarian carcinoma.

Ovarian epithelial carcinomas still have a poor prognosis. Great efforts are being made to improve treatment modalities; chemotherapy in particular is in a period of dynamic development. A basic requirement for the necessary research is a thorough knowledge of the natural history of ovarian cancer and its prognostic influence. In this report the importance of tumor stage, histologic type, and degree of differentiation is analyzed in an unselected and well-controlled Swedish ovarian cancer series comprising 501 cases. The great prognostic significance of tumor grade for survival is demonstrated and it is proposed that this histopathologic factor, as well as tumor stage and histologic type, should be included in the international classification of primary epithelial tumors of the ovary.

The effect on dosimetry of changing from the lithotomy to the supine position in treating uterine cancer with the Cathetron and external radiotherapy.

Uterine carcinoma can be treated by combined external and intracavitary irradiation. With an afterloading technique intracavitary irradiation is given with the patients in the lithotomy position. External irradiation is given with the patient in the supine position. A lead shield is used with the external fields to cover the tissues which are close to the intracavitary sources and receive high doses. The positions of the intracavitary sources seen on radiographs taken when the patient is in the lithotomy position are transferred to films taken with the patient in the supine position. From the latter the position and profile of the shield are calculated. It is shown that a reliable transfer of the positions of the sources and determination of the necessary shielding cannot be achieved in this way. It is therefore concluded that both intracavitary and external irradiation should be carried out with the patient in the supine position.

Low frequency vibration dilatation of the uterine cervix.

Dilatation of the uterine cervix by low frequency vibrations has been carried out before intra-uterine irradiation with 60Co by the afterloading technique. No complications or technical difficulties occurred. The method seems to be easy, safe, rapid and usually without unacceptable discomfort to the patient. The need for sedatives or general or local anaesthesia was reduced.

Serum fibrinogen/fibrin degradation products in patients with ovarian carcinoma.

Fibrinogen/fibrin degradation products (FDP) in the serum of 50 patients with ovarian carcinoma was examined by Nyléhn's immunochemical method. FDP were rarely found in early but frequently in advanced stages of ovarian tumours. With successful treatment, FDP decreased; otherwise persistently high FDP concentrations were observed in the serum. The determination serum FDP might be a valuable aid in assessing the effect of treatment.
