Prevention of meal-induced heartburn and regurgitation with metoclopramide in patients with gastroesophageal reflux.

Ten patients with gastroesophageal reflux disease participated in a randomized, double-blind, crossover study in which they received a single 20-mg oral dose of metoclopramide or a placebo 15 minutes before a provocative test meal. All patients had previously been challenged with the test meal and had exhibited symptoms of heartburn and regurgitation. Metoclopramide reduced the severity of heartburn from the onset, its effect reaching statistical significance within two hours and persisting for at least five hours. Eighty percent of the patients who received metoclopramide, compared with only 30% of placebo-treated patients, were completely free of heartburn at the end of the trial. A significant reduction in regurgitation during the 1 1/2 to four hours after the test meal was also noted with metoclopramide. No adverse effects occurred. The ability of metoclopramide to prevent the symptoms of heartburn and regurgitation induced by a provocative meal in patients with gastroesophageal reflux was clearly demonstrated.

The value of endoscopy in the diagnosis of lymphoid nodular hyperplasia.

A 16-year-old boy was hospitalized because of abdominal pain, diarrhea, and weight loss. Serum immunoelectrophoresis showed an absence of IgA and diminished levels of IgM and IgG. Radiological examination of the upper gut was suggestive of lymphoid nodular hyperplasia, a diagnosis confirmed by endoscopy and intestinal biopsy. In addition, infestation with Giardia Lamblia was proven by duodenal aspirate at the time of endoscopy.

Resting metabolism of Yucatan miniature swine.

Oxygen consumption, carbon dioxide production, rectal temperature and skin temperatures were measured in six conscious Yucatan miniature swine. Heat production was calculated from oxygen consumption and carbon dioxide production data. Under ambient conditions (20 degrees C), the parameters studied were similar to what has been reported for conventional domestic swine. During 90 minutes of cold exposure (0 degrees C), oxygen consumption increased by approximately 50%. At 0 degrees C, excessive shivering and grunting were noticeable and were associated with decreases in skin and rectal temperature.

Colonic involvement in salmonellosis.

Two cases of Salmonella colitis are described. Clinical and radiological differentiation from ulcerative colitis is difficult and necessitates repeated stool cultures. The management of Salmonella colitis is reviewed.

[The effect of an ambulatory long term training on performance, metabolic and coagulation parameters after myocardial infarct].

It is reported on the results of a training treatment for outpatients after myocardial infarction which was carried out twice a week during one and a half year, compared to a control group without physical conditioning. Out of 30 patients 14 continuously took part in the training lessons. In contrast to the control group in the training group a clear increase of the working capacity, a slight decrease of the body-weight, of the blood pressure in rest and of the triglycerides in the serum as well as a favourable influence on the glucose tolerance could be proved. The serum cholesterol level and the coagulation parameters did not change significantly. One patient of the training group and one of the control group died. One patient of the training group and one patient of the control group suffered a reinfarction.
