[Postoperative complications and criteria of their identification].

A postoperative complication is considered as a secondary process conditioned by the peculiarities of pathogenesis of the primary disease, or as a new disease which develops resulting from diagnostic and therapeutic measures. Timely diagnosis and adequate treatment, and advanced training of physicians as well contribute to reduction in the number of postoperative complications.

[The quality standards of patient treatment in hospital departments].

Standards have been compiled on the basis of the clinical picture of the disease as well as objective, laboratory, instrumental-apparatus data. The dynamics of the patient's health were evaluated using concrete indices--normalization or reduction by 10-30% of the initial systolic pressure, absence of painful sensations, increase of range of articular movements, normalization of body temperature, blood indices, urine and oth. Treatment quality values were determined by comparison of results to the preplanned and are evaluated by a three-point rate--good, fair, poor.

[Control of the quality of therapeutic process in hospital departments].

It is recommended to use the following indices for assessing the quality of hospital treatment: average hospital stay, the percent of repeated admissions with one and the same disease, the percent of discrepancy between clinical and postmortem diagnosis with subsequent expert evaluation of each case of death. The departments of surgery also find it necessary to take into account the frequency of postoperative complications, postoperative lethality and average hospital stay of operated patients. The well-founded complaints per 100 hospitalized patients can also be considered as indices.

[Criteria of quality evaluation of treatment process in hospital departments].

One of the indicators for the evaluation of hospital departments activities envisage the distribution of discharged patients according to the results of treatment: cured, improved, no changes, worsening of the condition. Usually differences arise in defining the indicator "cured". This indicator could be used if besides the recovery and compensation of the affected functions a sick list is closed or would be closed soon after discharge. For non-working population and children account must be taken of restoring the ability to work and behaviour affected by the disease. Among those discharged from the hospital in the fourth quarter of 1988--cured accounted for 34.1 percent, improved--60.4, those with no changes--5.4 and worsening of the condition--0.1 percent.
