Characteristics of patients with schizophrenia successive to childhood attention deficit hyperactivity disorder (ADHD).

The goal of this study was to investigate the characteristics of schizophrenic patients with a history of childhood attention deficit hyperactivity disorder (ADHD). The study was performed on 37 adolescent patients meeting the DSM-III-R criteria for schizophrenia and ADHD and 40 controls with schizophrenia only. Schizophrenic patients who were diagnosed in childhood as suffering from ADHD had more prominent developmental disturbances in infancy, more insidious course of schizophrenia, failed to respond to neuroleptics and had poorer outcome as compared to patients with schizophrenia only. The results of this study indicate that schizophrenia subsequent to childhood ADHD has a poor prognosis as compared to schizophrenia only.

[Clozapine in adolescence onset schizophrenia].

Clozapine is an atypical neuroleptic agent given when schizophrenia is resistant to other neuroleptic drugs. Its use is restricted because of the risk of agranulocytosis, which appears in 1% of cases and therefore necessitates frequent WBC counts. We cite the only 2 surveys of the treatment of adolescents with clozapine and report our experience. We treated 7 boys and 6 girls between the ages of 14-17 years (mean 16.6) with clozapine after failure of all other neuroleptic agents tried. Our usual dose was 300 mg/day, for an average of 245 days. In only 2 patients was improvement only partial. In 1 treatment had to be stopped because of symptomatic orthostatic hypotension which appeared 36 hours after the drug was started. In our opinion, clozapine should be considered when severe schizophrenia in adolescents is refractory to other neuroleptic agents, provided the WBC count is monitored.

Boundaries during adolescence.

This essay looks at the period of adolescence through the dimension of boundaries and examines how adolescence and boundaries affect one another. During adolescence external physical and internal psychological boundaries are blurred. The climate and milieu are well defined and having the environment provide clear boundaries facilitates the passage of this developmental stage.

Severe hypophosphataemia during binge eating in anorexia nervosa.

A patient presented with severe hypophosphataemia that had been precipitated during binge eating. It was corrected by restricting the binges, and by hyperalimentation through a duodenal tube together with intravenous supplementation with sodium phosphate for a short period. Phosphate concentrations should be monitored in patients with severe anorexia complicated by bulimic episodes.

Verbal hallucinations in psychotic patients.

"Verbal hallucinations" are sentences that psychotic patients may say repeatedly throughout a conversation which are out of context or unconnected to the topic of conversation. These hallucinations are not the outcome of a remembrance of an experience or an event and do not bring about any emotional relief or catharsis, but they supply valuable information. They resemble Jacques Lacan's description of the psychotic mechanism "Forclusion." This mechanism relates to experiences that did not undergo the process of primary symbolization through language, and experiences where words were attached but were not bound to the language structure. The result being that these experiences did not enter into the unconscious discourse of the subject. This information can reappear as verbal hallucinations in the psychotic patient. In such cases, the therapist, with the assistance of the patient's family, must investigate the meaning of the verbal hallucinations through research into the patient's and family's history in the phase prior to language development. When such a connection is discovered, the therapist must then bridge the hallucinations with the events unknown to the patient but contained in his subconscious. The therapist's role in such cases resembles that of a parent with a child: To translate the subject's experience through language from the physical schema to the body image and symbolic plane and in so doing, give meaning to meaninglessness. In our paper three short clinical cases are presented.

The hypoparathyroid syndrome: psychiatric observations and case description.

We present a case description of a 15-year-old youth admitted in a psychotic state to the Adolescent Department of Shalvata Mental Health Center and diagnosed as having Idiopathic Hypoparathyroidism. A review of the neurological and psychiatric manifestations accompanying Hypoparathyroid Syndrome is presented, together with several aspects we feel are relevant in reaching this particular diagnosis.

The treatment of adolescents and their parents in group settings in a psychiatric hospital.

This paper describes the structure, composition, function and treatment methods in an adolescent unit containing 16-20 patients, at the Shalvata Psychiatric Center. Treatment is based on organizing the adolescents in a group setting with organized activities. Description of group therapy with the adolescents, their families and group sessions of the team receives special emphasis. Our objectives went beyond a symptomatic improvement and were towards strengthening and integrating the personality. Emphasis was thus placed on creating a therapeutic and educational milieu conducive to new developmental as well as corrective emotional experiences, rather than on economy in length of hospitalization and therapeutic hours invested. This demanded considerably more investment in terms of period of hospitalization and number of personnel than was the usual practice with adult patients in the hospital.

Defensive passivity in adolescence.

There are potentially healthy adolescents who display excessive reluctance to move toward independent decision and action. This paper presents a clinical description of this "syndrome", conceptualizes it as a defensive maneuver against mourning and offers steps for therapeutic intervention.
