[Current status of diagnostic and surgical hip arthroscopy].

Due to the anatomy and topography of the hip, clinical diagnostic procedures are often not very significant. Even the use of highly technical examination methods mostly does not allow differentiation of specific hip problems and do not give exact information about the extent and stage of the lesion. This applies specifically for rheumatoid diseases. Arthroscopy of the hip joint, like other big joints of the extremities, closes this diagnostic gap. Directly visualized findings on the joint, in addition to the results of joint aspiration and histological biopsies, give one security in finding the right diagnosis and planning adequate therapy. The conventional surgical approach to the hip joint leads to large wounds and to corresponding risks and damage to the patient. The advantages of arthroscopy are obvious. It was shown that arthroscopic surgery can be used very efficiently in cases with loose-body, osteochondrosis dissecans, scattered cartilage fragments, or septic arthritis. The other therapeutic application is in cases with rheumatoid arthritis where an arthroscopically assisted synovectomy to the hip achieves a high rate of success without temporary luxation. Diagnostic arthroscopy and arthroscopic surgery to the hip have been used in the Orthopedic Department of the University of Ulm since autumn of 1984. The possibilities and limitations of this surgical procedure have been evaluated in more than 100 cases. Based on our experience with arthroscopy on hip joints, we think that this procedure is very helpful for making a diagnosis and administering therapy.

Pustular osteoarthropathy and its differential diagnosis.

A combination of costo-sterno-clavicular hyperostosis and palmo-plantar pustulosis, sometimes with hyperostotic spondylosis and spondylarthritis, is called pustular osteoarthropathy. In the Western hemisphere 40 cases have been reported and in the Far East the condition occurs more commonly. Five cases are described. Diagnosis is difficult because the skin lesions can occur before the costo-sterno-clavicular hyperostoses. The clinical condition is discussed together with the differential diagnosis. Pustular osteoarthropathy seems to be an enthesopathy and the palmoplantar pustulosis is interpreted as a form of psoriasis.

[Figure skating in high performance sports].

A top performance figure skater is compleled to expose his postural and locomotor system to maximum stress. To achieve the high level of performance that is expected from him, his training schedule must be most exacting. Parallel to this there is an enhanced risk of a sports accident and of damage to health conditioned by sports activities. We report on the results of a study conducted with 67 active and former top performance figure skaters in respect of joint mobility of the lower extremities, sports injuries and damage caused by sports. The type and rate of incidence of sports injuries and sports damage are described and compared with epidemiological data of the average population, the hobby figure skater, and related types of sports.

Semiarthroscopic synovectomy of the hip.

A new surgical procedure, semiarthroscopic synovectomy of the hip is described. The operation enables a radical synovectomy to be performed without the risk of necrosis of the femoral head from temporary luxation. The early results have been encouraging and the method appears to offer a low-risk alternative to conventional radical synovectomy.

[Sonography of Achilles tendon changes in athletes].

Sonography of the achilles tendon is a sure diagnostic method. It allows to recognise different pathological changes in the tendon. In a screening study we performed sonography of 156 achilles tendons in 78 sportsmen. We found a correlation between the results of clinical investigations, pain and sonographic findings. In some cases sonography showed structural changes in the achilles tendon without subjective complaints in the patients. Besides the pathological changes of the achilles tendon we observed an increasing caliper and echo density of the tendon in elderly patients. The delineation of the achilles tendon against the surrounding tissues was superior in younger patients. Disorders of the achilles tendon which had occurred in the patients on a previous occasion resulted in structural changes of the tendon in 75% of the cases, retrospectively identified by ultrasonography.

[Sports in advanced age in inflammatory rheumatic diseases].

The positive effect of moderate physical activity on cardiovascular system, psychological condition, immune reactions and joint biology are wellknown. They seem to facilitate pain control and to stimulate the metabolism of skeletal tissue and joints also in elder patients with rheumatoid arthritis if certain prerequisites are considered. The possible influence of physical activity on the natural course of the disease has not yet been investigated. The recommended type and extent of sports activity therefore have to be carefully selected with regard to the pathogenesis of the disease and their inflammatory changes as well as the patients individual medical conditions.

[New possibilities in the detection and surgical therapy of synovitis of the hip joint].

Missing pathognomonic clinical and radiological signs, an early diagnosis of hip joint synovitis is often impossible. Ultrasonography and arthroscopy of the hip joint are useful methods to accomplish recognition and exact description of chronic inflammatory hip joint diseases. These new diagnostic methods enable an early adequate treatment. Until today synovectomy of the hip joint is still a rare surgical procedure. Reasons are the difficulties in diagnosis of the synovitis and the insufficient radicality of synovectomy if the femoral head is not luxated temporarily. The semi-arthroscopic synovectomy of the hip joint allows a radical removal of the synovial membrane even in the fossa acetabuli, avoiding the inherent risks of a luxation of the femoral head.
