Ordinary and high-dose ipratropium in perennial nonallergic rhinitis.

The aim of the present study was to test the efficacy of the topically active cholinoceptor antagonist, ipratropium, in the treatment of rhinorrhea in perennial nonallergic rhinitis, with special reference to identification of subgroups of responders and increased efficacy from high-dose therapy. Thirty-six adult patients with watery rhinorrhea as a dominant symptom completed the study, which consisted of a 2-week run-in period followed by two 3-week treatment periods with placebo and ipratropium in an ordinary dosage (80 micrograms four times a day) in a double-blind, crossover design, and, finally, an open 2-week period with high-dose therapy (400 micrograms four times a day). The number of nose blowings was 47% lower during treatment with ipratropium in the ordinary dosage than during the placebo period (p less than 0.001). There was an additional reduction during high-dose therapy that was slight but statistically significant (p less than 0.05). Ipratropium had no effect on the number of sneezes or on nasal blockage index. During ordinary-dose therapy, side effects were slight and confined to the nose, whereas the high-dose therapy caused unpleasant nasal dryness and, in a few cases, systemic side effects. It was not possible to separate responders from nonresponders by case history, physical examination, or nasal methacholine testing. It is concluded that intranasal ipratropium is effective in the treatment of watery rhinorrhea in perennial nonallergic rhinitis and that 320 micrograms a day is sufficient in most patients.

Long-term results after surgical treatment of laryngeal stenosis in small children.

Thirteen severe cases of laryngeal stenosis were operated on at the ages of 1-3 years. The operations were performed by making a laryngofissure in the front part of the larynx and excising the stenosing scar. The back wall of the larynx was incised in the midline and the two halves of the cricoid lamina were separated from each other. An indwelling prosthesis was placed in the larynx for 4 months. All patients but one were decannulated. The positive clinical result of the operation proved to be permanent through the rapid growth phase of puberty in 7 patients out of the 10 who had reached puberty. Three patients developed a gradually worsening laryngeal stenosis during the rapid growth phase of puberty. One of them was successfully re-operated. The mean follow-up time was 12 years. Ventilatory function tests were performed on 9 patients, 4 of which proved to have normal ventilatory function or only slight airflow limitation in central airways.

Transantral orbital decompression in the treatment of Graves' disease.

A series of twenty patients operated on for severe endocrine exophthalmos is presented. All the operations were performed by removing the orbital floor, making a total ethmoidectomy and removing part of the inferior orbital margin; in some cases, a section of the lateral orbital wall was also removed. No serious complications were detected. In all but one patient there was an immediate, marked decrease in exophthalmometry. A marked decrease of the inflammatory signs of the conjunctivae was also observed within a week following surgery.

Orbital decompression in endocrine exophthalmos of Graves' disease.

Transantral decompression was performed bilaterally in 27 and unilaterally in 3 patients with endocrine exophthalmos of Graves' disease. In 28 patients there was an immediate reduction of proptosis and in about half of the patients in addition a marked decrease in chemosis and conjunctival injection indicating that these signs were mostly due to orbital vascular congestion. In one patient with exophthalmos of more than 2 years duration and progressive swelling of the eye muscles no response was observed. In another patient decompression did not reduce proptosis which, however, 4 months later responded to retrobulbar irradiation. In one further patient much more marked reduction of proptosis and disappearance of persistent periorbital swellings were obtained after glucocorticoid treatment given half a year after decompression. Postoperatively diplopia occurred in about half of the patients but corrective operations were required only in a quarter of the patients. Transantral decompression is an effective method for rapid treatment of progressive exophthalmos of Graves' disease. In patients with unilateral exophthalmos the asymmetry may be reversed after unilateral operation. The authors use decompression not only in the most severe cases (categories 5 and 6a-c according to the classification of the American Thyroid Association) but also in less severe cases (categories 2b-c and 3b-c) when there is a steady progression as a primary treatment possibly combined with other forms of therapy.

Laryngeal stenosis following papillomatosis--a report of three severe cases.

Three cases of severe laryngeal papillomatosis beginning in early childhood are presented. Several methods of treatment were tried, but lasting results were achieved by suction diathermy. The laryngeal lumen was totally obliterated in all cases during the course of the disease. The patients were successfully operated on by making a laryngofissure in the front part of the larynx, excising the scar and enlarging the lumen by a median incision in th back wall of the larynx. An indwelling prosthesis was placed in the larynx for several months. After removing the prosthesis, the laryngeal lumen was dilated several times by oesophageal sounds. All patients were successfully decannulated. The results of treatment are evaluated both clinically and by ventilatory function studies. Central airway obstruction persisted in two cases.

Ipratropium (Atrovent) in the treatment of vasomotor rhinitis of elderly patients.

Ipratropium is a parasympatholytic agent with high topical activity. The effect of intranasal ipratropium was studied in 34 patients who were over 60 years of age and who suffered from chronic watery vasomotor rhinorrhoea. The study was a double-blind cross-over placebo controlled trial. Trial parameters were: daily nasal symptom and side effect scores, nasal methacholine testing before and after treatment, and clinical evaluation. The active drug clearly reduced nasal discharge. Nasal methacholine testing showed reduction of nasal secretion after the active treatment period. The majority of the patients preferred the active drug to the placebo. Excessive drying of the nasal mucosa was the most common side effect. Ipratropium seems to have a beneficial effect on vasomotor rhinitis, which often is an irritating condition in elderly patients.

Astemizole in the treatment of hay fever.

A double-blind parallel trial of astemizole, pheniramine and placebo was carried out in 51 patients with hay fever. Astemizole is a new potent H1-antihistamine with long duration of action but devoid of central activity. Evaluation criteria were daily symptom- and side effect score cards, daily nasal peak flow measurements, clinical examination and blood tests. Control of nasal symptoms was significantly better with both antihistamines than with placebo. Tiredness and anticholinergic side effects were less common in the astemizole group than in the pheniramine group. No significant changes in the "safety evaluation" blood tests were observed.

Surgical treatment of chronic laryngeal stenosis secondary to vocal cord paralysis: pre and postoperative evaluation of ventilatory function.

Thirteen patients with chronic laryngeal stenosis were operated upon with total excision of stenotic scar tissue, enlargement of the lumen by sectioning the back and front wall of the larynx, and application of an indwelling Teflon prosthesis for 4 months. The stenosis was in many cases caused by long-standing recurrent paralysis complicated by attempted surgical correction of the stenosis. Following operation, the specific airway conductance increased in all patients (mean +/- SD pre and postoperative values 0.51 +/- 0.30 and 1.12 +/- 0.51 kPa-1.s-1, respectively). The FIV1/FVC ratio was determined in 9 patients and an increase was postoperatively noted in all of them (mean +/- SD pre and postoperative values 43 +/- 17 and 76 +/- 13%, respectively). The physiological findings confirmed the excellent clinical results of the treatment.

Inferior dacryocystorhinostomy in severe congenital dacryostenosis.

Inferior dacryocystorhinostomy is a simple and effective method in the treatment of such cases of congenital dacryostenosis in which conservative treatment and probing have failed and in which the atresia is located at the level of the nasolacrimal duct. The operative technique is described in detail. The series treated by this method consists of 8 patients, 3 of whom showed bilateral stenosis. On 2 patients adenoidectomy was performed in connection with the dacryocystorhinostomy. The follow-up time ranged from 2 to 34 months, and a good result was obtained in 7 cases. In one case the passage produced closed soon after the operation. Endonasal dacryocystorhinostomy was then successfully carried out by West's method. The operative technique described is well suited for patients under two years of age.

Serum lipids, postheparin plasma lipase activities and glucose tolerance in patients with prolactinoma.

UNLABELLED: Serum total cholesterol and triglyceride levels were determined in forty-seven women with prolactinoma and in eighty-four age- and weight-matched control women. Oral glucose tolerance tests (OGTT) were performed and postheparin plasma lipoprotein lipase (LPL) and hepatic lipase (HL) activities were determined in twelve patients before and after transsphenoidal removal of the prolactinoma. The mean levels of serum cholesterol and triglyceride were significantly higher in patients than in controls. The 90% cut-off line of controls for serum cholesterol was exceeded by 36% and that for serum triglyceride by 23% of the patients. The triglyceride levels were raised only in patients with GH-deficiency whereas patients with normal GH secretion had normal triglyceride. Plasma LPL activity was significantly reduced whereas plasma HL activity was in the upper range of normal. After the removal of prolactinoma the serum prolactin levels decreased in all patients and seven started to menstruate. The oral glucose tolerance was improved and the plasma insulin response decreased. Serum lipid levels and the lipase activities, however, did not change. CONCLUSION: prolactinoma is associated with metabolic abnormalities characterized by hyperlipidaemia, low plasma LPL activity and insulin resistance.

Recurrence of chromophobe pituitary adenomas after operation and postoperative radiotherapy.

The rate of recurrence is reported in a prospective study of 56 patients (28 men, 28 women) with large chromophobe pituitary adenoma (with or without hyperprolactinemia). The surgical approach was transfrontal in 44 and transseptosphenoidal in 12 patients. Cryoapplication was combined with the transsphenoidal operation. All but one patient received postoperative pituitary irradiation. Altogether, 11 (20%) clinical relapses (10 men) occurred between 0.5 and 6 years after the transfrontal operation. Patients that relapsed had had larger tumors than those remaining in remission. Occurrence of the tumors appeared with a deterioration of the visual field defect in 9 patients. There were no differences in the degrees of hypopituitarism in patients who relapsed as compared to patients remaining in remission.

Budesonide in the treatment of nasal polyposis.

A double-blind parallel trial of budesonide versus placebo was carried out in 19 patients suffering from nasal polyposis. The patients were randomly assigned to two groups, one receiving a daily dose of 400 micrograms of budesonide and the other a corresponding dose of placebo intranasally for 4 months. The total mean symptom scores and nasal peak flow values during the trial showed a statistically significant difference in favour of budesonide. On rhinoscopy, a clear decrease in mucosal congestion was noticed in the group receiving the active drug. The polyps diminished in number and size in the budesonide group. Mucociliary function, measured by the saccharin test before and after therapy, did not undergo significant changes in either group. Mean plasma cortisol values were of the same order before and after therapy. Local side-effects were mild in both groups, and there was no indication of mucosal drying or crusting attributable to the test solution. Pre- and post-therapy biopsies revealed a pronounced decrease in tissue eosinophilia following budesonide treatment. Neither nasal smears nor biopsies showed signs of morphological changes in the epithelium.

Pituitary function in prolactinoma. Effect of surgery and postoperative bromocriptine therapy.

Forty-five women and fifteen men with prolactinomas have been treated surgically. Patients with large tumours received pituitary irradiation and postoperative hyperprolactinaemia was treated with bromocriptine. The patients have been followed-up for 6-36 months following the operation. The tumours were larger and the levels of production higher in men as compared with women. All women had amenorrhoea. Galactorrhoea was present in forty-three women but not in the men. After surgery serum prolactin levels fell significantly in all women but remained above normal in thirty-six; prolactin remained high in twelve men. Bromocriptine effectively decreased the postoperative hyperprolactinaemia. The surgical complications were oculomotor nerve paresis in one woman and one man. After surgery six (23%) women developed impaired GH secretion, six (15%) impaired thyroid function, eight (18%) impaired cortisol secretion and five (17%) impaired LH secretion in isolation or combination which had not been present preoperatively. Three patients relapsed. Fifteen women menstruated after surgery and ten began to do so during the subsequent bromocriptine treatment. Thus, menstruation was restored in all six women with microadenomas in sixteen of twenty patients with intrasellar macroadenomas and three of nineteen patients with suprasellar adenomas. The preoperative LH-reserve proved to be an important prognostic indicator. Nine patients, i.e. 50% of patients desiring fertility became pregnant. In the men gonadal function deteriorated in four patients and did not improve in any without testosterone treatment.

[Treatment of chronic cicatricial laryngeal stenosis (author's transl)].

Eighty severe cases of chronic laryngeal stenosis in patients aged 1--78 years, where all other treatment had failed, were operated upon with laryngofissure of the front and back wall of the larynx followed by excision of the stenosis scar tissue. An endolaryngeal prosthesis was inserted and left in position for four months. The clinical result was good in 69 patients but, for various reasons, unsuccessful in eleven. The results of the treatment was also investigated by spirometry and body plethysmography. Contraindications for this operation are presented. The mean follow-up time was 6.9 years.
