[Current diagnosis and procedure classification].

Classifications of diagnoses and medical procedures support structurized medical documentation and make it accessible to computerized evaluation. Important figures of performance of in- and outpatient medical care can be derived from these data. Actually the importance of medical classifications increases in Germany because of the decision for introducing a new accounting system for hospitals on the basis of Australian AR-DRGs. The following review describes in detail these medical classifications which are prescribed by law in Germany (ICD-10 respectively ICD-10-SGBV and OP-301 [ICPM]). Some of their most important predecessors are also being referred to and discussed in the context of medical documentation. Some ideas are given about the further development of these medical classifications (e.g. PCS) and the medical documentation, taking into account especially the future DRG situation in hospitals.

ICD-10-SGBV and ICD-10-diagnosenthesaurus--advantages and disadvantages as well as further development.

In the year 2000 ICD-10-coding of diagnoses in ambulatory and hospital health care was legally introduced in Germany. Whereas the complete ICD-10 is used for coding of mortality, physicians got the special version "ICD-10-SGBV" for coding of diagnoses of treated patients. First experience with this version shows the necessity for its further improvement. In addition, the so-called ICD-10-Diagnosenthesaurus was developed and proved to be a very useful aid for easier and better encoding of diagnoses. It is not only widely used in Germany but also in Austria and Switzerland. National and international efforts are necessary to improve ICD-10 and its utilisation.

German adaptations of ICD-10.

The introduction of the ICD-10, published by WHO 1992-94 in English and by DIMDI 1994/95 in German, is a very slow process. Some states introduced ICD-10 for the preparation of statistics of mortality, but only few use it for morbidity. ICD-9 is in Germany only in hospitals still in use. Much effort was put into the improvement of the official ICD-10 and the development of additional aids for more simple and better encoding of diagnoses. Thus a revision especially for ambulatory health care (ICD-10-SGBV with the incorporated ICD-10-Basisschlüssel) and a collection of German terms and expressions of diagnoses that are not at all part of the official ICD-10 (ICD-10-Diagnosenthesaurus) were published. Three years ago a conversion table ICD-9/-10 was developed which can now be harmonised with WHO's Translator. The experiences with all these instruments are satisfying. The development of methods for automatic encoding of free-text phrases of diagnoses has now been started.

[Orthopedic basic documentation: requirements of diagnostic statistics and results in Göttingen university clinics].

For the first time the revised "Bundespflegesatzverordnung" (Federal Statutory Instrument on Hospital Care Rates) of 1985 has ordered all the hospitals in the F.R.G. to prepare annual statistics of diagnoses giving as part of the anonymous hospital statistics an overview about the medical variety of inpatients and enabling scientific evaluation of cases. The most important documentation items are patient's age, duration of stay, main diagnosis encoded according to ICD-9, and operation characteristic (whether or not any of the operations was carried out because of the main diagnosis). The computerized Goettingen documentation comprises all 15 items of the Minimum Basic Data Set of the European Communities and a few other additional details. The result is a comprehensive and in many directions useful medical documentation for which examples are given.
