Two illustrative cases with different features of cardiac injury caused by blunt chest trauma are described. The first patient had mild and obscure symptoms, detected on physical examination, and required observation only. The second patient had acute pericardial tamponade, necessitating surgical treatment. We present the different medical procedures that should be taken into consideration in management of such cases, although continuous monitoring, repeated physical examination, electrocardiograms, chest x-rays, and echocardiography proved sufficient in managing our two children. It is important that physicians who provide care to children suffering from blunt chest trauma have increased awareness of possible cardiac injuries.
Paralytic poliomyelitis presenting with quadriparesis, transient encephalitis and bulbar symptoms in 2 patients in close contact with recently vaccinated children with trivalent live oral polio vaccine is described. Symmetrical lower motor neuron involvement of deltoid muscles with electromyographic confirmation was found. Upper motor neuron signs, with symmetrical hyperactive deep tendon reflexes developed in the lower extremities. Poliovirus Type-2 vaccine-like strain was cultured from one patient and both patients showed significant antibody titers rises to poliovirus. Attention is drawn to the possible clinical differences between vaccine associated poliomelitis and the usual features found in wild strain poliomyelitis. It is suggested that in selected cases, non-immunized contacts be given inactivated polio-vaccine when the vaccinees are immunized with the live oral-vaccine.
Synthetic oligo(ribo-deoxyribo)nucleotides were analyzed and characterized by different solid-phase chemical degradation procedures, 5'- and 3'-end labelled mixed fragments were degraded by a slightly modified DNA cleavage procedure using 1 and 10% piperidine for the chain scission reaction and CCS anion-exchange paper. Besides the normal degradation products obtained by the usual modification and strand cleavage reactions of both deoxy- and ribonucleotide residues, additional bands were identified in the sequence patterns resulting from the hydrolysis of the RNA moiety induced by piperidine. Since both degradation reactions cleave the backbone of the mixed DNA-RNA fragments differently and produce nucleotide components with different charges, the degradation products do not interfere and can be resolved by gel electrophoresis on polyacrylamide. In addition, 3'-end labelled DNA-RNA oligomers were degraded by a RNA cleavage procedure using DE 81 anion-exchange paper as solid support. The combination of all three degradation methods allows to confirm the nucleotide sequence.
Accidents related to blood transfusion have become rare in neonates, due to the application of very strict rules. The 8 reported cases of post-transfusion hemolysis occurred in prematures less than 32 weeks gestational age. Signs consisted of hemoglobinuria and/or severe jaundice. In some patients exchange-transfusion had to be performed. Immuno-hematologic, bacteriologic and technical investigations did not show the etiology of these accidents. No similar clinical reports were found in the literature. Some authors suggest a mechanical origin.
The ultrastructural integrity of endothelial tight junctions in cerebral microvessels of the rat exposed to hyperbaric oxygen was examined to find whether or not there are alterations of the blood-brain barrier in the initial phase of central nervous system oxygen toxicity. Rats were pre-implanted with cortical electrodes for continuous EEG and provided with two polyethylene cannulae inserted into the common carotid artery. HRP was administered to unanesthetized rats through a cannula directed into the brain, before the onset of O2 pressurization. As soon as the early electrical discharges of oxygen toxicity were recorded (20 to 30 min), the animal was killed by injection with saturated KCl into the heart. Examination of small blood vessels in both LM and EM did not support the possibility that the blood-brain barrier is altered prior to the first electrical discharge appearing in central nervous system oxygen toxicity.

Protein-RNA interactions in the 5S rRNA-protein L5 complex from rat liver ribosomes were studied by limited digestion of free and protein bound 5S rRNA with ribonuclease A and T1. In the complex with protein L5 the digestion of 5S rRNA by ribonuclease T1 is decreased at G37 and G89, whereas U38 and C39, and to a lower extent also C10 and U12 become accessible for ribonuclease A.
A solid-phase method for simultaneous sequencing of different long RNA fragments has been developed using Whatman DE 81 anion-exchange paper as the support. The approach involves 8 operations including: immobilization of heat-denatured 3'-end labeled RNA fragment on DE 81 paper; washing; modification reactions; washing; aniline reaction; washing; RNA desorption by salt and ethanol precipitation. For modifying the RNA, the following reactions were selected for the routine: G with dimethylsulfate at 90 degrees C/sodium borohydride at 0 degrees C, A + G with diethylpyrocarbonate at 90 degrees C, U + C with hydrazine at 0 degrees C and C with hydrazine/5M NaCl at 0 degrees C. The losses of RNA material during the reactions with large excess of reactants were 50% during the reduction with NaBH4 and 30% during C-reaction. Almost no losses were observed during aniline reaction. The RNA could be recovered by desorption with 2M NaClO4 in 50-70% yield. The whole solid-phase procedure up to the sequencing gel takes about 2 hours and is much faster and more convenient than chemical RNA sequencing in solution according to Peattie, especially if many fragments are to be processed.
There is an urgent need for psychiatric services for mentally ill inmates in local jails. The authors recommend that a psychiatric team be established inside the jail to provide short-term crisis evaluation, disposition, and treatment; that special training be provided for jail booking personnel in the recognition of psychiatric problems during the initial jail screening and classification process; that special modules be developed for mentally ill inmates; and that inmates in need of hospitalization have easy access to hospital beds. They describe a psychiatric ward actually located within a county jail to treat mentally ill inmates who are considered security risks.
Binding of bisdiazoniumbenzidine cross-linked (BDB) and monomeric IgG to washed human platelets at 37 degrees C was assessed. Metabolic inhibitors and prostaglandin E1 permitted study of partially reversible and saturable binding of the ligands to platelets: in presence of 0.25 microM antimycin A and 6 mM deoxyglucose, maximal binding of 3H-BDB-IgG (10 min, 37 degrees C) was 0.52 fg per platelet and of 3H-IgG (20 min, 37 degrees C) 0.13 fg per platelet. In presence of 6 mM glucose, however, the amount of IgG bound per platelet was higher and not saturable. These findings suggest that IgG is actively engulfed by the platelets.
Cross-linking of proteins within the small subunit of rat liver ribosomes by the bifunctional reagent dimethyl 4,7-dioxo-5,6-dihydroxy-3,8-diazadecanbisimidate produced numerous covalently linked protein dimers which could be separated by a combination of ion-exchange chromatography on carboxymethyl cellulose and polyacrylamide gel electrophoresis. The protein components of the dimers were identified electrophoretically after periodate cleavage of the cross-link(s). The analysis revealed 42 cross-linked dimers involving 25 different proteins. Among these, proteins S3, S4 and S20 occurred in combinations with six, eight and seven different proteins, respectively. For proteins S13, S14 and S17 five protein neighbours could be identified, while 13 of the remaining proteins were linked to three or four different protein partners. The involvement of the majority of proteins in the formation of multiple cross-linked dimers implies that a large number of protein-protein interaction sites exist within the ribosomal subunit. A preliminary model illustrating the arrangement of 16 proteins in the small ribosomal subunit is presented and discussed with respect to possible functions, especially in the event of translation initiation.

Seven different immunoglobulin G (IgG) preparations for intravenous use were tested for their capacity to inhibit serotonin-release from washed human platelets induced by IgG coupled with bis-diazobenzidine. Using a reference standard consisting of the top third fraction of an ultracentrifuged gammaglobulin preparation for intramuscular use, two chemically treated preparations were less potent inhibitors than the standard while two preparations, one pH 4 treated and another albumin protected, were better inhibitors. A pepsin treated preparation was devoid of inhibitory capacity, whereas Fc fragments derived from human IgG were extremely efficient. Evidence is discussed that the inhibitory capacity is inversely correlated to the content of oligo- and/or dimeric IgG molecules.

Cross-linking of proteins within the small subunit of rat liver ribosomes by the bifunctional reagent dimethyl 3.8-diaza-4.7-dioxo-5.6-dihydroxydecanbisimidate results in the formation of various complex which were separated by two-dimensional polyacrylamide gel electrophoresis. Components of four complexes were electrophoretically identified after periodate cleavage as S2--S3, S3--S3a, S15--S15a, and S5--S25.
The incubation of liver microsomes or mitochondria with glutathione, in the presence of electrophilic compounds, decreased the glutathione concentration in the incubation medium. Product analysis revealed that glutathione conjugates were formed.
