What happens to patients after nursing home placement?

Follow-up of patients discharged from hospitals to nursing homes is often difficult for the busy practitioner to implement. A one page follow-up telephone questionnaire was designed by a hospital social work department to monitor the outcomes of patient transfers to nursing homes at one and three months post-hospital discharge. Time to complete the interviews averaged five minutes. This instrument is designed to inform social workers of the disposition of patient transfers and to monitor client satisfaction. Professional accountability and practice research are addressed in the follow-up process and the implementation of a simple, systematic data gathering instrument.

Using demographic data for ambulatory health care planning: a dental health practice management model.

The use of sociodemographic data in planning ambulatory health services is discussed and illustrated. Five global indices are identified as important for establishing contours of need within local community areas: social class, population heterogeneity, resident mobility, family organization, and general stress factors. Knowledge of sociodemographic distributions within a given community can serve as an adjunct for rational decision making in planning and placement of ambulatory health care services. It can also establish a means for evaluating whether extant health services reach their intended targets via comparisons to the social demography of patients receiving care in private practices or public clinics. Such analyses are germane to ambulatory health care practitioners in both the public and private sector.

"Burnout": definitions and health care management.

This paper examines the existing definitions and perceptions of the concept of "burnout" in the health professions. The authors suggest that the understanding of "burnout" is hindered by the lack of a single operational definition and a clear set of criteria. An example of social workers in a community mental health setting serves as an illustration.

An assessment of dental care for the underserved.

Despite changes and improvements in the delivery of dental-care in the United States, almost half the population is "by current standards of dental practice, a medically underserved population." The impact of fluoridation has reduced the incidence and resultant prevalence of dental caries, and insurance and alternative payment systems have helped to improve access to dental care for many Americans, but a large underserved population has not reaped the full benefits of these positive developments. This paper analyzes some major inequities of dental-care delivery in the United States and proposes some policy solutions.
