Prevention of hepatitis B virus in athletic training.

Hepatitis B virus (HBV) infection is highly communicable and is the leading cause of acute and chronic liver disease worldwide. In recognition that 10,000 to 15,000 health care workers are becoming infected with HBV annually, the Occupational Health and Safety Administration has instituted strict regulations and guidelines concerning the handling of blood-borne pathogens. Due to the exposure to blood-borne pathogens and potentially infectious materials, athletic training is an allied health care profession that has an increased risk of exposure to HBV. Therefore, it is essential that athletic trainers employ extensive preventive strategies to decrease the exposure to this health-and life-threatening infection.

Prevention of osteoporosis by medroxyprogesterone acetate in postmenopausal women.

The effect of medroxyprogesterone acetate 10 mg BID alone, conjugated estrogens alone or in a combination regimen for the prevention of osteoporosis was determined in 36 postmenopausal women using single photon densitometry. No significant differences in cortical or trabecular bone mass over time were detected in women between the three treatment groups, although a slight increase in bone mass was noted in women with the combined therapy. Medroxyprogesterone acetate appears efficacious in preventing postmenopausal osteoporosis, and may be especially useful in women with contraindications to estrogen replacement therapy.

Spontaneous rupture of pyometra due to leiomyomata. A case report.

Pyometra, an accumulation of purulent material in the uterus, is a rare pathologic entity. Spontaneous rupture of pyometra occurred secondary to degenerating leiomyomata; that is an extremely rare complication. Pyometra, the diagnosis of which is based on the classic symptoms of uterine enlargement, vaginal discharge and acute abdomen, requires rapid supportive therapy and surgical intervention.

Clinical efficacy of cefoperazone in obstetric and gynecologic infections.

Compared with second-generation cephalosporins, cefoperazone, a third-generation cephalosporin, has demonstrated an extended antibacterial spectrum. In addition, it has a long serum half-life and requires only twice-a-day administration. In this study, 66 patients with clinically diagnosed obstetric and gynecologic infections were treated with cefoperazone, and 61 were evaluated for efficacy. An overall cure rate of 93% (57 patients) was achieved. In view of the excellent clinical response and convenience of twice-a-day administration, we conclude that cefoperazone is a clinically useful single agent for the initial empirical treatment of patients with obstetric and gynecologic infections.

Use of tumor colony assay in clinical oncology.

We have evaluated the usefulness of the tumor colony assay in predicting chemotherapeutic drug response in our cancer patient population. We found that a wide variety of human tumors will produce clonal growth in this in vitro assay. Low growth rates in many of the common human tumors, however, severely restrict the utility of this assay in a large number of cancer patients. A retrospective analysis using the assay to predict anticancer drug response revealed a true-positive predictive rate of 63% and a true-negative predictive rate of 96%. We conclude that if growth stimulants can be developed to enhance clonal growth without altering the predictability of the assay, the tumor colony assay could prove to be extremely useful in selecting appropriate chemotherapy for individual cancer patients.

Amenorrhea following oral contraception.

A prospective study was done on 326 nulliparous women who ceased oral contraception. Follow-up was obtained on 311. The mean age was 20.3 years (S.D. 2.5). Eighty-nine per cent began menstruating within 60 days after stopping therapy, and only 7 took 180 days or longer to menstruate. Late menarche strongly correlated with post-pill amenorrhea (PPA), but continous length of time on oral contraceptives and type of oral contraceptive showed no significant relationship. The incidence of PPA was 2.2 per cent. All women did eventually menstruate spontaneously, the longest period of amenorrhea being 540 days.

Effects of haemoperfusion through charcoal or XAD-2 resin on an animal model of fulminant liver failure.

In a group of dogs in whom fulminant liver failure had been induced, perfusion of blood through activated charcoal resulted in a significantly longer survival than that of a similar group of dogs whose blood was not so treated. An otherwise progressive rise in blood ammonia concentration was halted in the treatment group. In another group of dogs with fulminant liver failure perfusion of blood through the resin Amberlite XAD-2 was associated with a fall in the serum bilirubin concentration and complete clearance from the blood of (14)C-labelled sodium glycocholate. Survival in this group of animals was not significantly prolonged. This was due at least in part to the occurrence of haemorrhage due to thrombocytopenia. Platelets adhere to the resin but do not adhere to the same degree to charcoal coated with a thin layer of polymer.
