The efficacy and safety of flumazenil were assessed in comparison to placebo in a double-blind randomised study of 31 adults intoxicated with benzodiazepines. The criteria of efficacy were the degree of sedation, and orientation in time and space. Patients who received flumazenil awoke within minutes but central depression returned partly one hour later, which reflects the short elimination half-life of the drug. Side effects were few and the results indicate that flumazenil is effective in the primary management of benzodiazepine overdose and in states where benzodiazepines have been taken with other drugs.

The purpose of the investigation was to assess the extent of nitrous oxide pollution during bronchoscopy (rigid ventilation bronchoscope) and further to elucidate the possible reduction in air pollution during local scavenging of leaking gas by a suction catheter placed in the pharynx. The study included 14 adult patients. The leakage of anaesthetic gas was 11.4 +/- 3.2 l min-1 and the median value of the nitrous oxide concentration in the breathing zone of the endoscopist was greater than 300 ppm. Air pollution could not be reduced significantly by scavenging with a suction catheter. The use of inhalation anaesthetics is therefore inadvisable during bronchoscopy in adults unless sufficient anaesthetic scavenging can be established. Other ways of reducing air pollution are discussed and the use of total intravenous anaesthesia or local anaesthesia is advocated.
Fine needle aspiration from the thyroid gland was performed in 10 patients with multinodular euthyroid goitre. This was done to evaluate the possibility of obtaining sufficient quantities of thyroglobulin by this procedure to allow biochemical characterization. For comparison aspiration from normal thyroid tissue was performed in seven subjects during operation for solitary thyroid lesions (cancer or cysts). The total amount of Tg in aspirate was, goitre 666 +/- 171 micrograms (mean +/- SE), normal 177 +/- 52 micrograms; purified Tg contained, goitre vs. normal: iodine 9.1 +/- 2.2 vs. 26 +/- 3.9; T4 0.86 +/- 0.39 vs. 4.0 +/- 0.7; T3 0.080 +/- 0.024 vs. 0.28 +/- 0.06 mol/mol Tg (all significantly different), compatible with iodine deficiency pattern in Tg from the patients with goitre. Similar values were obtained if iodine and iodothyronines were measured in crude aspirate (after alkaline ashing and pronase hydrolysis, respectively) and related to the amount of Tg in aspirate measured by radioimmunoassay; thus avoiding the cumbersome purification of Tg. Hence fine needle aspiration of the thyroid can be used to obtain Tg for biochemical analyses. This considerably facilitates Tg studies in humans for clinical or investigative purposes.
