[Timing of therapy of cryptorchidism].

Dysfunctions of the endocrine glands are among the most prominent causes of cryptorchidism. Whereas all gonads show spermatogonia during the first to sixth months of life, loss of spermatogonia is seen in 25% of children with unilateral cryptorchidism already in the second year of life. A randomised study revealed decreased in gonadotropin reserves which was more marked after unsuccessful treatment than after successful therapy. In 119 treated children, the quota of descended testicles was between 47 and 55%. Ultrastructurally, LH-RH exercises an action on Leydig's cells. Treatment should be initiated already in the first year of life. LH-RH is recommended as the first conservative treatment. If this is not successful, additional administration of chorionic gonadotropin (HCG) can achieve descent in an additionally high percentage of cases.

[Segmental colonic atresia - a contribution to the differential diagnosis of ileus in the newborn (author's transl)].

Segmental atresia of the colon is regarded as an urgent emergency in the neonatal period and quick diagnosis and surgical interference are necessary. In spite of the rarity of this condition it could be thought about in the differential diagnosis in infants with relevant symptoms and can be diagnosed by simple means. The author's own technique is described in relation to the case of atresia of the transverse colon. The aetiology, classification, clinical picture, diagnosis and treatment are discussed. The prognosis depends very much on early diagnosis.

[The effect of AR-L 115 BS on contractility dynamics of the isolated, metabolically supported dog heart (author's transl)].

In this study the contractility dynamics of the left ventricle are analysed independent of respiration, pre- and afterload, heart rate and perfusion pressure. These parameters alter varyingly following the administration of 2-[(2-methoxy-4-methylsufinyl)phenyl]-1H-imidazo[4,5-b]pyridine (AR-L 115 BS) so that the study plan selected must take these factors into account. The model employed consisted of a metabolically supported isolated pulsating dog's heart. The dose of AR-L 115 Bs used was 0.125 mg/kg body weight and it was infused over a period of 30 min. Altogether the study demonstrates that AR-L 115 BS has a marked positive inotropic effect on the isolated heart even under the conditions of infusion. The coronary resistance diminishes and the cardiac output increases.

Effect of LH-RH treatment on hypothalamo-pituitary-gonadal axis and Leydig cell ultrastructure in cryptorchid boys.

62 cryptorchid boys aged 2-6 years were selected at random either for surgical or for hormonal LH-RH treatment. As all biopsies from boys operated show typical histological and ultrastructural signs of cryptorchidism, it can be concluded that only true cryptorchid patients are included in our study. LH-RH treatment was successful in 16 (55%) of 31 boys. Median 30-min response values to LH-RH test of LH were initially normal in all boys. Those treated successfully remain normal after treatment whereas unsuccessfully treated patients have significantly lower LH 30-min response values at the end of the 4 weeks of LH-RH treatment. FSH response was not statistically different before or after treatment. This might indicate that a normal LH response is necessary for testicular descent. In the 15 boys operated after 4 weeks of unsuccessful LH-RH treatment, testes biopsies show recruitment of the Leydig cell precursors and development of juvenile Leydig cells with an increase in their content of lipoid droplets and smooth endoplasmic reticulum. No adverse effect on the number of spermatogonia could be observed after 4 weeks of LH-RH treatment.

[Mesenteric plication by the Childs-Phillips method for the prevention and therapy of small intestine obstructions. Experiences in 148 cases].

Transmesenteric small bowel plication (Childs-Phillips) was performed in 148 patients (99 adults, 49 children) at the University Clinic of Würzburg over a 10-year period. Of a total of 31 reinterventions, only 4 were caused by the method; recurrent intestinal obstruction occurred in 6.1%. In a total of 51 patients peritonitis was present at the time of mesenteric plication, which is not considered a contraindication for the method. Early postoperative mortality was 38.5%, but in none of these 57 cases was it due to the method itself.

Preoperative condition of neonates who have ultimately died.

In a retrospective study the case sheets of 42 neonates are examined. They were infants with oesophageal atresias, small and large intestinal atresias, exomphalos and gastroschisis, diaphragmatic hernia, intestinal obstruction as well as two children with a ruptured spleen secondary to birth trauma. Various laboratory investigations such as the pH, pCO2, standard bicarbonate and serum electrolytes as well as the child's weight on admission and, if possible, the birth weight were evaluated. There was no definite relationship between the preoperative laboratory values and the chances of survival of surgical neonates except if the values were extreme. Infections and septicaemia were the most important causes of postoperative complications.

[Computer assisted heart and circulation analysis in experimental septic shock (author's transl)].

Developping a model of shock applying to the clinical manifestation is necessary to investigate the pathogenesis of septic shock and to identify the letality causing organ. Perhaps the hyperkinetic syndrome may play an important role. The extensive experimental data is processed by a digital computer. Explanations are given for the experimental apparatus and the structure of the Fortran-program.

[Contractility of the left ventricle in experimental septicemia].

In experimental septicemia a decrease of myocardial contractility could be demonstrated. Periodically a correlation between disturbance of contractility and endotoxinemia exists. Starting point of infection and type of bacteria were of no detectable influence. As far as the disturbance of contractility is concerned one must take into consideration the cardiotoxic effect of endotoxin, the influence of shock mediators, lacticemia as well as the decrease of coronary perfusion pressure.

[Local radiotherapy combined with systemic chemotherapy in 3 children with osteosarcoma of an extremity].

Three boys with osteosarcoma of tibia or femur were locally irradiated with 97.5-100 Gy. Additionally they got chemotherapy according to T4-protocol. After 14-26 month two of them relapsed within, the third below the radiation field. There were no serious side effects of radiotherapy. In spite of amputation and chemotherapy they all developed metastases of lung within 18, 19 or 81 months. Two of them died and one is living with disease. We were not able to cure our three patients by combination of radiotherapy and chemotherapy. Radiotherapy is to date no adequate alternative to radically surgery. Only extraordinary cases should be treated by irradiation.
