Macrolide resistance in Streptococcus pyogenes isolates from throat infections in the region of Aachen, Germany.

Macrolide-resistance was assessed in 216 consecutive Streptococcus pyogenes isolates from throat infections in the region of Aachen, Germany. Seventeen isolates were resistant to erythromycin: 12 isolates revealed a macrolide (M) phenotype and harbored mefA, and five strains expressed an inducible macrolide-lincosamide-streptogramin B (MLSB) phenotype of which four strains harbored ermA(TR) and one strain contained ermB(AM). Telithromycin (HMR 3647) and quinupristin/dalfopristin remained active particularly against the ermA(TR)-containing S. pyogenes isolates studied. Random amplified polymorphic DNA analysis identified multiple clones among erythromycin-resistant strains, but did not discriminate beyond the emm-type. mefA was present in three isolates either with emm2, emm12, or emm75, and in nine isolates with emm4. All four strains with ermA(TR) contained emm77, and the single strain with ermB(AM) harbored emm1. Despite the relative low rate of macrolide-resistance, these data suggest that at least three different macrolide-resistance determinants are prevalent in Germany and that mefA has spread rapidly into multiple clones of S. pyogenes.

Anti-Bacterial Agents↗

[Mycobacterium simiae in Germany].

Mycobacterium simiae has been isolated from a patient with a pulmonary infection. During 1 1/2 years 10 different isolates of this strain could be cultivated. In this case it has not been proved that Mycobacterium simiae was the etiological agent, although the probability is very high.

[Nitrous oxide in the working environment of anaesthetic personnel. A gas chromatographic analysis (author's transl)].

Nitrous oxide as well as halothane is hazardous for organ dysfunction in chronically exposed man. It is necessary for each anaesthetist, in cooperation with the hospital authorities, to take care to eliminate overflow not only of halothane but also of nitrous oxide from anaesthetia locations. Using gas chromatography, we studied the concentration of nitrous oxide during routine daily work in the environment of anaesthetic personnel. With our method of discharging anaesthetic gases, we were able to reduce the concentration of nitrous oxide.

Air Pollutants↗

[Measurements of cuff pressure of different low pressure cuffed endotracheal tubes under mechanical ventilation (author's transl)].

Three different low pressure cuffed endotracheal tubes (Lanz, Portex soft-seal, Kamen-Wilkinson) were studied in cadaver tracheas and patients in our intensive care unit during ventilation. In the air filled low pressure cuffs without controlled pressure system the cuff pressure and the ventilation pressure are almost identical. Rises of intrathoracic pressure during mechanical ventilation are reflected by increased cuff pressure. Tubes with controlled pressure system or foam-filled cuffs have lower peak cuff pressures, especially during sighing, coughing and suctioning.

Airway Resistance↗

[Control of respirator pressure and drainage of the anesthetic gases with Kuhn's device in pediatric anesthesia (author's transl)].

By adding a manometer and a pressure valve to the anaesthetic equipment used for newborns and children (Ayre-T-piece, modified by Kuhn) we are able to control airway pressures continuously and to draw out all narcotic gases from the operating room. The concentration of halothanee thus can be diminished to a very low level around the aneasthesist and the surgeon.

Air Pollution↗

[The concentrations of halothane in the operating theatre and its dependance on various methods of removal (author's transl)].

Measurement of halothane concentrations by means of gas chromatography was used to compare the efficiency of 1) a filtration system and 2) ducting of gases into the exhaust of the ventilation system of our operating threatre. The concentrations were found to be lower when the latter method was used. Measurements made subsequently provided data on halothane concentrations occurring during routine service when different methods of anaesthesia were used, with and without ducting of gases into the ventilation exhaust system.

[Limitation of prolonged nasotracheal intubation (author's transl)].

The debate concerning the relative merits of about prolonged nasal intubation or tracheostomy on adult patients continues. We had only very positive experiences in our intensive care unit describing 7 patients in whom the average duration of nasal intubation was 28,7 days (Range 26 to 33 days). All patients had a regularly weekly bronchoscopy to avoid any damage to the larynx or trachea.

[Preoperative skin disinfection in interventions on the hip joint].

Gram-positive cocci in particular, mainly Staphylococcus aureus and Staphylococcus epidermis, can be identified on patients' skin as facultative causes of wound bed infection. Disinfection the evening prior to surgery causes a statistically significant reduction in the number of skin organisms. The only explanation of the increase in the number of such organisms in isolated patients following disinfection the evening prior to surgery, and also after preoperative disinfection, is recontamination of the disinfected skin areas. Covering them with sterile surgical trousers and adhesive foils does not provide sure protection from recontamination. Studies of this kind may enable specific hospitals to establish a perioperative antibiotic prophylaxis for their own spectrum of micro-organisms.
