The value of hyaluronidase treatment of different tissues before demonstration of fibronectin by the indirect immunoperoxidase technique.

The influence of testicular hyaluronidase treatment on the immunohistochemical localization of fibronectin in different tissues (human articular cartilage, large intestine, synovial membrane and experimental granulation tissue) as well on frozen as on formaldehyde fixed, paraffin embedded tissue, has been studied using the indirect immunoperoxidase technique. Pretreatment with hyaluronidase is essential in demonstrating fibronectin in frozen sections of human articular cartilage. In the other tissues examined treatment with hyaluronidase was not essential, but gave a more optimal staining quality. The effect of hyaluronidase treatment was to some extent dependent on the duration of treatment. In formaldehyde fixed, paraffin processed tissue the improvement with hyaluronidase treatment was only seen when the hyaluronidase followed pepsin digestion of the deparaffinized tissue sections.

Demonstration of fibronectin in human articular cartilage by an indirect immunoperoxidase technique.

Fresh frozen tissue sections of human articular cartilage was treated without and with human testicular hyaluronidase (2 x 10(6) units/l) for 60 min at 37 degrees C and stained by the indirect immunoperoxidase technique with rabbit antihuman fibronectin. The rabbit antihuman fibronectin was purified by affinity chromatography on human fibronectin-Sepharose. Fibronectin was only found on the acellular surface of the articular cartilage in tissue sections not treated with hyaluronidase. In this surface layer, probably identical to "lamina splendens", the arrangement of fibronectin was as a membrane. No collagen was seen in this area by van Gieson staining. No staining for fibronectin was found in the cartilage matrix or in the chondrocytes. Treatment of the cartilage tissue with hyaluronidase resulted in visualization of high amount of fibronectin in the cartilage matrix, with the highest intensity around the chondrocytes. The staining of the acellular surface layer of the articular cartilage was identical with the results obtained without hyaluronidase treatment. These results indicate that articular cartilage is rich in fibronectin probably in complex with hyaluronic acid, and that the chondrocytes produce fibronectin in situ. It also demonstrates the steric hindrance of hyaluronic acid aggregates in diffusion of the antibody and the value of hyaluronidase treatment of tissue before demonstration of fibronectin.

The effect of D-penicillamine and methylprednisolone on the morphology of experimental granulation tissue in rats.

The effects of D-penicillamine and methylprednisolone on the formation of granulation tissue was studied in rats with Visella-sponge induced granulation tissue after 42 days of treatment. D-penicillamine in doses of 20 mg-, 100 mg-, 500 mg/kg/day caused no morphological differences compared with the control groups as regards ingrowth of granulation tissue and its content of collagen fibres, inflammatory cells, vessels and glycosaminoglycans. Treatment with methylprednisolone, 0.2 mg-, 2.0 mg-, and 5.0 mg/kg/day caused a dose dependent reduction in the ingrowth of granulation tissue. Methylprednisolone caused a reduction in the number of fibroblasts and collagen fibres, but no differences in the content of glycosaminoglycans could be demonstrated between the treated and the control groups. Granulation tissue produced during combination treatment with D-penicillamine 100 mg and methylprednisolone 2.0 mg/kg/day did not differ from that observed after methylprednisolone treatment alone. Thus, only high doses of methylprednisolone gave rise to an antiinflammatory action in this morphological study on granulation tissue, whereas D-penicillamine had no effect within the present dose interval. The quantitative alterations are in agreement with parallel biochemical analyses. However, the inhibitory effect of D-penicillamine on collagen crosslinking was not associated with light microscopic changes in the ultrastructure of the collagen fibres.

Fibronectin in experimental granulation tissue.

The temporal appearance of fibronectin in experimental granulation tissue has been studied using the immunoperoxidase technique on material fixed in formaldehyde, embedded in paraffin and pretreated with pepsin. Furthermore, the relationship between the distribution of fibronectin and connective tissue fibres, demonstrated as either argyrophilic or red by the van Gieson method, has been investigated. Fibronectin was demonstrated as part of the fibrinous network in the sponge before the formation of granulation tissue. In the development of granulation tissue fibronectin was seen in the initial cellular stage codistributed with the newly formed fibres, first the argyrophilic fibres and later with those stained red by the van Gieson method, too. With maturation of the granulation tissue, predominated by these fibres, the amount of fibronectin was gradually reduced. The presence of fibronectin in the sponge already before the formation of granulation tissue suggests that fibronectin in the initial stage of inflammation derives from plasma. The increase in amount of fibronectin corresponding to the cellular (fibroblastic) proliferation and the formation of fibres, both argyrophilic and van Gieson positive, indicates that fibronectin in young granulation tissue is synthesized by fibroblastic cells. The relationship between the morphologically demonstrated argyrophilic fibres and van Gieson positive fibres and the biochemical type III and type I collagens is discussed.

The influence of fixation and tissue preparation on the immunohistochemical demonstration of fibronectin in human tissue.

The influence of fixation and tissue preparation on the immunohistochemical localization of human fibronectin in gastrointestinal tract tissue has been examined using indirect immunoperoxidase technique. The most optimal staining result with strong intensity and well defined localization was obtained on frozen sections of unfixed material. Nearly identical results with improved morphology were obtained when staining paraffin sections of tissue fixed in 96% ethanol, 96% + 1% acetic acid and absolute acetone. All other fixatives tested, 10% neutral buffered formalin. Lillie's AAF, Bouin's fixative, Clarke's fixative, 4% formaldehyde, 4% formaldehyde + 0.5% cetylpyridiniumchloride (F-CPC), 4% formaldehyde +0.1% glutaraldehyde gave unsatisfactory results. However, proteolytic digestion with pepsin of paraffin sections prior to staining of buffered formalin and F-CPCfixed material gave results comparable with those obtained on unfixed frozen sections are regards definition of the staining whereas staining intensity was decreased in some degree. No improvement was observed when using proteolytic digestion of tissue fixed in other fixatives.

Proteoglycans, DNA, and RNA in rat granulation tissue, skin, and aorta. Biochemical and histological studies.

Proteoglycans in skin and aorta and nucleic acids in skin were studied in rats with subcutaneous sponge implants and in unoperated rats and compared with the sponge induced granulation tissue during a 42 day period. In rat aorta there was a predominance of the sulphated glycosaminoglycans, in particular heparan sulphate, while in skin there was a predominance of hyaluronic acid and dermatan sulphate. The subcutaneous sponge implantation caused a decrease in the dry weights of skin and aorta but did not influence the concentrations of the measured variables. In aorta the concentrations of chondroitin sulphate and dermatan sulphate decreased with age while in skin there was a slight fall in the concentrations of hyaluronic acid and heparan sulphate with age. The rate of biosynthesis of proteoglycans as estimated from the 35S-sulphate uptake was higher in skin than in aorta. A sequential development was noticed in granulation tissue with an initial high content of hyaluronic acid followed by increasing amounts of chondroitin sulphate and dermatan sulphate in older granulation tissue. The RNA/DNA ratio reached a maximum in 14 day old granulation tissue, but was unchanged in skin throughout the 42 day period. It is concluded that the distribution of proteoglycans and nucleic acids differs from tissue to tissue. This may be of relevance for tissue differences in susceptibility to diseases and to the effects - and side-effects - of drugs. The morphological and biochemical development of sponge induced granulation tissue from rats was similar to other types of granulation tissue during the first 4 weeks.

Latent prostatic cancer in a consecutive autopsy series.

In a consecutive series of autopsies, 223 prostates were removed. Latent prostatic cancer (LPC) was found in 50 cases (22%). All the tumours were adenocarcinomas. The frequency increased with age. No conclusive evidence was found of a relationship between prostatic hyperplasia and LPC. The extent of the tumour seemed to have greater prognostic significance than the degree of differentiation.

Formation of granulation tissue in subcutaneously implanted sponges in rats. A comparison between granulation tissue developed in viscose cellulose sponges (Visella) and in polyvinyl alcohol sponges (Ivalon).

A comparison was made between the granulation tissue formation in two different synthetic sponge types. Visella and Ivalon, of different sizes. The granulation tissue formed in the two sponge types did not differ qualitatively, and had the character of wound tissue and inflammatory tissue in man. The rate of tissue formation in the Visella sponges was faster and the tissue was more homogenous than in the Ivalon sponges. Fourteen-day-old Visella implants of either size contained more granulation tissue than Ivalon sponges, probably owing to the smaller pore size of the former material. This may also account for the more frequent occurrence of giant cells in the Visella implants. In contrast to the Visella sponges, the trabeculae of the Ivalon polymer showed calcification and positive staining properties with histological staining procedures, and deformation was frequent among the Ivalon implants. Thin sponges of either type closed in about 21 days, thick ones after about 42 days of implantation. Calculated per 2 cm3 of implant, thin sponges produced more tissue after 14 days of implantation than thick noes. It is concluded that the Visella sponge type is best suitable for this experimental model of inflammation.

Prognostic significance of p53, Her-2, and EGFR overexpression in borderline and epithelial ovarian cancer.

The objective of the study was to evaluate the prognostic effect of p53, Her-2, and EGFR in borderline and epithelial ovarian cancer. Tumor tissue from 85 patients with borderline and 783 patients with epithelial ovarian cancer stage I-IV were analyzed immunohistochemically for p53 positivity and over-expression of Her-2 and EGFR. In the ovarian cancer (OC) group 415 patients (53%) had p53-positive tumors, 272 (35%) had tumors with Her-2 over-expression, and 483 (62%) had over-expression of EGFR. In the OC group the classical prognostic factors (older age, higher FIGO stage, and poorer differentiated stage) had significant prognostic value in both uni- and multivariate analyses. Multivariate analyses in the OC group proved p53 positivity to increase mortality significantly depending on the grade of the tumor. Her-2 likewise increased the risk of mortality significantly in this group depending on the grade of the tumor. EGFR on the other hand did not have any additional prognostic effect in the OC group after adjustment for the classical prognostic and molecular factors was made. In the borderline group Her-2 and EGFR over-expression in combination, adjusted for age and p53, significantly improved the prognosis.

Analysis of cervical smears obtained within three years of the diagnosis of invasive cervical cancer.

The cytologic histories of 376 women presenting with invasive carcinoma of the cervix were analyzed. In total, 202 (53.7%) of these women had had 355 smears taken during the three years preceding presentation. All 320 smears with an original cytologic diagnosis of less than cancer were reviewed. The original cytologic diagnosis was low in 95 (30.6%) of 310 adequate smears. Originally, 96 (30.9%) of the adequate smears were evaluated as negative; at review, only 55 (17.5%) of the adequate smears were evaluated as negative. Comparing the review diagnoses to the 355 total smears, the rates of negative smears were 13.5% (42 of 310) for squamous-cell carcinoma, 30.0% (12 of 40) for adenocarcinoma and 20.0% (1 of 5) for adenosquamous carcinoma (P less than .05). The cellular composition of the smear was significantly related to the cytologic detection of abnormalities: endocervical cylindrical and/or metaplastic cells were seen in only 45.5% of smears diagnosed as negative, but in 84.4% and 97.8% of smears diagnosed as atypia and cervical intraepithelial neoplasia, respectively P less than .00001). Smears without endocervical cells should be considered inadequate and should be repeated.

Efficacy of the cytobrush versus the cotton swab in the collection of endocervical cells.

Cervical smears should contain endocervical cells to be accepted as adequate for a cytologic diagnosis. Before this study was undertaken, one-third of the smears received in the Cytology Laboratory of Odense University Hospital were inadequate. In an attempt to increase the rate of adequate smears, a randomized study was undertaken to compare the efficacy of the combined spatula-Cytobrush method to that of the spatula-cotton swab method traditionally used by doctors in Denmark. The incidence of smears containing cylindrical and/or metaplastic cells was 89% by the spatula-Cytobrush method as compared to 62% by the spatula-cotton swab method (P less than .001). There were large differences between the rates of adequate smears from the various doctors when using the spatula-cotton swab method (range, 14% to 82%); these differences were far less when using the spatula-Cytobrush method (range, 75% to 100%). A large-scale application of the spatula-Cytobrush method should result in fewer repeat smears required and fewer false-negative smears.
