Loneliness correlates in advanced alcohol abusers. I. Social factors and needs.

A previous report on 34 alcohol abusers undergoing rehabilitation revealed that, among 86 medical, social and psychological variables, "feelings of loneliness" were most important for the prognosis. The current analysis was performed in order to illuminate the less satisfactory prerequisites regarding external social factors and needs of the lonely alcohol abuser. Ninety-five abusers, the previously mentioned sample included, were examined twice with an approximate interval of two years. The results indicated that there were no simple or obvious correlations to the external social situation (the amount of contacts, social network, etc). Feelings of loneliness were more closely associated with dissatisfaction with the quality of existing relationships, independent of quantity. Furthermore, even in relation to work and activities, the lonely abusers showed a general dissatisfaction with the existing situation, independent of its nature. In addition, the lonely abusers felt dissatisfied in a number of need domains, although they were only inclined to stress the priority of some social needs. To sum up, the lonely abuser showed a recurrent pattern characterized by discontentment and dissatisfaction, in combination with some traces of passivity in essential life-spheres.

Enprostil and cimetidine: comparative efficacy and safety in patients with duodenal ulcer.

The safety and efficacy of enprostil, 35 micrograms twice daily, and of cimetidine, 400 mg twice daily, in the treatment of duodenal ulcers were compared in a randomized, double-blind, parallel, multiclinic study. Endoscopy was performed before treatment and at 2-week intervals for 6 weeks or until the ulcer healed. Patients recorded their drug compliance, antacid use, ulcer symptoms, and adverse experiences daily. One hundred and six patients entered the trial, of which 104 were eligible for the initial endoscopy analysis. Base-line characteristics were similar in the two treatment groups. The cumulative healing rates in the enprostil group were 56%, 86%, and 92% at 2, 4, and 6 weeks, respectively, and those in the cimetidine group were 53%, 84%, and 90% (NS). The healing rates for nonsmokers at 6 weeks were 96% in the enprostil group and 97% in the cimetidine group, which were significantly greater than those for smokers--88% and 81%, respectively. There were no significant differences in the duration, severity, or frequency of daytime or nighttime pain between the groups. Seventeen of the enprostil patients (32%) reported 21 adverse experiences during the trial, and 20 of the cimetidine patients (39%) reported 23 adverse experiences. No patients withdrew because of adverse experiences. The two drugs were similarly safe and effective in the treatment of duodenal ulcer.

Subunit exchange between human and dog prealbumins.

When human prealbumin is mixed with carb,mylated human prealbumin or dog prealbumin hybrid prealbumins containing subunits of both types are formed. Spontaneous hybrid formation was a slow process. By renaturation from 6 mol/l quanidine hydrochloride, hybrids, distributed according to random combination of subunits were formed. The hybrids were stable, had a molecular weight similar to their parent molecules and bound thyroxine and retinol-binding protein. Only the hybrids with human and dog prealbumins participated in thioldisulphide exchange reactions as their parent molecules. The ability of prealbumin to exchange subunits of low molecular weight may be the reason why prealbumin concentration in cerebrospinal fluid is higher than expected from its plasma concentration and molecular weight.

Retinol-binding protein from human urine and its interaction with retinol and prealbumin.

In freshly collected urine from a patient with glomerulotubular proteinuria there were two bands which contained retinol-binding proteins. The cathodal band showed fluorescence in the ultraviolet. After extraction with organic solvents only the anodal non-fluorescent band remained. After addition of an excess retinol only one band remained which by mobility corresponded to the cathodal band. The anodal of the two bands was therefore probably the apo form and the cathodal the holo form of the same retinol-binding protein. Their proportions, determined by densitometric scanning were approximately 4/1 (anodal/cathodal band). More than 85% of the retinol-binding protein in the urine bound to prealbumin-Sephrose. The apo retinol-binding protein from urine had the same electrophoretic mobility on agarose gel el-ctrophoresis and the same pattern on isoelectric focusing as an retinol-binding protein prepared from serum. The carboxy-terminal amino acid sequence of the retinol-binding protein from freshly collected urine that bound to prealbumin-Sepharose, was -Arg-Leu. The amino-terminal sequence was Glu-Arg-Asp-Cys-Arg-Val-Ser-X-Phe-Arg-Val-Lys-Glu-Asn-Phe-Asp-Lys-Ala-Arg-Phe-X-Gly-Thr-Trp-Tyr-. This sequence and the amino acid composition are compatible with the view that the retinol-binding protein in urine is the same as in plasma.

Interaction between prealbumin and retinol-binding protein studied by affinity chromatography, gel filtration and two-phase partition.

The interaction between prealbumin and apo or holo retinol-binding proteins has been studied by affinity chromatography, gel filtration and two-phase partition. At physiological ionic strength apo and holo retinol-binding protein form 1:1 molar complexes with prealbumin. Mean dissociation constants for the prealbumin compex with apo retinol-binding protein and holo retinol-binding protein with all-trans retinol, retinoic acid, retinal and retinyl acetate were calculated from the partition data as 0.33 +/- 0.11 x 10(-6) M and 0.075 +/- 0.015 x 10(-6) M respectively (mean +/- S.E.M.). The difference was statistically significant. Quantitative data on the amount of retinol, retinol-binding protein and prealbumin in plasma and urine were in good agreement with the ratio of the dissociation constants for the complexes of apo and holo retinol-binding proteins with prealbumin as determined in the partition experiment. The magnitude of the dissociation constants was compatible with previously published data on the turnover of retinol-binding protein.

Purification of retinol-binding protein from serum and urine by affinity chromatography.

A new method for the purification of retinol-binding protein from urine and serum is described. The method is based on the reversible binding of retinol-binding protein to retinoic acid linked to Sepharose and prealbumin linked to Sepharose. The yield is comparable to conventional methods using ion exchange and gel filtration and the product is an apo retinol-binding protein of high purity. The product has the same electrophoretic mobility and molecular weight and the same ability to interact with retinoic acid and prealbumin as retinol-binding protein prepared by conventional methods.
